2 SOLICITATION INFORMATION WASHINGTON SUBURBAN SANITARY COMMISSION CONTRACT NO. BT/BR6146A16 NEES LANE TRANSMISSION WATER MAIN REPLACEMENT Description of Project: Replacement of approximately 220 feet of 4 inch water main, 960 feet of 8 inch water main, 1145 feet of 10 inch water main, 1130 feet of 20 inch water main, and 2000 feet of water house connection to serve Nees Lane, Countryside Court, and Countryside Drive in the Silver Spring area of Montgomery County, Maryland. The expected value range for this contract: $1,000,001 to $2,500,000 Part IV, Page 2 of 62

3 CONTRACTOR QUALIFICATIONS A. Minimum Traffic Control Qualifications: Also see Specifications Section Bidder and/or their subcontractor(s) shall be responsible for traffic control work and shall demonstrate in the Bid the minimum qualifications to perform such. Requirements for minimum qualifications and Bid submittal are as follows: 1. Bidder and/or their subcontractor(s) must utilize in the contract at least one field individual as a Flagger and one field individual as a Traffic Manager, both certified by the State of Maryland (the ATSSA American Traffic Safety Services Association - Certification is recognized by SHA and accepted by WSSC to fulfil the requirement for Flagger) 2. Submit with the Bid the names of these individuals on company letterhead and a copy of each certification. B. Minimum Erosion and Sediment Control Qualifications: Also see Specifications Section Bidder and/or their subcontractor(s) shall be responsible for erosion and sediment control work and shall demonstrate in their Bid the minimum qualifications to perform such. Requirements for minimum qualifications and Bid submittal are as follows: 1. Bidder and/or their subcontractor(s) must utilize in the contract at least one supervisory field individual with a State of Maryland Erosion and Sediment Control Training and Certification. 2. Submit with the Bid the name and title of this individual on company letterhead and a copy of this certification. C. Minimum Plumbing Qualifications: Also see Specifications Section Bidder and/or their subcontractor(s) shall be responsible for pressure tapping cast iron, ductile iron and polyvinyl chloride water pipes and shall demonstrate in their Bid the minimum qualifications to perform such. Requirements for minimum qualifications and Bid submittal are as follows: 1. Bidder and/or their subcontractor(s) must utilize in the contract at least one field individual with a Tapping Certification and a field supervisor experienced in the performance of this work. Installer must have at least 3 projects in the last 3 years of similar size and type that they worked on. Field supervisor must have a minimum of 3 years in the performance of this work. 2. Submit with the Bid the resumes of the Installer and Field Supervisor and copy(ies) of the certification(s). 3. Resumes shall include, at a minimum, past employers for the past 5 years, 3 similar projects in the last 3 years with references, and names and telephone numbers of contact persons. 4. Any change in Installer and Field Supervisor shall require a resume to be submitted and approved in writing by WSSC prior to change. Part IV, Page 3 of 62

Minimum Chlorination and De-chlorination Qualifications: Also see Specifications Section Bidder and/or their subcontractor(s) shall be responsible for chlorination and de-chlorination of water pipes and shall demonstrate in their Bid the minimum qualifications to perform such. Requirements for minimum qualifications for chlorination and de-chlorination are as follows: 1. Bidder and/or their subcontractor(s) must utilize in the contract at least one Field Supervisory with 3 years of experience in chlorination and de-chlorination of pressurized water pipes using pressurized liquid chlorine as described in AWWA B301 or method described in AWWA C Submit with the Bid the resume of the Field Supervisory and copy(ies) of all certifications/licenses held by this individual. 3. Resume shall include, at a minimum, past employers for the past 5 years, 5 similar projects in the last 3 years with references, and names and telephone numbers of contact persons. 4. Prune roots and limbs as needed for machine access and maintenance/installation operations along Countryside Court; Countryside Drive East; Countryside Drive West; Nees Lane; and Pond View Road Nees Lane Remove one triple Red Maple and grind stump, Grind stump and associated flare roots (within 1 of trunk) to 12 depth below ground surface and remove grindings; fill hole with topsoil and level with existing grade; seed with grass and add straw mulch. Replace tree removed with one 1.25 to 1.75 caliper Okame Cherry; plant replacement tree 15 left WHC and 7 behind back of curb. * Minimum 48 hour notice should be provided to fronting property owners where trees are to be impacted. The following ISA standards for open cut trenching shall be used to avoid impact to roadside trees. (Please note: Condition #7) If open cut trenching is within standard distances noted, responsible person must contact Environmental Group for field inspection and possible amended tree authorization for removal. All work accomplished under this authorization shall comply with the Maryland Department of Natural Resources Roadside Tree Care Law. (COMAR or Natural Resources Article et seq.). 2. All work including trimming, root pruning, and removals shall be performed by a Licensed Tree Care Expert under the supervision of a Roadside Tree Care Expert. 3. Tree branch trimming shall be in such a manner so as not to cause damage to other parts of the tree or to other plants or property. Pruning of limbs must be in conformance with the ANSI A300 standards. 4. Roots one inch in diameter or greater shall be cut flush on the tree side of the trench. Excavation to expose roots for pruning must be performed by hand digging. 5. All stockpile fill or excavated material must be either placed on impermeable material or placed outside the dripline of the tree. 6. Tree removal is necessary to eliminate a hazard to public health, public safety, and /or property. If the expressed authorization is for tree care work other than removal and tree removal becomes necessary, then the WSSC or its representative is required to contact the Environmental Group at to obtain a revised authorization. 7. WSSC or its representative is required to consider alternative repair/construction methods that avoid open trenching and tree removal as a condition of this authorization. 8.
