1 UPPER SKAGIT INDIAN TRIBE COMMUNITY PLAZA WAY ASPHALT PAVEMENT OVERLAY ADDENDUM NO. 1 (7 pages total) Issue Date: May 11, 2016 General: Recitations of each and every section of the bid documents impacted by this addendum are not given. The described changes impact all relevant portions of the bid documents whether specifically cited below or not. PRE-BID MEETING ATTENDEES AND AGENDA: The list of attendees at the mandatory Pre-Bid Meeting has been posted to the project website and the attendees are noted on the Planholders List. The Pre-Bid Meeting Agenda is attached. BID FORM: An updated Bid Form is attached. Remove and replace the existing Bid Form (page 33). GENERAL CONTRACT CLARIFICATIONS: 1. Schedule of Construction (Specifications Page 67): Replace the third bullet point of Item 14. with the following: All work that impacts use of the facilities (milling, paving, striping) shall be scheduled only on Tuesdays or Thursdays to avoid impacts to the Tribal Administration Building. Work that would impact access to the Health Clinic must occur after 12:00pm (noon) to avoid conflicts with activities at the clinic. Work impacting the Health Clinic must be scheduled at least 2 weeks in advance to allow rescheduling of appointments. 2. Wheel Stops: Contractor shall remove all wheel stops in overlay area and re-install all wheel stops after paving. The only exception is that the wheel stops along the Administration Center Building shall be removed and replaced with extruded concrete curb, as shown on Sheet C2.1. The removed wheel stops from this area shall be stockpiled in the gravel area to the west of the Gym parking lot on Sheet C3.5 and ownership retained by the Tribe. Any broken/cracked wheel stops throughout the project shall be replaced by unbroken wheel stops from this area. Any broken/cracked wheel stops shall be disposed of off-site by the Contractor. It appears that three existing wheel stops are broken/cracked and will need to be disposed of. Wheel stops in paved areas shall be painted per Note 2, Sheet C2.2. Prior to painting, the wheel stops shall be pressure washed, dried, and prepared according to paint manufacturer s recommendations. There are 34 wheel stops in paved areas that need to be painted. Wheel stops in gravel areas do not need to be moved or painted. 3. Mill and fill / Dig-out Quantities: The Mill and fill and Dig-out estimated quantities are adjusted to reflect that the majority of the areas shown on the plans will be Mill and fill instead of Dig-out. The quantity of CSTC is also adjusted to reflect the decrease in estimated CSTC needed for Dig-outs. UPPER SKAGIT INDIAN TRIBE PAGE 1
2 4. HMA Quantity: The HMA estimated quantity is adjusted to 650 tons. See attached revised bid form. 5. Staging: Equipment and construction materials may be staged in the gravel parking area west of the Gym parking lot or another location on the reservation. Coordinate with Owner. 6. Who is responsible for disposing of excess soil from dig-outs, subsurface drains, and the concrete channel apron? The Contractor is responsible for disposing of excess soil at an approved site outside of the Reservation. See Section 2-09 Structure Excavation (page 105). SHEET C2.1: Demolish and remove the concrete approach to the Forestry Access Road between the Education & Employment Building and the Warehouse & Natural Resources Building. This concrete is roughly 12 ft by 5 ft in area. See photo below. Replace this paved area with HMA per Detail 1, Sheet C3.2 with the exception that instead of 22 of CSTC, the depth of CSTC will be dictated by the thickness of the demolished concrete. Payment for this concrete demolition is included in the Trimming and Cleanup bid item. Instead of adding 2 of CSTC to the full area of the gravel areas shown, install a 5 ft wide (min.) CSTC shoulder per Detail 1B, Sheet C3.1 at the gravel areas to provide a smooth transition. Coordinate with the Owner. The CSTC bid item quantity is adjusted accordingly to reflect this decreased quantity in the attached Revised Bid Form. SHEET C2.2: Add note: UPPER SKAGIT INDIAN TRIBE PAGE 2
3 5) While an attempt was made to show all existing striping on this sheet, Contractor is responsible for replacing all existing striping, whether shown on these Plans or not. Specifically, many of the NO PARKING markings in the area north of the Administration Center Building are not shown on these Plans. There are at least 9 existing NO PARKING markings that need to be replaced. SHEET C3.1, Detail 1, B: Are there many areas in which topsoil will be required? Topsoil shoulders will be required in areas that have an existing grass shoulder. All areas requiring topsoil will also require seed per the following specification. Bid items for topsoil and seed are added to the attached revised bid form Erosion Control and Water Pollution Control (2).GR (2)Seeding, Fertilizing and Mulching (2)B.GR (2)B Seeding and Fertilizing (2)B.INST1.GR8 Section (2)B is supplemented with the following: (2)B.OPT1.FR8 (December 4, 2006) Grass seed, of the following composition, proportion, and quality shall be applied at the rates shown below on all areas requiring roadside seeding within the project: Kind and Variety of Seed in Mixture by Common Name and (Botanical name) Pounds Pure Live Seed (PLS) Per Acre Nobility Perennial Ryegrass 105 Amazing GS Perennial Ryegrass 105 Longfellow II Chewings Fescue 70 Gibilbralter Creeping Red Fescue 70 Total Pounds PLS Per Acre 350 Seeds shall be certified Weed Free, indicating there are no noxious or nuisance weeds in the seed. (January 3, 2006) Fertilizer shall be a commercially prepared mix of and shall be applied at the rate of 10 pounds per 1000 square feet. SHEET C3.1, Detail 3: Is a grindout edge sufficient instead of needing a sawcut at limits of grindout area? If the edge of the grindout results in a clean, vertical edge for the new joint, this will be sufficient. But UPPER SKAGIT INDIAN TRIBE PAGE 3
4 if grinding results in a rough edge, it will need to be sawcut prior to placement of 2 of HMA. Cost of sawcutting is incidental to the project. SHEET C3.2, Detail 2: The trench for the 2 inch Sch 40 PVC shall be a minimum width of 12 inches for the solid PVC drain pipe between the existing catch basin and the perforated PVC drain. Replace 2 Sch 40 PVC with 4 Sch 80 PVC. Maintain 12 cover (min.) over all PVC drain pipe. SHEET C3.3: Add the following notes: 4) Contractor shall remove sod and vegetation within the fenced area where it is present. Remove dirt to a depth of 6 inches. Area of sod removal is roughly approximated at 900 square feet. This removed material must be disposed of off-site per Section 2-09 Structure Excavation (page 105). Payment for removing sod and vegetation is included in the Rip and Recompact bid item. 5) Contractor shall use all asphalt grindings from the project to fill and surface the Forestry Access Road. Asphalt grindings shall be compacted to form a smooth, even surface. Final cross-slopes shall be a minimum of 2% slope, and final longitudinal slopes shall be a minimum of 0.5%. Payment for installing asphalt grindings, grading and compacting is included in the Rip and Recompact bid item. 6) Contractor is to remove and stack all large rocks (rocks greater than 4 ) against the rockery retaining wall located along the east side of the access road. Payment for removing large rocks and placing in rockery is included in the Rip and Recompact bid item. SHEETS C3.4 and C3.5: Clarification for Note 1: Seal cracks at old patch joints even if less than 1/8 inch in width. Coordinate all crack sealing locations with the Owner. SHEET C3.6: Add the following note: 1) Replace the existing handicap symbol paint in the southernmost parking space with a thermoplastic handicap marking per WSDOT (2). The Community Plaza Way Asphalt Pavement Overlay Project consists of a 2 inch asphalt overlay over an existing paved parking area approximately 42,570 square feet in surface area. Existing pavement will be milled at match lines to existing pavement, sidewalks, and concrete slabs (total area of milling is approximately 7320 sq ft). Existing damaged asphalt will be dug-out or milled and filled per the details in the Plans. Dig-out / mill and fill areas will be as directed by the Tribe with a total area of roughly 2400 sq ft. 2 inches of crushed rock will be added to adjacent gravel parking areas and compacted with a total area of approximately 6450 sq ft. An adjacent access road will be ripped and recompacted to fix potholes and drainage. Existing structures in the asphalt such as valve boxes, manhole rims, and catch basin grates that are not in mill areas will be raised to match the new finish grade surface and grouted appropriately. Work includes removing all wheel stops during the work and replacing after the overlay, painting the replaced wheel stops, replacing existing striping and asphalt berms, replacing one catch basin, installing subsurface drains, installing extruded concrete curb, installing concrete channel apron, striping for Bedah Lane, crack sealing for the Tribe s cultural center and gym parking areas, and other work, all in accordance with the Contract Plans, the Contract Provisions, and the Standard Specifications. B. Restoration at all sites to pre-project conditions is also a condition of the work to be performed.
6 C. All bidders must be licensed contractors registered in the State of Washington, be registered on the federal System for Award Management website, and must not be on the Federal Debarment List. All work performed on this project will be subject to Davis-Bacon federal wage rates. 4. Schedule: Time for completion is defined as 30 working days after Notice to Proceed is issued. Completion Timeline: A start date of July 18, 2016 or later is specified. Coordinate start date and Notice to Proceed date with the Owner. In addition, all work must be completed prior to end-of-year 2016; however, Contractor is to consider the seasonal weather impacts and plan accordingly. Note scheduling constraints: All work that impacts use of the facilities (milling, paving, striping) shall be scheduled only on Tuesdays or Thursdays to avoid impacts to the Tribal Administration Building.
