1 ADDENDUM NO. 2 PROJECT: ARCHITECT: Omaha Nation Food Distribution Building Macy, Nebraska Project No. 7-7 PETERS AND ASSOCIATES, ARCHITECTS, P.C. 427 Dayton Circle, Suite Omaha, Nebraska Date of Issue: March 2, 208 Date of Bid: March 5, 208 NOTICE TO ALL PLANHOLDERS Please insert this Addendum into your copy of the Contract Documents. The following changes to the Contract Documents for the Omaha Nation Food Distribution Building, Macy, Nebraska, are issued by the Architect, and shall have the same force and effect as though part of the original issue. CHANGES TO THE SPECIFICATIONS. Provide a deduct alternate to delete the emergency generator, delete the automatic transfer switch, and additional items as called out in the revised Section 00, Unit Prices and Bid Alternates, attached. Revised bid form PROPOSAL FORM REVISED FOR GENERAL CONSTRUCTION is attached. 2. Section 283, Chain Link Fence. a Miscellaneous Materials, paragraph Delete the last sentence: Fabric shall be knuckled on the top and bottom selvage. b. Section 283, Chain Link Fence, 2.0 Materials, 2.0., item.3, change terminal post size from 3 ½ dia. to 3 O.D. Item.4, change gate post size from 4 ½ dia. To 4 O.D. 3. Section 4720, Cast Stone, 2.0 Products, 2.0 Architectural Cast Stone. Add the following paragraph: MarcStone shall be an approved manufacturer. 4. Section 2300, Cabinets and Countertops, 2.03 Countertops, Change paragraph to read: Countertops indicated as plastic laminate shall be fabricated of high density particle board cores, covered with high pressure plastic laminate. Provide standard plastic laminate with /2" radius, waterfall edge for all countertops, except at restroom lavatories." 5. Prior Approval Electrical:. For light fixtures, the following manufacturers are approved subject to shop drawing review: A. Type 5 Corelite B. Type 8 Vibia C. Type 0 McGraw-Edison D. Type 2 Phoenix CHANGES TO THE DRAWINGS. 3/A-2., Education Kitchen C. Hide the refrigerator blocking the left side of the drawing. Add note describing typical plastic laminate countertop edge as /2 radius waterfall edge (typ.). See attachment for revised elevation. 2. Wire racks shown on interior elevations /A-2.3, 2/A-2.3, 7/A-2.3, and 8/A-2.3 show (4) shelves. Shelves specified are for (5) shelves. Provide 63 vertical posts and (5) shelves per unit, typical, in lieu of the number of shelves shown on the interior elevations. 3. See attachment AD-2.2 for exterior hollow metal door jamb detail at brick wainscot. Detail occurs at door #04, and door #2B.

Exterior recessed can lights in roof overhang, not included in revision cloud, are shown as Type. These are to be Type as indicated on the original drawings. All other requirements of the drawings and specifications shall remain in effect. This Addendum shall be attached and made part of the Contract Documents. The Bidder acknowledges that he has received and familiarized himself with the following Contract Documents as prepared by PETERS AND ASSOCIATES, ARCHITECTS, P.C.: Drawings, entitled Omaha Nation Food Distribution Building including the following: Cover Sheet, C-., C-.2, C-.3, C-.4, C-.5, A-., A-.2, A-.3, A-2., A-2.2, A-2.3, A-3., A-3.2, A-3.3, A-6., A-7., A-7.2, A-7.3, A-7.4, A-7.5, S-., S-.2, S-.3, M-., M-2., M-3., M-4., M-5., M-5.2, E-., E-2., E-3., E-3.2, and E-4., dated February 2, 208. Specifications entitled Omaha Nation Food Distribution Building dated February 2, ADDENDA. The Bidder acknowledges receipt of Addendum through. 3. The undersigned, having familiarized himself with the Contract Documents, and the local conditions affecting the cost of the work at the place where the work is to be done, hereby proposes and agrees to perform everything required to be performed and to furnish all labor, materials, and equipment, and all utility and transportation services necessary to perform and complete all the work required as shown on the Contract Documents for the construction of the Omaha Nation Food Distribution Building located in Macy, Nebraska. 4. The Bidder agrees to hold his bid open for a period of Thirty (30) days after the Bid Date. 5. The Bidder agrees to enter into a contract with the Owner in the form as prescribed, if awarded on the basis of this bid, and to furnish a Performance and Labor and Material Payment Bond in accordance with the Specifications within ten (0) days after the award of contract. 6. Base Bid: The undersigned hereby proposes and agrees to perform the work for the sum of: Dollars ($ ). The amount shall be shown in both words and figures. In the case of a discrepancy, the amount shown in words shall govern. (Note: All Federal Excise taxes and State and local Sales Taxes are excluded from this proposal.) 7. Unit Price No. Under this unit price, identify the cost of all work associated with stabilization of unsuitable subgrade soils encountered below limits of overexcavation, using stabilization methods as specified in Section 2300 Earthwork of the Specifications. This Unit Price shall identify the cost of stabilization based on cubic yardage of unsuitable materials to be removed. Unit price shall be: Dollars ($ ) per cubic yard. BF-REV-

5 SECTION 00 UNIT PRICES AND BID ALTERNATES.0 GENERAL.0 The following Unit Price is included herein as part of the work which may or may not be included in the construction contract. The Contractor shall submit a bid for the Unit Price in the space provided on the Bid Form. Requirements for the Unit Price are included in the Specifications. 2.0 UNIT PRICE NO. 2.0 Under this unit price, identify the cost of all work associated with stabilization of unsuitable subgrade soils encountered below limits of overexcavation, and stabilization methods as specified in Section 2300 Earthwork of the Specifications. The depth of stabilization material needed, including crushed stone and the percentage of cement, will depend on the actual subgrade conditions encountered in the field at the time of construction, as determined by the Soils Engineer. This Unit Price shall identify the cost of stabilization based on cubic yardage of unsuitable materials to be removed. 3.0 ALTERNATE NO. 3.0 Provide a deduct alternate for the following: DRAWINGS MECHANICAL. Sheet M-. Mechanical Floor Plan A. Remove gas serving generator, shut-off valve, and sheet note Sheet M-3. Mechanical Details A. Gas Riser - Remove gas serving generator, and shut-off valve. B. Gas Riser Change total input MBH to 264. GENERAL ELECTRICAL. The following shall be changed: A. Emergency generator shall be deleted. Stub out conduit and cap to 3ft away from building, conduit size as indicated on drawings, for future installation of generator. B. Automatic Transfer Switch (ATS) shall be deleted and replaced with large junction box of equal size for future installation of ATS. C. Route Panel EM feeder through new junction box. Provide 0ft of coiled conductors in junction box. DRAWINGS ELECTRICAL. Sheet E. Lighting Plan: The following shall be changed: A. One () Type and two (2) Type 9 light fixtures on northeast façade of Vestibule 0 shall be provided with emergency battery packs. Mount emergency battery packs above accessible ceiling in Food Distribution Area 02. B. All interior lights shown as emergency/night lights shall be provided with emergency battery packs. C.
