THIS ADDENDUM FORMS A PART OF THE CONTRACT DOCUMENTS. THE FOLLOWING CONDITIONS TAKE PRECEDENCE OVER ANY CONFLICTING CONDITIONS IN THE DRAWINGS AND SPECIFICATIONS. (205) Terry Bouchillon, Parks & Recreation (205) General: Scope of Work: No verbal instructions by Owner or Consultants shall be considered binding unless issued by Addendum. No work shall be done on the job unless directed to do so by Architect of Record, Gary Ohlman, gary.ohlman@birminghamal.gov Bids: No Bid may be withdrawn for sixty (60) days, although bids may be withdrawn within 3 working days of bid date without penalty. Low bidder(s) will be asked to supply a complete breakdown of costs to Owner within 7 calendar days. Bid Form: Must use Form of Proposal Section to submit Bids. Current Birmingham Business License and Alabama General Contractor s License required. A copy of these licenses shall be attached to the Bid Proposal (Note the Birmingham Business License can be attached to the bid proposal after bids are received.) Contractor s license number must also be on the outside of the envelope containing the Bid Proposal. If additions or deductions are noted on the outside of the Proposal Envelope, the notation shall be signed by the person signing the Proposal Form, or an authorized representative of the Company. If notation is signed by an authorized representative, a letter designating this authorized person shall be attached to the Proposal. Notice to Proceed can take calendar days to be issued. After Council approval, the Contracts shall be sent to the successful Bidder. The Contractor shall have 15 calendar days to return the signed Contracts along with Performance and Payment Bonds, Insurance Certificates, and the signed Agency Agreement Form. If executed Contracts are not received back within this time period, the Contractor will be declared non-responsive and be subject to forfeiture of his Bid Security. 1

6 PRE-BID CONFERENCE AGENDA Site Conditions: Each Bidder must examine the documents and site conditions and satisfy himself as to existing conditions. Minority Participation: Comply with Birmingham Plan Construction Industry Program. Section of the Project Manual. Sales Tax Abatement: Refer to Section Contractor shall charge 10% sales tax on ALL MATERIALS NO EXCEPTIONS, including materials that will be withdrawn from stock. Materials shall be purchased using special Purchase Orders issued by the Finance Department. Invoices shall be to the City of Birmingham in care of the Contractor with no sales tax charged. Project duration: 60 calendar days from NTP. Notice to Proceed can take calendar days to be issued. Liquidated Damages: Damages will be assessed per Section paragraph Supervision: The Contractor will be responsible for having a full time superintendent on the job site at all times. Alternates: NONE Unit Prices; NONE Allowances: Include in the bid an allowance: of $ 5, to your Base Bid to be used solely at the Owner s discretion for contingencies. If unused, this money will be credited to the Owner at the end of the job via a Change Order. Comments and Questions 2

Electric unit heater manufacturer or mechanical contractor to provide options noted on the ELECTRIC UNIT HEATER schedule.
