POSITION DETAILS Position Title Purpose of the position Lifestyle Assistant Work collaboratively to enhance the quality of life for Residents. The Lifestyle Co-ordinator will nurture individual interests, maintain motivation and stimulation of residents, through relevant activities and personal support, maintaining close relationships with families and ensuring Extra Services are met. All positions contribute to the provision of quality and sustainable health services within the ethos of the philosophy, mission, values and ethics of St Vincent s Health and Aged Care. POSITION KEY SELECTION CRITERIA Qualifications Technical Skills Professional Registration/s required Personal Competencies Job Competencies Essential qualifications: Police Check or Blue Card. Relevant qualifications or experience in Residential Aged Care sector. Desirable qualifications: Intermediate computer skills using MS Office and Healthcare database programs. Essential technical skills: Desirable technical skills: Ability to maintain collaborative and effective relationships with multidisciplinary team members. Ability to work flexibly within the facility Ability to lead by example and problem solve. Effective interpersonal and communication skills, including written, verbal and computer literacy. Behaviour consistent with the mission and values. Appropriate problem solving/analytical abilities. Proven organisational and prioritising skills with the ability to remain focused in an environment of competing demands. Self-motivated and well organised, with the ability to work unsupervised. Ability to display a strong empathic demeanour Experience in contemporary dementia care and aged care National Criminal History Check Ability to provide efficient delivery of Lifestyle Coordination Services to clients and staff and adhere to facility processes with a professional attitude Demonstrated ability to achieve KPOs as described below.
2 Ability to provide services as part of a health care team that are client focused, efficient and prompt. Ability to accurately maintain facility databases Ability to utilise pager and telephone system. Ability to participate in education activities and to contribute to the development of self and team members. Ability to maintain a working knowledge of relevant policies/ procedures and use these in work practices. Demonstrated application of contemporary infection control standards and practices. Demonstrated application of safe practices (e.g. manual handling, hazard identification). Ability to make a contribution towards effective risk management. Ability to participate in quality improvement activities. POSITION KEY PERFORMANCE OBJECTIVES (KPOs) RESPONSIBILITIES OF THE ROLE All activities are focussed upon provision of a quality service to patient/residents/clients in an efficient and effective manner consistent with the Values. National Standards All practices are consistent with the National Safety and Quality Service, EQUiP National and relevant professional and ethical standards and guidelines.
