This workshop will focus on using data to identify cohorts of falls and recognize driving forces that contribute to those categories of falls. Since falls are often driven by different factors within organizations and service lines, workshop attendees will develop performance improvement plans that address recurring trends within your organizations. Application has been made to obtain approval for nursing and NCERS (NHA) continuing education credits for both days of education. Spring Education Series Objectives Demonstrate a data driven system-wide approach to strategically reducing fall categories that have common contributing factors Analyze falls data to identify trends leading to falls within the organization Practice designing performance improvement projects that will reduce falls targeted at the common contributing factors identified in the data analysis

Mimi McCahill This session will provide a strategic overview about the falls-related claims activity within Caring Communities and Peace Church Risk Retention Group (PCRRG), industry trends relating to falls, and proposed regulations illustrating why effective falls management practices will be an important initiative in meeting your organization s mission of care. In addition, we will describe the methods and goals of a quality assurance and performance improvement (QAPI) approach to falls management. Key concepts discussed include data collection, compilation, analysis, interpretation and reporting. Part Two features analyzing a non-aging services scenario to evaluate the quality of data and achieve more effective performance improvement results. Page 2

This includes data type and data field selection to assist in identifying root causes related to falls. During Part Two participants will use information provided in Part One to identify performance improvement opportunities through data trending and analysis of a falls database. Strategies such as interventions, measurement, monitoring and evaluation will be presented as elements in developing successful Performance Improvement Plans ( PIPs ) that lead to genuine improvement. Specific fall prevention and management process, outcome and balancing measures will be described. Change concepts related to championing a quality improvement PIP will be taught. Describe the difference between a rapid cycle PDSA pilot project and an organizational initiative Recognize the importance of frontline staff participation in PIP pilot projects Use data to develop one outcome and two process measures List three change concepts 9:15 AM 10:15 AM Moving From Trends to Root Causes In this interactive three part session the facilitators will use case studies to illustrate the process of exploring data trends to identify root causes. Audience members will participate in a guided exercise, then will break into small groups to complete a root cause analysis on falls case studies. 5 10:30 AM 12:00 PM Developing a Performance Improvement Plan ( PIP ) The facilitators will use case studies to illustrate the process of developing a PIP. Participants will participate in a guided exercise, then develop a performance improvement plan using previously identified root causes. Discuss the process of developing a PIP Investigate root causes to identify PIP focus areas Write a falls management PIP 12:00 PM 1:00 PM LUNCH 1:00 PM 3:00 PM Participant Exercises: Now It s Your Turn Team PIP Presentations and Discussion Representatives from each group will report on their PIP, including timeline, team members, interventions, PDSA cycles and measurement. Teams will have the opportunity to refine their PIP s using facilitator and audience feedback.
