1 975 Alloy Drive, Suite 201 Thunder Bay, ON P7B 5Z8 Tel: Fax: Toll Free: February 2017 Job Description JOB TITLE: REPORTS TO: JOBS REPORTING: Director of Sub-Region Planning and Integration VP Health System Strategy, Integration and Planning Planning Lead DIVISION/DEPARTMENT: Health System Strategy, Integration and Planning LOCATION: North West LHIN, Thunder Bay, Ontario POSITION SUMMARY: The Director, Sub-Region Planning and Integration directly reports to the Vice President Health System Strategy, Integration and Planning and provides leadership for the planning, integration, and performance management at the sub-region level. The Director is responsible for the administrative management of the sub-region operations in conjunction with the clinical lead. The Director acts as a resource to community stakeholders and the sub-region clinical lead in the assessment of community and population health needs and provides support to other LHIN program areas to identify and implement appropriate communication, stakeholder engagement methods and performance monitoring for the sub-region level. In addition, the Director will have specific portfolio management and is responsible for the management of complex planning projects, integration activities, performance and providing advice to the executive team and staff regarding planning models and research/best practices. MAJOR RESPONSIBILITIES: 1. Leads health system planning, integration, coordination, service innovation and quality improvement across health care service providers and leads the planning and implementation of major initiatives, specifically for a sub-region in alignment with the Ministry s strategic directions 2. Maintains an awareness of current issues in the health care system related to the Integrated Health Service Plan strategic priorities and key result areas with a particular emphasis on sub-region issues related to the implementation of change initiatives. 3. Analyzes sub-regional health care system to identify gaps and opportunities, initiating action to address provincial strategic priorities.

2 4. Responsible for participating in the negotiation and allocation of volumes for Quality Based Procedures and Wait Times, for the sub-region. 5. Monitors the Quality Based Procedures and Wait Time allocations for the sub-region and associated performance indicators to support in-year reallocations and the identification of areas for performance improvement. 6. Coordinates and aligns with sub-regional Clinical and Home and Community Care Leads, providing leadership and direction for central resources supporting sub-regional activities. 7. Coordinates cross-functional teams of staff in the other divisions in order to comprise a virtual team to support sub regional functions. 8. Coordinates and conducts analysis of local health-care system based on provincial data, community input and other sources for purpose of identifying gaps and establishing planning and integration priorities. 9. In collaboration with the VP Health System Strategy Integration and Planning advances the strategic priorities of the overall division. 10. Works with staff to identify appropriate stakeholder management methodologies to meet planning requirements. 11. Assists with and coordinates the delivery of LHIN community events including planning and logistics at the sub-region and regional level. 12. Identifies, selects and uses appropriate planning approaches, such as: needs-based predictive models and community input, to be used in the development of Sub-Region work plans, service capacity assessments, Integrated Health Services Plan (IHSP) and other related health capacity planning. 13. Promotes high-quality research, develops planning methodology and provides leadership for actionable, measurable results-driven improvement strategies. 14. Works closely with the Director of Quality and Performance, Health Quality Clinical Lead and Clinical specialist to advance cross-sector Quality Improvement planning at the sub-region level that is aligned to the overall strategic directions and planning priorities of the NWLHIN. 15. Works closely with the VP Human Resources and Organizational Development, Health Force Ontario lead and other Directors to assess Primary Care and other Health Human resource capacity while also supporting recruitment efforts at the Sub-region level. 16. Conducts effective stakeholder consultation related to planning, assessed impact of planning recommendations and decisions, advances integration activities, and promotes consensus among stakeholders for action required. 17. Manages project plans. Provides advice and input from the planning and integration perspective to the development of health care system funding plans. 18. Develops innovative approaches for provider partnerships and selects among alternative approaches to achieve system coordination and integration. 2

Works with and engages staff, providers and stakeholders to facilitate the development of strategies and activities that improve access, coordinate care and promote integration at the sub-region level and regionally. 20. Participates in and contributes to provincial and inter-lhin planning and community engagement activities as required. 21. Provides group leadership to department planners/staff and manages their work and priorities during project assignments and manages project plans and budget allocations for planning, integration and coordination initiatives. 22. Provides and manages technical planning input towards the statistical analysis of health improvement initiatives. 23. Builds internal and external collaborations and partnerships to advance planning and integration outcomes. 24. Monitors and evaluates planning and community engagement outcomes. 25. Shares leading practices for coordination of planning or projects with other LHINs and participates in dialogue on community engagement methods and performance evaluation. 26. Provides advice and input from the planning and community engagement perspective to the development of health-care system plans for stakeholders, planning, and the senior leadership team. 27. Supports the Director of Communications and Engagement in developing content and related messages for the department. 28. Negotiates integration and coordination agreements with providers and stakeholders. 29. Active contributing member of the management team. 30. Other duties as required. Planning, Delivery and Evaluation: Drives organizational excellence by implementing leading practices and providing direction in the development of policies, procedures, systems and tools. KNOWLEDGE: 1. Knowledge of Ontario health sector and workforce trends within this sector. 2. In-depth development knowledge of local health issues, priorities and needs while recognizing the broader trends in health care policy and system development 3. Knowledge of the needs of Aboriginal peoples (urban and community) and Francophone communities. 4. In-depth understanding of the Ontario health-care system, its stakeholders, programs and issues. 5. French language an asset. EXPERIENCE: 4

Minimum 5-7 years of experience in Senior Management in health care or related field (or equivalent) in one or more of the following areas: health care planning, system planning, health care service delivery, health research, epidemiology, population health. 2. Demonstrated experience with facilitation and negotiation techniques and processes, including experience developing and facilitating large complex sessions involving diverse stakeholders. 3. Demonstrated experience working effectively in partnership with broad range of individuals and organizations of diverse backgrounds, preferably within the health care sector. 4. Skills and experience in event planning and implementation. 5. Expert knowledge of planning models and approaches. Demonstrated experience in community outreach, consultation or stakeholder management and familiarity with current theory and techniques of community engagement. 6. Excellent project management skills including budgeting, planning and communication. 7. Proven skill and success in large, complex planning projects. 8. Expert ability to select and apply multiple planning methodologies as appropriate. 9. Experience in a team environment 10. Valid driver s license. EDUCATION REQUIREMENTS: Post-secondary/ Master s degree in health administration, business administration, or related health discipline. Canadian Health Executive an asset NATURE AND SCOPE: 1. Interpersonal Contacts: - Internally: models exemplary interpersonal and communication skills with employees across the organization to lead and coordinate health system planning and integration. Works closely cross functionally with LHIN staff to identify opportunities for community consultation, planning and performance improvement. Must also communicate with employees throughout the organization to present, discuss information and problems related to undertaking the priority setting analysis and health planning functions. - Externally, must maintain relationships with individuals and organizations both from within the healthcare sector and across the broader community. Must also, communicate directly with various stakeholders to obtain and exchange the information required by the planning group to perform their analytical work, and gains consensus and agreement on the needs, priorities and integration plans and initiatives. Proven ability to communicate with senior leaders, clinicians, health service providers to obtain and exchange information as required by the 5

6 6 Ministry, LHIN reporting requirements and gains consensus and agreement to achieve LHIN Strategic goals and priorities. 2. Level of Responsibility: Responsible and accountable to support the Vice President Health System Strategy, Integration and Planning, the CEO and the Board for system transformation, modeling sustainable improvements in health care delivery, priority setting analysis, health system performance, contract and allocation. Decisions and actions have the potential to significantly impact the LHIN organization and the delivery of health care services in the Northwest. 3. Physical and Sensory Demands: Minimal demands typical of a senior position in an office environment. 4. Working Environment: Minimal exposure to disagreeable conditions typical of an office position exposed to stress and pressure associated with multiple priorities and deadlines. 5. Travel is required: Some demand on personal time for evening and weekend work. Periodic administrative relief/coverage for other senior administrators.
