Box 5666 Spartanburg, South Carolina Re: Asbestos Inspection Report 904 Beverly Drive, Spartanburg, SC CRE Project Number: IN Dear Mr. Reynolds: Crossroads Environmental, LLC (CRE) completed an asbestos inspection of the residential house structure, located at 780 River Street in Clifton, South Carolina on April 5, The inspection was performed by a SC-DHEC Licensed Asbestos Inspector, and in accordance with South Carolina Department of Health and Environmental Control (SC-DHEC) and Environmental Protection Agency (EPA) Requirements. A detailed summary table of the sampling is included in Attachment I; however, this report should be read in its entirety. Area Description/Scope of Work The house, located at 904 Beverly Drive in Spartanburg, South Carolina is a one-story brick structure with a sloped shingled roof. CRE performed limited sampling at the residence on August 15, 2016 in preparation for limited renovation activities. The determination was made to demolish the house rather than renovate; therefore, Spartanburg County requested that a comprehensive inspection of the house be performed. Inspection Strategy/Sampling Protocol The inspection consisted of grouping suspect asbestos containing materials into homogeneous areas based on the color and texture of the material, and then performing representative sampling of the materials included in those homogeneous areas. SC-DHEC has requirements for the minimum number of samples that can be collected from each homogeneous area (three samples of each miscellaneous material, three samples of each type of thermal system insulation, and the sample requirements 1258 Boiling Springs Road Spartanburg, SC Phone / Fax

Following completion of the on-site inspection/sampling, samples were submitted to an accredited laboratory for analysis. As of June 27, 2008, SC-DHEC requires that any non-friable organically bound (NOB) material that is suspect to contain asbestos, such as floor tile, mastics, roofing material, and caulking must be analyzed by transmission electron microscopy (TEM) if polarized light microscopy (PLM) analyses of that material indicate that no asbestos was detected. Results EPA recognizes a material as Asbestos Containing Material (ACM) if an asbestos content of greater than one percent asbestos is detected in a representative sample analyzed by polarized light microscopy. Results indicated that greater than one percent asbestos was detected in the drywall joint compound, ceiling surfacing, and kitchen/laundry room flooring. Relevant Regulatory Requirements/Recommendations Friability- Friable materials are defined as materials that can be reduced to powder by hand pressure. It should be noted that non-friable materials may become friable depending on the method of removal. All non-friable materials must be removed by properly accredited asbestos personnel and in accordance with all state and federal regulations. If the non-friable materials are removed in a friable manner, then all regulations in regard to friable abatement will apply, and the abatement must be performed by a SC-DHEC Licensed Asbestos Contractor. All ACM must be removed by a SC-DHEC Licensed Asbestos Abatement Contractor. Closing Statements and Limitations Attachment I includes a table with descriptions, results, and sample locations of the suspect materials. Attachment II includes a copy of the analytical results from the laboratory. Attachment III includes a sketch and/or photographs of the sample locations. Attachment IV includes a copy of the SC-DHEC Asbestos Inspector s License. Every effort was made to identify all materials in accessible areas. There is the possibility that suspect materials were not identified in inaccessible areas. 43 Limited Lead and Asbestos Inspection September 20, Beverly Drive, Spartanburg, SC Page 1 of 4 CRE Project Number: ILP SUMMARY Spartanburg County Community Development hired Crossroads Environmental, LLC (Crossroads) to perform testing for lead and asbestos on materials that will be affected by upcoming renovation activities. If lead and/or asbestos was detected in any of the components (vinyl siding, drywall, flooring, windows, etc.) that will be affected by the renovation, Crossroads was responsible for making recommendations to Spartanburg County Community Development as to how to perform the renovations in a safe manner that would prevent the homeowner and occupants from being exposed to asbestos and/or lead during the renovation activities. Spartanburg County Community Development provided the Crossroads inspector with a list of work to be performed at the 904 Beverly Drive home. Work that has the potential of impacting suspect asbestos-containing materials and/or lead-based paint includes the following: Exterior Replace vinyl siding Replace windows Re-shingle Paint exterior Paint doors Interior Drywall repair Paint entire interior Complete kitchen renovation Complete bathroom renovation Replace insulation Replace flooring Rewire Crossroads performed the requested testing on August 15, Asbestos was detected in the joint compound and ceiling surfacing located throughout the house. No lead-based paint was detected, with the exception of lead glaze on the tub in the master bathroom. Part I: IDENTIFYING INFORMATION/SITE DESCRIPTION Crossroads Environmental, LLC (Crossroads) was contracted by Spartanburg County Community Development to perform a Lead-Based Paint and Asbestos Hazard Assessment at the house located at 904 Beverly Drive, Spartanburg, South Carolina. The scope of work included testing all components that will be affected by renovation activities for lead and asbestos content. If lead and/or asbestos were detected,

The Inspection was performed by Kay H. Horton, SC Lead Inspector Certification No. SC- I , Exp. 10/14/2017; SC Lead Risk Assessor Certification No. SC-R , Exp. 10/31/2016; and SC-DHEC Asbestos Inspector License No. MP-23394, Exp. 2/09/2016. The house is a one-story ranch style brick house with a sloped shingled roof. The walls and ceilings are drywall, and the floors are covered with carpet or vinyl flooring. Part II: SAMPLING PROTOCOL Lead The inspection was performed using a Niton XLp 300 (Serial #: 20420) analyzer, which does not require substrate correction. Following proper calibration of the XRF, representative components were tested for lead content. The surface-by-surface inspection was performed according to EPA Guidelines. Following proper calibration of the XRF, the building components that will be disturbed during renovation were tested. The testing was performed in general conformance with Chapter 7: Lead-Based Paint Inspection of the U.S. Department of Housing and Urban Development s Guidelines for the Evaluation and Control of Lead-Based Paint in Housing (HUD Guidelines), and to specifications provided by Spartanburg County. A Risk Assessment was performed on any components painted with LBP that will be included in the renovation activities to determine if there is an immediate hazard. The Risk Assessment of those components was performed in general accordance with the U.S. Department of Housing and Urban Development s Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing (HUD Guidelines). The painted components were categorized by visual assessment as intact, fair, or poor based on the approximate surface area of deteriorated paint as outlined in the HUD Guidelines. In reviewing the lead results table, Table I., included in Appendix A, please note that the building side designations are as follow: Beverly Drive A D B C

The following suspect materials were identified and sampled: Drywall and joint compound Ceiling surfacing Samples of these materials were collected in accordance with the Environmental Protection Agency's (EPA) National Emissions Standards for Hazardous Air Pollutants (NESHAP 40 CFR 61 Subpart M). PART III: RESULTS Lead According to the Environmental Protection Agency (EPA), paint containing 1 mg/cm 2 of lead (by XRF) or 0.5% by weight (paint chip analysis) is considered lead-based paint (LBP). No lead-based paint was detected, with the exception of the lead glaze on the tub in the master bathroom. Asbestos The EPA and SC-DHEC recognize a material as Asbestos Containing Material (ACM) if an asbestos content of greater than one percent asbestos is detected in a representative sample analyzed by polarized light microscopy. Greater than one percent asbestos was detected in the joint compound and ceiling surfacing. PART IV: HAZARD CONTROL Lead No lead-based paint was detected; however, since the tub in the master bathroom contains lead, care should be taken to remove it without damage if it is replaced. Asbestos Due to the presence of asbestos-containing joint compound and ceiling surfacing, it is recommended that in addition to the contractor being an EPA Certified Lead Firm, that
