Delete: Prequalification requirements of ODOT. Form of Bid (Page 4 of 5). Add: Fifth: Traffic Delays ODOT (c) - Stopping or holding vehicles beyond the limits specified will inconvenience the traveling public and will be a cost to the Agency. (1) Traffic Delays Beyond 20 Minutes - It is impractical to determine the actual damages the Agency will sustain in the event traffic is stopped or held longer than the 20 minute limit listed in Therefore, the Contractor shall pay to the Agency, not as a penalty, but as liquidated damages, $500 per 20 minutes, or for a portion of 20 minutes, per lane, including shoulders for stopping or holding traffic longer than 20 minutes. In addition to the liquidated damages, any added cost for traffic control measures, including flagging, required to stop or hold traffic beyond the 20 minute time limit, will be at no additional cost to the Agency. The required traffic control measures will be as determined by the Engineer. Assessment of liquidated damages will be issued in writing and stop when the Engineer determines that traffic is no longer stopped or held beyond the 20 minute limit. ODOT is the approving agency/engineer for all TCP s submitted and traffic control requirements within the Crater Lake Hwy 62 right-of-way. Penalties will begin at 7:01 am. Detailed Specifications Section 1 (Page 1 of 33), Night Work. Add: Night work is required on this project. ODOT official night work hours are from 7pm to 7am. The Contractor shall notify, coordinate with, and obtain approval from ODOT for night work at least two (2) weeks in advance of the work. The Contractor shall plan, coordinate, and conduct the work so as to complete the roadway surface restoration with HMAC of all trench excavation by the end of each shift. Contractor to then form and place concrete median to ODOT Standards. Hwy 62 Trench Crossing Sawcut and AC Replacement Width: 72" For Bidding Purposes: 1.) Per ODOT Hwy 62 has 12" of existing AC paving from curb to curb. For Bidding Purposes: 2.) Per ODOT the existing PCC slab is 27' wide, 8" deep with variable depth AC cover, up to 12". The existing PCC base slab is approximately 10' south of the existing centerline of Crater Lake Hwy 62 to 37' south of said centerline.

We had our mandatory pre-bid meeting today for the installation of the proposed 14-inch water line along the south side of Hwy 62 near Hubbard s Hardware and Lava Lanes Bowling Alley. Below is a list of contractor questions that came up in this pre-bid meeting. I ve attached our plans (bid set) and our project schedule for your reference. Could you please review these questions, then give me a call so we can discuss them over the phone. Our bid opening is on March 25 th, and I d like to respond to these contractor questions as soon as possible, hopefully next Monday if possible. 1. Will the crossing work be required to be done at night? The Whittle Ave crossing can be accomplished during the day with flaggers and appropriate TCDs. The installation across Hwy 62 will not be allowed between the hours of 7am and 7pm (typical Hwy 62 lane restrictions). In both cases, as with all phases of construction, the Contractor should provide a comprehensive TCP (Traffic Control Plan) showing all TCDs and closures, including Flagging stations and lights for night work. 2. Will slurry be required in the trench backfill or crushed rock? Our experience with pipes under/crossing roadways has led ODOT to prefer Class E CLSM slurry backfill above the pipe compaction zone (See RD300 and RD302) vs. a ¾-0 aggregate fill. For those areas outside of the roadway, aggregate backfill is standard 1
8 practice. Please note that there is a rapid set CLSM commercially available to assist the Contractor in reducing the time of lane closures. 3. Will trench plates be allowed or will paving be required each day? Trench Plates would be allowed on the Whittle crossing but NOT on the Crater Lake Hwy crossing. Trench Plates are not allowed on State Highways that have pre-construction posted speeds greater than 35mph (see Std Specification Section , bullet number 14). Please note this restriction includes the Highway shoulders. 4. If paving is required, how does slurry backfill work? Hopefully the contractor will stage the work using a highearly material that sets within 1 hour. The Contractor will need to contractor their supplier for specific material performance guidelines to determine what length of pipe can be installed for each 12hour shift. 5. What is the thickness of a.c. paving replacement required? Please refer to Std Dwg RD Does ODOT have off-site easement ( construction easements) on the adjacent properties and will the contractor be allowed to use the easements? Proposed construction easements are not in place at this time. The Contractor will need to contact Roger Allemand to determine if any easements are valid prior to construction. 7. Will ODOT allow the southerly bike lane/shoulder/drop lane to be closed during construction (utilized for construction trucks) of the water main? Closing the shoulder/bike lane would require a comprehensive TCP showing the TCDs and would most likely only be allowed between the hours of 7pm and 7am. Note the Contractor would also be required to provide alternate routing for Pedestrians and Bicycle users, including devices and signing. Thanks, Rodney L. Grehn, P.E.
