1 JOB DESCRIPTION JOB TITLE: GRADE: REPORTS TO: Practitioner Young People (YP) subject to Job Evaluation Service Manager SPOC and YP DIRECT REPORTS: - LOCATION: to be confirmed Purpose of Job To ensure that young people reach their goals in line with service objectives To ensure young people reconnect with their community via transitions to jobs, housing, training, self employment To ensure young people build a stake in the community going forward. To maximise contact with other agencies who deal with young people, making the best use of sustainable support across the city, which will involve multi agency working with other statutory and voluntary services Duties and Responsibilities 1. To manage a caseload within required standards 2. To work with service user referrals in a way that is properly planned and carefully managed to ensure that needs are met and risks are managed appropriately and that service users are able to set and achieve realistic and achievable goals in line with contractual requirements and service objectives. 3. To work as part of the WISMS assertive outreach service Page 1 of 5

2 4. To undertake whole family assessments where appropriate and to develop integrated care packages using a whole family approach. 5. To work with young people in transition and to support the development of this work within the wider service 6. To develop and implement (where appropriate) intergenerational activities in helping the young person to achieve their goals 7. To support the development of family interventions within the wider service through case discussion, group supervision and proactive contributions to the development of the service model 8. To provide brief and longer term structured interventions, developing, implementing, and reviewing care plans 9. To establish and maintain effective links with professionals, individuals and organisations in building a network of support for young people eg schools, colleges, faith and community groups, future employers etc. 10. To use the ILLY information management system in line with service policies, procedures and guidelines 11. To develop and maintain young people involvement activities in line with best practice and cutting edge innovations 12. Represent and promote the work of the service, encouraging the active participation of service users, families and the wider community 13. Create and maintain relationships with funders, partners and external bodies to enhance the quality of the service, develop business and ensure that performance objectives are aligned to the appropriate bodies 14. Engage fully in regular supervision with your line manager. 15. Adhere to Health and Safety policies and procedures at all times and comply with legislation and statutory duties. 16. Adhere to Data Control and Information Sharing Protocol at all times. 17. Act in line with, promote and carry out all responsibilities with full regard to Equality and Diversity Policy. Page 2 of 5

Ensure individual expertise and subject area knowledge is up to date through Continuous Personal Development, including sharing good practice, engaging with training and acquiring / maintaining any individual qualifications required of the role. 19. As necessary, and in addition to the above, undertake other activities commensurate with the nature of the post. This job description may be reviewed and amended periodically in line with the requirements of the organisation. Specific objectives and accountabilities to be discussed and agreed with the line manager, and recorded in performance planning and review documentation. 4 PERSON SPECIFICATION Essential Experience Experience of delivering high performing services to young people and in conjunction with multi disciplinary agencies. Experience of working with young people in a recovery focused environment Experience of partnership working Demonstrable experience of generating and exploiting community based resources for the benefit of young people Experience of using intergenerational approaches and of working with young people in transition Experience of managing caseloads, one-to-one, and group work interventions with young people Knowledge Relevant professional qualification eg, youth work Knowledge of substance misuse recovery programmes and some or all of the following: the criminal justice system, education and employment, social housing and crime prevention. Knowledge of relevant client assessment and case management processes Sound understanding of safeguarding and child protection procedures and processes Sound understanding of the impact of social issues on young people and risk taking behaviour in adolescence Sound understanding of substance issues affecting age group and clinical impacts of such substances Knowledge of the work of the recovery service and how it integrates with partners, funders and external organisations. Knowledge of the legal requirements of the service in relation to its work with its service users. Knowledge of the support needs of multiple disadvantaged groups.
