1 HSSE Leadership Tour Checklist Purpose The purpose of this document is to provide Apache Managers/Senior Representatives with guidelines for undertaking Health, Safety, Security and Environmental (HSSE) Leadership tours. Furthermore, it is intended to provide a consistent approach to site visits. Introduction HSSE Leadership tours play an important role in underlining Apache s commitment to visible HSSE leadership. It is widely recognized by industry, professional institutions and the Health and Safety Agencies that management visibility at work locations has a direct effect on HSSE performance and development of culture. Furthermore, visible HSSE leadership is a key component of Apache s own behavioral framework and cultural development strategy. Scope This procedure is applicable to all sites and offices where Apache undertakes work. This includes, but is not limited to, locations where Apache employees and contractors are engaged in activities. This will include Apache-owned and operated assets and also sites owned and operated by third parties where Apache personnel, staff or contractors, are undertaking work or where Apache has an interest (e.g. construction yard, drilling rig, project office, asset owned and operated by another Operator in which Apache has an interest). Leadership HSSE tours are primarily aimed at Apache Management/Senior Representatives who are not based on a specific project. 1 Responsibilities 1.1 HSSE Manager/Site Manager The HSSE Manager/Site Manager (e.g. Facility Manager, Offshore Installation Manager, Apache Senior Representative on-site) for the location shall ensure that where necessary the visiting senior manager is accompanied during their visit to provide support and local knowledge. 1
2 1.2 Managers/Senior Representatives Each Apache Manager/Senior Representative undertaking a Leadership HSSE tour is responsible for producing a report outlining the observations during the visit and identifying where action or intervention is necessary. The Apache Manager/Senior Representative shall ensure that feedback is provided to the Site Manager at the location before leaving the site. A copy of completed forms (Refer to Addenda 1 and 2) should also be returned to the onshore Apache HSSE Specialist for logging and, if necessary, implementation and tracking of any actions within the Enablon Action Tracking Database. 2 Applying the Guidance 2.1 Creating a Plan Each Manager/Senior Representative is responsible for updating their own management visit schedule, informing the onshore Apache HSSE Specialist of locations, anticipated completion dates and so on. The Apache HSSE Specialist will update the annual Apache HSSE Plan audit schedule accordingly. As detailed in Paragraph 4.2 a copy of completed forms (refer to Addenda 1 and 2) should also be returned to the onshore Apache HSSE Specialist for logging and, if necessary, implementation and tracking of any actions within the Enablon Action Tracking Database. The Apache HSSE Specialist will update the annual Apache HSSE Plan audit schedule to confirm completion. 2.2 Planning a Plan HSSE leadership tours play an important role in demonstrating management visibility and commitment to HSSE. It is therefore vital to properly plan for a site visit to ensure maximum effectiveness. When agreeing the visit with the Site Manager please ensure you have communicated your objectives for the day and make requests to meet any specific groups in advance. Confirm that a will form part of your visit. Addendum 1 contains a checklist of things to do before you make your visit, during your visit and following completion. You can use this as an aide and check off as you progress through each part. 2.3 Conducting a Plan When you arrive at your location make sure you do an induction and familiarize yourself with the emergency and/or evacuation procedures. Test your knowledge with your host. Make sure you wear all appropriate PPE and follow local safety rules. 2
3 The checklist at Addendum 1 provides a list of prompts to consider during HSSE Leadership Tours. Apache s Expectation is that each Manager/Senior Representative set an example, both to Apache personnel and Contractors on the site. This means getting involved, asking plenty of questions and seeking clarity on how performance has been achieved at the location being visited, as well as giving an insight into how performance may be improved. Before leaving site a short debrief should be undertaken with the Site Representative/ Manager, summarizing the visit and any areas for improvement using the form in Addendum 2. Any positive aspects or best practice observations must also be highlighted. 2.4 Recording a Plan Leadership HSSE tours shall be recorded on the Leadership HSSE Report form (Addendum 2). This form must be used to record significant observations, positives as well as any weaknesses and actions agreed. The visit report shall be appended with all the relevant documentation raised during the visit, for example AIM for ZERO observation cards, toolbox talk records, etc. and forwarded to the onshore HSSE Specialist, who will update the Apache Annual Audit schedule and arrange for any resultant actions to be logged and tracked to completion within the Enablon Action Tracking Database. 3
