31 Cnfirming Accuracy f Hspice Sample Files Survey vendrs shuld cntact their hspice clients befre data cllectin begins if there are missing r incrrectly frmatted data in the file prvided by the hspice r if sample cunts d nt add-up If a hspice des nt submit a mnthly sample file t its survey vendr, the survey vendr must nt assume that there are zer survey-eligible decedents/caregivers fr the mnth Update: The hspice must cnfirm in writing that there are zer survey-eligible decedents/caregivers fr the mnth If zer survey-eligible decedents/caregivers is cnfirmed, survey vendrs shuld submit a Hspice Recrd t the CAHPS Hspice Survey Data Warehuse, including the sample size, the cunt f ineligibles due t n publicity and live discharge If there is nt cnfirmatin f zer survey-eligible decedents/caregivers, then a Hspice Recrd must nt be upladed. A Discrepancy Reprt fr this hspice must be submitted. 31

48 Quality Assurance: Telephne During telephne attempts, survey vendrs must: Update telephne infrmatin Check that entire sample has received telephne attempts fr each hspice client Review call attempts t cnfirm first attempt within first seven days f fielding perid and that all applicable cases receive five attempts Mnitr scheduled call backs t ensure attempt is made at requested time Mnitr interviewers fr accuracy Check that data are being captured crrectly 48
