1 Verificatin Summary Reprt (VSR) Instructins The verificatin prcess must be dne at the spnsr (district) level. Accessing the VSR Lg int IWAS; Click n System Listing n the left side f the screen; Click n Web-based Illinis Nutritin System (WINS); On the Spnsr Dashbard under Reprts, click n the NSLP Verificatin Summary Reprt link. If a field des nt apply, r there is n data t be entered, please enter 0 (zer). T return t any prir step, return t the Spnsr s WINS dashbard and click n the NSLP Verificatin Summary Reprt link under the Reprts heading, OR use the Previus buttn. All steps may be edited as lng as the Submit buttn has nt been clicked in Step 5. Step 1 APPLICATION Cunts as f Octber 2, 2017 Any numbers prvided in this sectin will be carried frward t the apprpriate sectin f Step 3 f the VSR. All SFAs with schls, sites, r Residential Child Care Institutins (RCCIs) cllecting husehld eligibility applicatins must reprt in this sectin. Reprt the number f SNAP/TANF, Fster, and Incme APPLICATIONS apprved as f Octber 2. Line 1: Enter the number f APPLICATIONS apprved as FREE eligible based n: A SNAP r TANF ID number being prvided n the applicatin; A cmbined SNAP/TANF and Fster child applicatin; OR an applicatin that nly has a Fster child being indicated. A cmbined Incme and Fster child applicatin shuld be reprted with the apprpriate FREE r REDUCED-PRICE incme applicatin infrmatin. Line 2: Enter the number f APPLICATIONS apprved as FREE eligible based n Incme infrmatin being prvided. Line 3: Enter the number f APPLICATIONS apprved as REDUCED-PRICE eligible based n Incme infrmatin being prvided. Line 4: Ttal number f applicatins. This bx aut-calculates based n data entered in n Lines 1, 2, and 3. Line 5: Enter the number f Errr-prne Incme applicatins frm all applicatins entered n Lines 2 and 3. Line 6: This bx aut-calculates based n the ttal number f applicatins listed n Line 4 If NO applicatins were cunted/cllected and included n lines 1 thrugh 3 abve; Or if verificatin will nt be perfrmed, did ALL sites perating the NSLP and/r SBP meet at least ne f the fllwing exemptins: Check N/A, if applicatins were cllected Check Yes if the verificatin prcess will nt be perfrmed due t meeting ne f the listed exemptins

2 Step 2 Directly Certified Students Any numbers prvided in this sectin will be carried frward t sectin 3 f Step 3 f the VSR. Are ALL sites listed belw exempt frm perfrming direct certificatin due t ALL sites being enrlled in CEP r ther Prvisin? Check YES, if all sites are enrlled in Cmmunity Eligibility Prvisin (CEP) Check N, if any site is nt enrlled in Cmmunity Eligibility Prvisin (CEP) If the spnsr/district is a mix f CEP and/r RCCI sites, ONLY fill ut direct certificatin infrmatin fr thse sites NOT perating CEP r that are NOT RCCIs. By schl/site, indicate the number f STUDENTS directly certified. Clumn 1: STUDENTS electrnically direct certified as receiving SNAP benefits. SNAP extensin f benefits students shuld als be included. Clumn 2: STUDENTS electrnically direct certified as receiving TANF, Medicaid, Fster, AND any students dcumented as being Hmeless, Migrant, Runaway, Head Start r Even Start. TANF and Medicaid extensin f benefits students shuld als be included. Clumn 3: STUDENTS receiving FREE meal benefits due t prviding an actual paper statement r dcument frm the SNAP agency shwing they receive SNAP benefits. If a field des nt apply, r there is n data t be entered, please enter 0 (zer). Standard VSR Sectins 1 thrugh 4 Step 3 Sectin 1 - ALL spnsrs r districts with schls, sites, r RCCIs perating the NSLP and/r SBP must cmplete this sectin regardless if all schls are exempt frm verificatin. Reprt the number schls, sites, r institutins perating the NSLP and/r SBP, and the number f students with access t the NSLP and/r SBP as f the last perating day in Octber. Line 1: Clumn A- TOTAL number f schls r sites (nt including RCCIs) perating the NSLP and/r SBP and Clumn B- TOTAL number f enrlled students at thse schls r sites with access t the NSLP and/r SBP. Line 2: Clumn A- TOTAL number f RCCIs perating the NSLP and/r SBP and Clumn B- TOTAL number f enrlled students at thse RCCIs with access t the NSLP and/r SBP in RCCIs. Line 3: Of the RCCIs reprted n Line 2; Clumn A- enter the number f RCCI site(s) with *DAY students and Clumn B- enter the number f ONLY the DAY students with access t the NSLP and/r SBP in RCCI site(s). *DAY students (NOT residents), are students wh s meal eligibility is determined based n an applicatin r direct certificatin as applicable

3 Line 4: Of the RCCIs reprted n Line 2; Clumn A- enter the number f RCCIs with NO DAY students and Clumn B- TOTAL number f residents/students. Line 5: This bx aut-calculates based n data entered n Lines 1 and 2, Clumn B. Sectin 2 - Only SFAs with sme r all schls, sites, and/r RCCIs perating under an alternative prvisin must cmplete this sectin. Fr RCCIs perating an alternate prvisin, include bth day and residential students. Reprt the number f students with access t the NSLP and/r SBP as f the last perating day in Octber. Line 6: Clumn A- enter the number f schls r sites perating the Cmmunity Eligibility Prvisin (CEP) and Clumn B- enter the number f enrlled students at thse CEP schls r sites with access t the NSLP and/r SBP. Sectin 3 - This sectin will aut-fill based n data entered in n Step 2. If changes need t be made t the directly certified student cunts, return t Step 1. If a field des nt apply, r there is n data t be entered, please enter 0 (zer). Sectin 4 - All SFAs with schls, sites, and/r RCCIs cllecting individual husehld applicatins must reprt this sectin, including schls and/r RCCIs in a Prvisin 2 r 3 base year. Clumn A, applicatin cunts will aut-fill based n the infrmatin prvided in Step 1. (If changes need t be made t the applicatin cunts, return t Step 1) Clumn B- Reprt the number f students as f the last perating day in Octber. Please be aware, any changes t the applicatin cunts may affect the riginal 3% sample size that was calculated. Line 11: Clumn B- Enter the number f students apprved as FREE eligible based SANP, TANF, r Fster infrmatin being prvided n an applicatin. Line 12: Clumn B- Enter the number f students apprved as FREE eligible based n Incme infrmatin being prvided n an applicatin. Line 13: Clumn B- Enter the number f students apprved as REDUCED-PRICE eligible based n Incme infrmatin being prvided n an applicatin. Line 14: This line aut-calculates based n data entered in n Lines 11, 12, and 13; Clumn A. Line 15: This line aut-calculates based n data entered in n Lines 11, 12, and 13; Clumn B. Sectin 5 Line 16 aut-calculates the ttal number f FREE and REDUCED-PRICE students in sectins 3 and 4, Lines 10 and 15.

Line 7: This line aut-calculates the number f applicatins that must have the results f verificatin reprted n Lines 9 thrugh 13. The accuracy f this number is based n upn data frm Lines 2, 3, and 6. Line 8: This line aut-fills based n calculatins dne n Line 7.
