NESHAPs ACM Report Property F May 21, 2013 ASBESTOS INSPECTION REPORT Field inspections were performed over a period of five days in April, 2013 at 12 commercial units located at the following addresses in Rock Island, IL: Address Occupant th Street Apostolic Sanctuary / The Refuge th Avenue Express Contracting th Street Jean s Platinum Stylz th Street Express Contracting th Street Suite A Vacant th Street Suite B Boost Mobile th Street Vacant th Street Vacant th Street Tatoo Parlor th Street Asian GB Store th Street Comfort Store th Street Dollar General The purpose of the inspections was to determine the presence of asbestos containing materials (ACM). Suspect materials identified during the inspection were sampled and analyzed in accordance with federal and state NESHAPs requirements. This report itemizes materials tested and provides laboratory results of those materials. The approximate square footage / linear footage of the materials testing positive for ACM are provided together with the approximate location, where appropriate. Suspect bulk ACM samples, both friable and non-friable, were analyzed using polarized light microscopy to determine if the suspect ACMs contain more the 1% asbestos. Attachments include laboratory analytical reports, photographs of sample locations, the Township Assessor s information for the building, a schematic of the building layout, and the credentials of the inspectors. Inspection Summary The scope of inspection included the interior and exterior of the commercial structure. The Property is targeted for demolition and redevelopment by the City of Rock Island. Destructive sampling was employed in some areas to gather representative samples of suspect ACM (asbestos containing materials). The inspection included apparent homogeneous sampling locations of suspect materials, including layers within materials. Random samples were collected from the roof, ceiling tile, drywall/mud, floor tile/mastic, and cove base/mastic. Samples were delivered to EMSL laboratories at its Chicago location to be analyzed for the presence of asbestiform material. The laboratory analyzed 170 bulk samples, including samples with multiple layers. The asbestos inspection was conducted in accordance with OSHA regulation 29 CFR , in specified areas agreed upon by the CLIENT. Analysis of the samples was completed by Polarized Light Microscopy Dispersion (PLM) Staining Method EPA/600/R-93/116. 1

NESHAPs ACM Report Property F May 21, 2013 This survey did not include the internal components, material in storage within the building, or materials considered outside the definition of building materials. Care should be taken when removing, restoring, or disassembling these items. Description of the Buildings in the Scope of Work The survey took place in April 2013 at the addresses previously listed. The commercial space is one story in height. The Property is part of a larger structure; a south common wall adjoins a unit described by the City of Rock Island as Property G ( th St.). Per the Assessor s report, the building was constructed in 1963 and comprises approximately 36,600 square feet. This square footage appears to include an addition that was constructed onto the west side of th Street, as observed. The building foundation was observed to be a concrete slab (no basement) with peripheral concrete walls. The frame is steel with fiberglass insulation panels on the underside of the flat roof, which is covered with a rubber membrane. The interior was observed to have non-structural drywall walls, suspended ceilings, and vinyl cove base. Some concrete floors are bare; others are covered with vinyl or ceramic tiles. Forced-air HVAC units are located in closets of each unit. HVAC ductwork was observed to be without insulation. No pipe insulation was observed in the ceiling cavities. Suspect asbestos materials sampled and analyzed under the prescribed scope of work are listed below. The address of the sample location precedes the Area Number. The following discussion includes individual materials tested, and determined to contain a minimum of 1% ACM: Black FTM 928-FT FT DWM DWM-01 Black floor tile mastic found in 6 of the 12 subject units, each located in the northern portion of the building, was consistently determined to contain regulated quantities of asbestos. EnviroNET estimates the affected area measures approximately 12,000 square feet. This material is considered a Category II non-friable material in good condition. The bottom layer of 12 x 12 floor tile in Express Contracting s office was found to contain a regulated quantity of asbestos. Black mastic is present beneath this floor tile (i.e., see area estimate above). This material is considered a Category II non-friable material in good condition. 9 x 9 floor tile in the back hallway of Jean s Platinum Stylz was found to contain a regulated quantity of asbestos. Black mastic is present beneath this floor tile (i.e., see area estimate above). This material is considered a Category II non-friable material in good condition. Drywall mud in Jean s Platinum Stylz was found to contain a regulated quantity of asbestos (2% Chrysotile). EnviroNET estimates 1,000 square feet of drywall in this area. This material is considered friable. Drywall mud in Express Contracting s showroom was found to contain a regulated quantity of asbestos (2% Chrysotile). EnviroNET estimates 2,350 square feet of drywall in this area. This material is considered friable. Limitations to the Inspection: Based on interview information and on observations, limited assumptions were made as to the consistency of building materials over the larger spaces. The inspection was conducted systematically in accordance with NESHAPs requirements. However, EnviroNET cannot warrant that all ACM material was identified and analyzed, only that apparent materials were appropriately profiled. Because the Property was occupied, sampling there was aimed at inconspicuous locations. Carpet was pulled back from corners in most if not all rooms. EnviroNET surveyed accessible (interior and exterior) suspect asbestos containing materials, excluding the inventory, appliances, material in storage within the building, or materials considered outside the definition of building materials. Based on the experience of the inspectors and the observations made, the conclusions of this report are believed to be accurate and true, despite the limitations as described 7
