11 SECTION NOTICE TO BIDDERS Sealed proposals for furnishing all plant, equipment, transportation, tools, materials, labor and skills necessary and incidental to perform all work described in the Proposed Contract Documents entitled: MADONNA REHABILITATION HOSPITAL TCU ADDITION & IMAGING RENOVATION Bids will be received at the office of Erickson Sullivan Architects, located at 110 S. 14 th Street, Suite 200, Lincoln, Nebraska, until noon, 12:00 PM, local time on Friday, June 8, 2018 and will then be privately opened. The said Documents have been prepared by Erickson Sullivan Architects and may be obtained after May 16, 2018 from A & D Technical Supply for a refundable deposit of $40.00, contact to obtain documents. Documents are also available for viewing at the Lincoln Builders Bureau. Project Description: In general, work includes an approximately 4,400SF one-story addition with full basement on the southwest corner of the Madonna Rehabilitation Hospital. Main level will include six new patient rooms and will tie together two existing wings. Lower level will include storage and some office space. Construction will typically consist of concrete foundation walls, brick and CMU masonry bearing walls, precast hollow core floor structure and steel joist roof structure. Connection to the existing single story building wings occurs at areas of slab on grade construction, and will include underpinning of existing footings to allow the new basement adjacent to unexcavated spaces, and demolition and renovation of existing adjacent patient rooms on the main floor. A new lower level tunnel alongside the existing building will connect the addition basement to the existing building basement outside of the footprint of the existing building. Site work will include grading, walks and utilities. The Imaging Renovation portion of the project includes an approximately 1,600 s.f. renovation of existing storage spaces in the lower level of the Madonna St. James wing, converting these spaces into a new medical imaging suite including CT, Radiology, Ultrasound and associated rooms. Work will include demolition of existing walls, ceilings and finishes and modifying existing mechanical and electrical utilities for head clearance. Construction will include new walls, ceilings and all finishes, a new restroom, lead shielding for the radiology rooms, and associated HVAC and Electrical work. Project Schedule: The project includes the following dates of completions: TCU Renovation Substantially Complete / Main Level: December 20, 2018 TCU Renovation Substantially Complete / Lower Level and Imaging Renovation Substantially Complete: February 1, 2019 All bids shall be made on the printed proposal forms attached to and made a part of the Proposed Contract Documents and submitted in a sealed envelope marked Madonna TCU Addition. Bids which are mailed shall be placed in a separate sealed envelope, labeled as described in this paragraph, inside the mailing envelope Bids mailed to this office shall be addressed to: Bids delivered in person shall be delivered to the same address. No bid bond or performance bond is required by the Contractor for this project. 14 ALTERNATES: Proposed for Alternate Work, as defined in Division 1, as follows: Alternate No. 1: This alternate deduct shall change all 6 service pipe to be 4 service pipe. Alternate No. 2: This alternate deduct shall route 4 sewer service to existing house as shown by dashed line on C201. Install cleanouts at minimum every 75. Remove and replace existing pavement as necessary, match existing type and thickness. No 8 or 6 pipe to be installed. No manholes to be installed. Deduction: Dollars ($ ). (Amount shall be shown in both words and figures; in case of discrepancy, the amount in words will govern.) Alternate No. 3: This deduct shall eliminate all work related to the roof top screen wall.
