CP09008 INDUSTRIAL CANAL LEAD MAINTENANCE / REHABILITATION SUGAR CANE SPUR PREPARED BY LAKE CHARLES HARBOR & TERMINAL DISTRICT 150 MARINE STREET LAKE CHARLES, LA (337) Phone (337) Fa Receipt and acknowledgement of this addendum must be noted in the bidder s proposal. The following items should be noted by bidders on the above referenced project: 1. See attached Pre-Bid Agenda and Attendance List 2. Bidders shall be fully aware to the Construction Sequencing set for with Section GR-15 within the Special Provisions. 3. All Rail, joint bars, and shoulder plates shall be reused. 4. Contractor to provide new bolts & rail spikes. 5. Owner to provide joint bars as necessary. 6. Rail ties designated for the Port to retain ownership shall be marked immediately prior to Contractor mobilization. The contractor shall store ties designated to remain in an area for Port to transport back to the Port facility. Ties shall be neatly stacked and bundled as directly by the Owners onsite representative. 7. Contractor shall dispose offsite of all ecavated materials and ties that are not to be reused or kept by owner in accordance with all local, state, and federal laws and 1

2 regulations. Contractor will provide a manifest to the Owner pursuant to the intended disposal site. 8. Contractor shall submit a Critical Path Schedule in a MS Project or Primavera format. Microsoft Ecel spreadsheets are not acceptable. 9. Owner will re-establish two (2) elevation benchmarks and identify the beginning station for the project as indicated within the project plans. 10. The intended final rail elevations for this project shall coincide with eisting rail elevations and in accordance with AREMA guidelines and industry standards. Minor grade adjustments within the designated subgrade repair areas may be adjusted to promote smooth transitions. 11. Main rail traffic can be diverted to the adjacent spur when rail work is concentrated between Stations 8+00 through from All remaining work will be subject to the Construction Sequencing specifications pursuant to Special Provisions GR Rail Tie spacing shall be on 21 inch centers. 13. The eisting rail is anchored and shall be re-anchored at No Direct Pay. 14. A single Double End Gauge Rod shall be installed with the reinstallation of the #8 Turnout at Station M/L. The costs for this work will be included within Line Item REF. NO After proof rolling subgrade, soft spots designated to be removed as directed by the Port s onsite representative shall be measured and paid in accordance with Line Item REF. NO In the event ecavation of soft areas are directed, the crushed stone backfill will be compacted, measured, and paid for in accordance with Line Item REF. NO Section GR-12 in the Special Provisions shall be deleted in its entirety. 18. Following is the revised Louisiana Uniform Public Work Bid Form and Unit Price Sheet. Bidder shall use this form to submit bid. Line item #2 has been deleted. Line item #8 has been deleted and is now part of Line item #9. Line item #14 and #15 the quantity has changed and the unit of measure has changed to cubic yards. The Transportation Security Administration will issue workers a tamper resistant Smart Card containing the worker s biometric (fingerprint template) to allow for a positive link between the card itself and the individual.
