3 PRE-CONFERENCE WORKSHOPS Monday, October 19, 2015 The workshops run in parallel throughout the day, with refreshments and lunch included. Please note that you cannot switch between workshops. Workshop A: Safety Through Human Factors Incident Investigation and Procedure Design This highly interactive full-day workshop focuses on two key topics in the human factors area: incident investigation and procedure design. You will first have the opportunity to take part in a practical session on the analysis of human factors involved in incidents. You will receive guidance and take part in exercises on identifying the behaviours involved in incidents, determining whether they are intentional or unintentional. This will then be followed by an interactive group analysis of the behaviours involved in a real case study. The second part of the workshop will focus on the design of procedures to promote safety performance. This will commence with an exercise on writing a simple step-by-step task list. Once finalised, the workshop leader will try to complete the task using the analysis to identify where the task description needs to be improved, and how to improve it. Guidance is then provided for presenting a procedure and delegates will have the opportunity to critique an example procedure and recommend improvements. Finally you will learn how to complete a task analysis as a structured method for conducting this type of work in the future. Workshop highlights will include: Distinguishing between intentional and unintentional behaviours Understanding appropriate techniques for understanding errors and intentional acts Interactive demonstration of analysis of behaviours during an incident Techniques for describing tasks in detail for procedure writing Guidance on the presentation of procedures to improve reliability and compliance Improving procedure design through the application of human factors practices Workshop leader: Richard Scaife, Director and Elaine Skilling Chartered Ergonomics and Human Factors Specialist, The Keil Centre Limited Workshop B: Managing Major Accidents with Quantitative Risk Analysis (QRA) Techniques Many major process accidents could have been avoided if prevention, mitigation and protection barriers had been properly designed, kept and maintained. These barriers include an adequate design of site layout and the development of effective risk reduction strategies. This interactive workshop will explore how plant operators can build a visual overview of their process safety data and use this information for development of risk reduction strategies in the area of major accident management. The main method explored during this session will be Quantitative Risk Analysis (QRA) techniques. Attendees will have the opportunity to share their own approaches and techniques and comprehensively review TOTAL's approach to major accident management. The basis of TOTAL s application is an exhaustive list of possible Loss of Containment (LOC) events and related information. The tool includes state-of-the-art models for simulation of the physical effects and their impact of a wide range of accidents that are possible in industrial sites.
