Delivering this commitment is through: the implementation of our He Pou Oranga Tangata Whenua Determinants of Health framework: respect for and promotion of our Kawa and Tikanga Māori; ensuring cultural safety; seeking to eliminate disparities in health between Māori and Non Māori. All staff have a part to play in this commitment. Primary Purpose Kupenga Hao Ite Ora Support Net is a Bay of Plenty District Health Board service. Support Net provides Needs Assessment & Service Co-Ordination ( NASC ) services in the Bay of Plenty for people with disabilities and older people. The service manages access to support services for people with disabilities and manages District Health Board budgets for the provision of residential and community support services, carer support and other services provided under discretionary contracts. Support Net also has involvement with applications for equipment to ENABLE, the national equipment management service.

2 Needs assessment is a process of determining the current abilities, resources, needs and goals of a client using the interrai assessment tools. The purpose of the process is to determine what a client needs to maximise independence and participate as fully as possible in society, in accordance with their abilities, resources, culture and goals. The results of this clinically focussed assessment may identify health and disability issues which will require referrals to appropriate agencies or professionals. A client s needs will also include where appropriate the needs of their family/whanau and carers. Service co-ordination is the process of identifying services required to meet the prioritised assessed needs and goals of the client and, where appropriate, their whanau/family and carers. Service co-ordination also determines which of those needs can be met by government funded services, explores the contribution of informal support (family, whanau, friends, neighbours) and all options and linkages for addressing prioritised needs and goals. Principal Accountabilities Responsibility Outcome Needs Assessment Facilitation of an interrai assessment in partnership with client, family/whanau. Assessment skills utilising clinical expertise will identify client needs and provide accurate documentation of the client s situation. Ensures the client and/or carer has access to relevant information and knows about their rights. Referrals responded to with appropriate and timely prioritisation. A referral for specialised assessment is identified and actioned when needed. Identified risks for clients are documented and appropriate action/referral undertaken to mitigate adverse outcome for client. Service Co-ordination Identify the support required to meet the needs/goals of the client/family/whanau by utilising assessment outcome measures. The cost of publically funded service is with-in the Bay of Plenty District Health Board guidelines. Standard Support Net authorisation, referral and contracting procedures are followed. Considers innovative ways of meeting service gaps. An auditable separation is established between the functions of needs assessment and service co-ordination.

3 Operational Obligations - Demonstrate a commitment to quality service provision. - Efficiently and effectively manages resources. - Work collaboratively with other health professionals. - Anticipate workflow and manage resources and interventions accordingly. - Meet BOPDHB Organisational and Legislative requirements including but not limited to The Code of Health and Disability Consumers Rights, The Privacy Act. - Promotes the NASC service in the community and contributes to the development of NASC. - Works to improve client satisfaction. - Actively participates in risk minimisation activities. Professional Development Attend team meetings and full staff training. Maintain activities relating to competency for Annual Practicing requirements. Maintains activities related to competency in interrai assessment including annual e-learning requirement. Takes responsibility for own learning by recognising training needs to ensure knowledge and skills are current and relevant to position. Attends all interrai specific training to maintain required competency for users. Participate in case presentation and peer evaluations. Mentor and coach team members as appropriate and maintain a collegial approach at all times. Participates in bi-annual audits of assessments and co-ordinations NOTE: It is the individual s responsibility to meet the competency and registration APC requirements of their professional body and to maintain current APC. Utilise Information Technology Demonstrates competent computer skills. Has knowledge of Microsoft software. Demonstrates an ability to access and use clinical information systems.

4 Effective Communication Skills - All communication with clients, family/whanau, staff from other organisations, and BOPDHB staff is conducted in a respectful manner. - Build and maintain positive working relationship with stakeholders. - Confidentiality and privacy are maintained at all times. - Practises Support Net Tikanga office guidelines. Cultural Safety Has knowledge and understanding of the Treaty of Waitangi and its application in terms of the Articles and Principles. Has an understanding of Te Reo Maori me ona Tikanga (Maori language and Maori cultural norms) All cultural differences are acknowledged by respecting spiritual beliefs, cultural practices and lifestyle choices. Other tasks as determined by Regional Manager or Team Leader, Support Net from time to time may be required. 5 Person Specification Qualifications: - Registered Health Professional with current practice certificate - Gerontology knowledge - Current driver s licence Experience: Previous experience in needs assessment and or community work Current knowledge/experience of health and disability services Skills and Abilities: - Demonstrates awareness and sensitivity to health and disability issues affecting older people. - Demonstrates an understanding of te reo me ona tikanga (Maori language and maori cultural norms). Has an understanding of the Meihana Model as a framework for clinical assessments. - Demonstrates excellent communication skills including conflict management. - Commitment to developing and maintaining effective work relationships. - Demonstrates ability to establish credibility and maintain integrity in all situations while balancing service requirements. - Ability to manage budget and resource allocation. - Ability to prioritise, manage time efficiently. - A strong commitment and genuine interest in quality and service. - Ability to respond positively to change and to new opportunities. - Ability to self-evaluate and reflect on practice. Personal Attributes This position requires a person with experience and knowledge in the area of assessment and service coordination for older people. The person will have the ability to work autonomously while working well as a team member. Networks across the disability sector including service providers, will be established and nurtured. It is preferable that the applicant has a good understanding of tikanga maori, and desirable that they are fluent in te reo maori. Values Demonstrates behaviours consistent with the BOPDHB values.

6 You agree to demonstrate flexibility and a willingness to perform a variety of tasks to promote and support BOPDHB initiatives. You are required to meet the Health and Safety at Work Act 2015 requirements as set out in the BOPDHB Health and Safety policies and protocols. This includes completing successfully any health and safety training provided by the BOPDHB.
