1 Role purpose: To assist with developing a programme of activity to befriend, improve wellbeing and reduce isolation of disadvantaged older people. Be a key part of the recruitment of volunteers and be their main point of contact, to support them in helping older people to become less socially isolated. Match older people with befriending relationships. Match older people with new and existing interest groups and organisations in their local community. The recruited post holder must be able to work at a strategic and operational level, quickly establishing credibility with local groups of people. The right candidate must motivate and inspire people across the community, have excellent communication skills, personal resilience combined with a passion for working alongside people in later life. Responsible to: The Community Helpers Manager Based at: Age UK Sutton central Sutton office. Core Values: Respect For Others Focused and Committed Making A Difference Working Collaboratively Key Competencies: Relationship building Planning and Organisation Time management Decision making Commercial awareness Summary of Duties - Key Accountabilities & Deliverables Marketing and co-ordination - to be responsible for taking referrals from primary care to include from older people themselves and their relatives and the home from hospital service - to assess all new referrals for the Community Helpers project as well as helping older people connect to other relevant support (by referring clients to internal and external services). - to match older people at risk of isolation and loneliness to befriending relationships

2 - to match older people with new and existing interest groups in their local community - to monitor and support the progress of older people both directly and with the support of volunteers, including making referrals to internal and external services [using existing tools] - to perform other duties in connection with the general work of Age UK Sutton when required Community Helper support - to assist with the recruitment of community helpers from across all generations - to motivate community helpers as they establish a network of interest/activity groups - to maintain regular communication with volunteers and to be their main point of contact, providing guidance and support Recording and reporting - be responsible for inputting data into Age UK CRM from initial referral through assessment, to activity and annual review - to collect case studies - to feedback progress of clients and volunteers to Community Helpers Manager Development & organisation requirements - attend staff meetings & personal supervision meetings - proactively support service delivery across the organisation - attend Age UK Sutton events e.g. Age UK campaign events & Take Part Take Pride - participate in training/education to improve performance considered relevant to the post & to achieve agreed targets - work with Age UK Sutton policies on quality, Health & Safety, Confidentiality etc
