1 Instructions for Submission of Contractor's State, ment of Qualifications (SOQ) UF Bid Number ITB17KO-107 Whitney Research Village SCOPE OF WORK New work shall include all Labor, Material Supervision and Management to construct the Whitney Research Village in accordance with the plans and specifications prepared by Marquis Latimer + Halback and Associates. This project goal shall be USGBC LEED "Gold" certification with Basic Certification mandatory. Contractor is to support this effort and is required to participate in this program and any associated work to achieve above referenced goals. Contractor is to have experience in USGBC LEED process or other sustainable building practices. Responsibilities shall include weekly meetings with the LEED USGBC team assigned to this project for coordination and updating LEED information for contractor assigned goals. The work is expected to commence in December 2016 and be completed by September 2017 and liquidated damages are expected to be approximately $1, per calendar day. Work on an active campus, along with a constricted site and the need for exceptional safety and environmental controls, will present logistical challenges to the contractor. The User Group is the Whitney Marine Laboratory. Applicants unfamiliar with work at the University of Florida (UF) are strongly urged to familiarize themselves with the UF Design and Construction Standards, the UF General Terms and Conditions, UF GC contract, and other materials available on the University of Florida Planning Design & Construction website. The downloading of these documents is not authorized during Contractor Qualification. Successful bidders will have rights to download or be given a pdf copy of all documents.

2 PRE-QUALIFICATION The method of delivery is design-bid-build (also known as "hard bid"), with a pre-qualification process utilizing the following criteria. The maximum possible score is noted in parentheses: 1. Past Performance on Similar Projects (100) The Owner will evaluate the quality of the applicant's past performance considering currency, relevancy, sources, context and trends. The assessment of the applicant's past performance will be used as a means of evaluating the relative capability of the firm to successfully perform the Work. The University reserves the right to obtain information for use in the evaluation of past performance from any and all sources it deems necessary. The applicant shall demonstrate satisfactory performance on five (5) similar projects within the past five (5) years. Similar Relevant Projects Include: Residential Housing Units. Construction in a Coastal Environment University campus or government project experience. LEED USGBC projects and / or sustainable building construction. Projects with intense schedule, management, and logistical demands - busy campus, adjacent facilities that will remain occupied during the work. Performance on projects that were more similar (i.e., included more of the above-listed characteristics) will be viewed more favorably than performance on less similar projects. Performance on similar projects that included key contractor personnel who will be involved with the UF ITB17KO-107 project (superintendents, project managers, etc.) will be more favorably viewed than performance on similar projects for firms that include less or no involvement by personnel slated for this project. Use the Past Performance Project Data Form to document project, staffing, budget, scope, and schedule information, along with contact information for the Owner, A/E and (3) major subcontractors, for each of the five (5) similar projects. Complete one 2-page form for each project. Verify that the information provided is still valid and add contact information for the current Owner's representative if different from the Owner's representative at the time the applicant's services were provided. Past Performance Project Data Forms can be found at 2. Personnel Assigned to the Project (100) Name all personnel who will be a part of the construction team for this project. Identify each person's title, project specific responsibilities and city of residence. Enclose resumes for all key staff tailored to demonstrate their experience as it relates to this project. Provide an organization chart or diagram if necessary to clearly explain lines of authority, duties and responsibilities. 3. Qualifications & Licensure (50) Provide proof of license to perform work as a General Contractor in the State of Florida, along with proof of proper incorporation, if applicable, from the Florida Secretary of State. Complete Sections 1, 2, and 3 of AIA Document A305.

Financial Standing (50) Provide the contact information, references, and statements required by Sections 4 and 5 of AIA Document A305, including verification of the applicant's capability to provide performance and payment bonds for this project up to $2,000, Safety Record (50) Outline the firm's approach to job-site safety management, training & education, and enforcement. Provide a copy of the firm's insurance experience modification rate (EMR) for the past three years. 6. Workload (50) Provide a simple list of all current and pending work by the applicant firm. Further, provide a more detailed list of all current and pending work that involves the key personnel identified in item 2 (Personnel Assigned to the Project), noting the size, contract amount, scope, owner, A/E, current status, and projected completion date of each. EVALUATION Each applicant's Statement of Qualifications (SQQ) submittal will be reviewed by a committee consisting of UF representatives from Planning Design & Construction, Whitney Marine Laboratory, Physical Plant Division, Design Architect and Purchasing Services. The contractor's response to each of the (6) above-listed criteria will be evaluated and scored by each committee member, with a maximum possible score of 400. The scores for each of the (6) criteria will then be averaged. Submittals with a score of 300 or more will be approved to bid the project. That notwithstanding regardless of scoring, failure to provide proof of current license(s) and bonding capacity will be grounds for immediate disqualification. NOTE: This process concludes with the determination of pre-qualified contractors who are eligible to bid. Relative standing (scores) from the pre-qualification process bear no relevance to the bidding process. FORMAT The SQQ submittal shalt consist of the following, in order: I. Completed and Signed AIA Document A305 (Contractor's Qualification Statement) II. Past Performance Project Data Forms (five 2-page forms) Ill. Qualifications & Licensure ( license, charters, statements, etc.) IV. Financial and Bonding Submittals (information, statements, references) V. Safety Information VI. Workload Information Provide both one (1) hard copy and one electronic copy of the submittal prior to 2:00 pm on November 9, Do not deliver the e-copy on CD or flash drive to UF. Instead, e- mail the file per the following instructions. Late submittals, blank/corrupt files, unsigned submittals, incomplete submittals, or those on a form other than the project-specific CMQS are grounds for disqualification.

Late, ed, or facsimile (FAX} submittals are not acceptable and will not be considered. The (1) hard copy of the SQQ submittal shall be bound and limited to not more than 40 (8.5" x 11 ") pages printed on one side, excluding covers and dividers. Submissions in excess of 40 pages will not be disqualified, but all pages after 40 will be discarded and not considered. PRE-SUBMISSION INQUIRIES A mandatory Pre-Qualification Proposal Meeting will be held at 2:00 PM Friday, October 28, 2016 at the Whitney Marine Laboratory CMS Building, Room 202, 9505 Ocean Shore Blvd. St. Augustine, FL The purpose is to generally explain the pre-qualification process and other project information such as the technical scope of work, site constraints, and the schedule. A site visit / tour will be included immediately following the meeting. All inquiries, comments, and requests for information must be submitted via no later than 4:00 PM on November 3, 2016 to the Architect, Marquis Latimer + Halback at jeremy@halback.com. Responses to such inquiries will be ed or posted to the Purchasing web site. No hard copy reply will be issued.
