1 HEALTH&AND&SAFETY&SERVICES&& February 6, 2015 ASBESTOS INSPECTION: Southern Utah University Mountain Center 351 West University Blvd. Cedar City, Utah HSS Project Number: CLIENT: Attn: Robert Anderson Utah Department of Facilities and Construction Management State Office Building Salt Lake City, Utah EXECUTIVE SUMMARY: Health and Safety Services (HSS) was asked to perform an asbestos inspection for the Southern Utah University s Mountain Center, for Southern Utah University Facilities Management located at the said address listed above. The inspection was intended to identify building materials suspected of containing asbestos, determine quantities and distribution of these materials, and then collect bulk samples, for analysis of their asbestos content prior to renovation activities for the structure. SCOPE OF INSPECTION: The scope of inspection was limited to assessment of materials suspected of containing asbestos from Southern Utah University s Mountain Center and out buildings. Only those areas and materials listed in this report have been inspected for asbestos. Further inspection will be required if additional materials not listed in this report are found or the scope of the renovation/demolition changes. The building is a seasonal use structure. A brief description of the building s exterior is a log cabin structure on concrete slab with an asphalt shingle roof. Interior finishes are as follows: log, sheetrock, sheet vinyl and carpet. The main structure also has adjacent out buildings such as a pump house, storage areas, pavilions, and an exterior restroom facility. These structures are also made of log, concrete and metal. The additional structures were also inspected for suspect asbestos containing materials. For a list of the suspected building materials identified see sampling chart below. 2 HEALTH&AND&SAFETY&SERVICES&& ASBESTOS INSPECTION PROTOCOL This inspection was performed in accordance with the Utah Division of Air Quality (DAQ) requirements found in the Utah Air Quality Rule R Renovation and Demolition: Asbestos Inspection Procedures. This document provides the information required by the regulation. This document is required to be on site during all abatement, renovation, and demolition activities. All samples were analyzed by polarized light microscopy (PLM) at EMC Laboratories, Inc. of Phoenix, Arizona (NVLAP # ). A total of 17 samples were collected that pertain to this site. The following table summarizes the sampling data. A copy of the laboratory s report is attached. 3 HEALTH&AND&SAFETY&SERVICES&& ASBESTOS CLASSIFICATIONS Regulated Asbestos Containing Materials (RACMs) include all friable Asbestos Containing Materials (ACMs) including Category I and II non-friable asbestos materials that have become friable through aging or will be rendered friable during the renovation/demolition process. Category I non-friable includes asbestos packings, gaskets, resilient floor coverings, and asphalt roofing products. These materials may remain in place provided they are not rendered friable by demolition or renovation activities. Category II is any non-friable ACM not included in Category I. Category II non-friable asbestos materials must be removed prior to demolition or renovation activities. POTENTIAL FOR ADDITIONAL MATERIALS Environmental inspections are non-comprehensive by nature and are unlikely to identify all environmental problems or eliminate all risk. This report is a qualitative assessment. Health and Safety Services performed this asbestos inspection in accordance with the generally accepted standard of care for the area at the time of the study. warranty, either expressed or implied, is made including any expression of warranty that all asbestos and/or hazardous materials have been discovered. This survey is only intended to encompass the areas/materials designated within this report. Should the scope of the demolition/renovation be altered to encompass areas and/or materials not specified within this report, additional assessment will be required. The report shall not be reproduced except in full, without written approval by our laboratory. The samples not destroyed in testing are retained a maximum of thirty days. The laboratory measurement of uncertainty for the test method is approximately less than 1 by area percent. Accredited by the National Institute of Standards and Technology, Voluntary Laboratory Accreditation Program for selected test method for asbestos. The accreditation or any reports generated by this laboratory in no way constitutes or implies product certification, approval, or endorsement by the National Institute of Standards and Technology. The report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the Federal Government. Polarized Light Microscopy may not be consistently reliable in detecting asbestos in floor coverings and similar non-friable organically bound materials. Page 5 of 5

The inspection included: 1. Identify building materials suspected of containing asbestos, determine quantities and distribution of these materials, and then collect bulk samples, for analysis of their asbestos content; The HVAC vibration dampeners are assumed to contain asbestos. asbestos was found in the 17 bulk samples collected. 2. Quantify other items within the building requiring special handling and disposal (see the list in this report for details); and 3. Determine whether paint in the building is over the HUD level of 1.0 milligram per square centimeter (1.0 mg/cm 2 ) and therefore considered Lead-based Paint (LBP). The green metal door in the living room has 1.0 mg/cm 2. OTHER HAZARDOUS WASTES The following is a list of the other hazardous wastes or materials identified throughout the structure that require special handling and disposal: - 3 Refrigeration Unit - 1 Mercury Thermostat - 34 Lights - 17 Ballasts LEAD-BASED PAINT INSPECTION An RMD brand X-ray fluorescence instrument, Model: LPA-1, Serial Number: was used to detect LBP. Only the green metal door in the living room area was found to contain LBP. Please see the attached report for further details of the LBP inspection.
