The request prcedures: Frm f request: The requester must use the prescribed frm t make the request fr access t a recrd. This must be made t the head f the private bdy. This request must be made t the address, fax number r electrnic mail address f the bdy cncerned. The requester must prvide sufficient detail n the request frm t enable the head f the private bdy t identify the recrd and the requester. The requester shuld als indicate which frm f access is required. The requester shuld als indicate if any ther manner is t be

5 used t infrm the requester and state the necessary particulars t be s infrmed. The requester must identify the right that is sught t be exercised r t be prtected and prvide an explanatin f why the requested recrd is required fr the exercise r prtectin f that right. If a request is made n behalf f anther persn, the requester must then submit prf f the capacity in which the requester is making the request t the satisfactin f the head f the private bdy. Fees: A requester wh seeks access t a recrd cntaining persnal infrmatin abut that requester is nt required t pay the request fee. Every ther requester, wh is nt a persnal requester, must pay the required request fee: The head f the private bdy must ntify the requester (ther than a persnal requester) by ntice, requiring the requester t pay the prescribed fee (if any) befre further prcessing the request. The fee that the requester must pay t a private bdy is R50. The requester may ldge an applicatin t the curt against the tender r payment f the request fee. After the head f the private bdy has made a decisin n the request, the requester must be ntified in the required frm. If the request is granted then a further access fee must be paid fr the search, reprductin, preparatin and fr any time that has exceeded the prescribed hurs t search and prepare the recrd fr disclsure. 5. Other infrmatin as may be prescribed The Minister f Justice and Cnstitutinal Develpment has nt made any regulatins in this regard. 6.
