The infrmatin belw summarizes ur Club prgram s purpse and general plicies. Yur primary pint f cntact fr the Club Prgram will be yur designated Advisr; Sunburst s Grup Sales staff will wrk clsely with them t ensure that yur Club members and chapernes enjy their experience at Sunburst. Prgram Descriptin & Eligibility Sunburst s multi-week Ski & Snwbard Club prgram prvides an pprtunity fr Club members t participate in skiing r snwbarding activities with their classmates and friends. Our Club Prgrams are geared twards schl grups, church grups r ther nn-prfit rganizatins. In rder t sign up and participate in ur Club prgram yu must have a minimum f 15 qualifying Club members: they must be at least 6 years f age but n mre than 18 years f age at sign-up. Club members are eligible t sign-up fr Sunburst s Zne Lessns and/r rental equipment at discunted prices. Parents ARE NOT allwed t be members f the club, but can receive separate discunts if serving as chapernes n Club nights. Parent Meeting Nights A Parent Meeting Night is required t be held fr every Club. This is an pprtunity fr parents f Club members t learn mre abut ur Club prgram and speak with a Sunburst representative. The parent meeting is free f charge and cvers any changes in plicy alng with specifics f paperwrk. Yur Advisr must set up a time and date thrugh the Grup Sales ffice fr the Sunburst representative t cme in and give a presentatin and answer any questins yur grup may have. Our plicy is t nt have a meeting in the presence f cmpetitrs, as we feel it is inapprpriate. If fr sme reasn yur Advisr needs t hst tw ski hills Parent Meeting nights n the same night, we wuld wrk with them t arrange separate rms r different time slts. Multi-Week Offerings and Pricing Sunburst ffers Club grups the ptin f a 4 Week, 5 Week r 6 Week Prgram. Chapernes/Chaperne Tickets When a grup arrives the advisrs are required t visit the Grup Sales ffice t infrm us f wh is n the hill n a particular day. This infrmatin is extremely imprtant in the event that a grup member is injured and Sunburst staff r ther Chapernes need t cntact yu. Yur grup s Advisr and Chapernes will be respnsible fr mnitring the activities f the members f yur Club and ensuring their participatin. Cmplimentary chaperne tickets are supplied t adult chapernes accmpanying the Club n the evenings yur Club is here at Sunburst. Yur Club qualifies fr ne free chaperne ticket with each 15 paid tickets. A grup f 50 wuld qualify fr 3 tickets, a grup f 100 wuld qualify fr 6 tickets, and s n. The chaperne ticket des nt include rentals r lessns. Parent Pricing Parents and ther adults accmpanying the Club ARE NOT eligible t receive the abve prices. If they wish t ski r snwbard with the grup, they will arrive in the Grup Sales ffice each night separate frm the Club, and will receive a 25% discunt n lift tickets and/r rental equipment fr whichever day and time it is that the Club is meeting. Sunburst ffers a certain amunt f Chaperne Lift Tickets fr free (see Chaperne sectin), but any adult withut equipment will still need t pay fr this separately. Helmets Helmets are nt required by Sunburst but their use is highly recmmended. Helmet requirements are different with each Club - please cntact yur advisr t check yur specific requirements. If a Club member r chaperne needs t rent a helmet frm Sunburst, they may d s fr a fee.

3 Payment We require that yur Club s invice be paid in full prir t the first day f yur Club s arrival. A single payment fr yur grup is required. Sunburst accepts check, credit card, r cash payments. Yu will nt receive yur Club Cards until apprximately ne week after all the paperwrk is prcessed and the payment is made in full, including any applicable N Release frm fees. We encurage yu t pay fr yur Club in advance s yur members can receive their Club Cards n the first day. Unlike ther ski areas, where Club Cards are nt issued until after their multi-week prgram has ended, Sunburst will issue Club Cards a week after yur grup s paperwrk is prcessed and payment is made s that yu can enjy yur Club Card benefits sner. Club Cards Club Cards are issued t each Club member and are nn-transferable. The Club Card permits the member t btain a lift ticket fr him/herself fr $19.00 r a lift ticket and rental fr $29.00 n nn-club days/nights. The Club Card must be physically presented in rder t receive this discunt. These cards are issued t the Club Advisr apprximately ne week after all Club paperwrk has been prcessed and the grup has paid in full; they will nt be distributed befrehand fr any reasn. Club Cards will be distributed by yur Advisr. The earliest pprtunity t receive yur Club Card will be the first day f yur Club unless yur Advisr picks them up directly frm Sunburst. If a child des nt have the card physically present then they will nt qualify fr discunts. If fr sme reasn a child lses his/her card, we can replace the card fr a fee f $ Club Assurance Prgram/Refunds Sunburst ffers a Club Assurance prgram that enables a Club member t receive a pr-rated refund in the event the member is injured befre r during the Club s scheduled events and cannt participate. If members are interested in the pssibility f receiving a pr-rated refund, they must select the Club Assurance ptin n the Club Registratin Frm and pay the applicable $20 Club Assurance fee. If they select and pay fr this ptin and the Club member is injured befre r during the Club, Sunburst will issue a pr-rated refund at the end f the seasn prvided we are supplied with a dctr s nte. If a Club member des nt elect t purchase Club Assurance, we are unable t prvide any refunds. Make-up Day(s) If inclement weather frces Sunburst t clse n yur Club s scheduled day, a Make-up Day will be set. Sme grups set up a Make-up Day in advance but many thers will simply wrk with the Grups Office afterward t set up a new date. Typically Make-up days are set n the same weeknight as the Club wuld nrmally be n, but can be shifted if necessary. Make-up Days are intended t permit a Club t avid missing a date due t inclement weather that frces Sunburst t clse; if a Club member misses ne f their Club days while Sunburst is pen, they will nt receive an individual Make-up Day. If the Club member has elected t purchase Club Assurance, he/she will be eligible fr a pr-rated refund if the reasn fr their absence is due t injury.

4 Lift Tickets Club members will receive a yellw tag which must be wrn and visible while they are at Sunburst. The tag is printed with the child s name, advisr s name, grup, days and times f the club, lessn status, and parent s phne number. The tag serves as their lift ticket - if they dn t have it they will nt be allwed n the hill. If a Club member lses a tag r therwise des nt have the tag physically with them, Sunburst will issue a replacement tag fr $5.00. Lessns The lessns prvided in the Club package are Sunburst s Zne Lessns: grup lessns where participants mve thrugh znes at their wn pace t increase their skills. The Club participant can cme and g frm the lessn as they please - if they mve t a larger hill and decide they need t practice a certain skill r mve again, they may return t a zne at any time during the night t wrk n it. Sunburst s Zne Lessn prgram has been designed t help beginner skiers and snwbarders learn t cntrl speed and directin s that they can understand the limits f their ability and ensure that they ski/snwbard within their ability. As indicated abve, Sunburst s Zne Lessn prgram is primarily intended fr beginner level skiers and snwbarders. If yur child is interested in a private r semi-private lessn during nn-club days r evenings, yu can cntact Sunburst at (262) x245 t schedule a lessn. Required Paperwrk Fr Advisr/Club Member & Parent Sunburst will Club frms t the Advisr upn cnfirmatin f the Club dates. The Advisr will print and distribute yur Club s frms and gather and rganize them after yu return them. The frms and Club payment will then be submitted by the Advisr t Sunburst. Schl Spreadsheet Fr Advisr s use nly. Yu may create and edit the Schl Spreadsheet in Ggle Dcs r ther prgrams, but yu MUST KEEP THIS FORMAT, including all clumns! The spreadsheet is where we pull infrmatin fr labels and tags- that means the child s lift tickets. If the frmat is ignred then we are unable t pull infrmatin t create the tags, s we will nt accept any spreadsheet created in errr: we will send such spreadsheets back t the advisr t be cmpleted prperly, which may significantly delay the registratin prcess fr yur grup and may even result in yur first Club dates being pstpned. If an advisr inputs a name wrng due t frmatting changes the Club will be required t pay fr any crrectins r revisins required. All infrmatin requested shuld be prvided s Sunburst has all the infrmatin necessary in case questins arise, including which participant paid hw much. Accuracy in prviding infrmatin t Sunburst will help ensure that the registratin prcess runs smthly. Registratin Packet There are multiple pages in this packet fr every child. The registratin packet cntains frms that shuld be cmpleted and signed by the Club member s parent and returned t the Club Advisr as sn as pssible. All frms must be stapled tgether and sent t Sunburst by the Club Advisr in a single delivery (nt multiple bags ver the curse f multiple days). Registratin packets must be returned t Sunburst n later than 2 weeks befre yur Club s first scheduled date; if the Club

5 Advisr turns in yur frms later than 2 weeks befre yur first scheduled date, we may be unable t prcess the infrmatin and yur first Club date may need t be rescheduled. Issuance f Club Cards t yur grup may als be significantly delayed. Sunburst Ski and Snwbard Club Registratin Frm 2017/18 Required by every Club member. This lists the package ptins. Please mark Club Assurance if desired and include applicable fee. Sunburst Ski and Snwbard Club Registratin and Release f Liability Frm Required by every member if they d nt chse the N Release ptin (see belw). Yu MUST sign the Chairlift Permissin sectin r yur child will nt be allwed t participate in the Club. 6 Club Member & Parent Infrmatin Including N Release Frms Yu will be asked t sign either a Release r N Release f Liability frm. The Release frm is Sunburst s regular liability release frm: there is n additinal charge assciated with the use f this frm. If yu elect t utilize the N Release frm, an additinal fee will be assessed and the fee fr each N Release frm yu select must be included with yur registratin. The Wiscnsin Skier Safety Act requires us t give yu this ptin. It is yur chice which frm yu use. Unfrtunately Sunburst cannt advise yu as t which frm is mst suitable fr yur particular needs. Please read the frms and cmpare prtectins yurself t understand the difference. Yu must chse either the Release OR the N Release fr each ptin - nt bth. Each N Release frm has a separate fee attached t it, which is in additin t the regular club package price. If the applicable fee(s) fr the N Release ptin(s) d nt accmpany the applicatin Sunburst shall, as part f its plicies, prcess the applicatin with the understanding that the standard Release(s) apply. There are different frms fr Ski r Snwbard rentals due t the different binding systems fr each sprt. Yu must have the frm crrespnding t yur selected activity. If there is any chance that a child may switch partway thrugh the club, sign bth activity frms (thugh if yu select N Release fr bth, yu wuld pay twice). If any frm r infrmatin is missing frm a registratin packet r the N Release fee is nt paid, the Club Member will nt be allwed t participate in Club activities until these issues are addressed. If a child is missing a frm, the Advisr shuld nt allw them t bard the bus until/unless they have the prper paperwrk. Club members are prvided certain rights in accrdance with the Wiscnsin Skiers Safety Act and als have certain duties. All Club members are required t understand the fllwing: Under Wiscnsin law, each individual engaged in skiing r sledding has a duty t d all f the fllwing: 1. Obey all psted warnings and signs. 2. Keep ff f clsed trails and ut f clsed areas. 3. Knw the range f his r her ability and engage in skiing r sledding within that ability. 4. Assess the difficulty f the trails and terrains that are pen t skiing r sledding. 5. Maintain cntrl f his r her speed and directin. 6. Be able t stp r avid ther individuals r bjects. 7. Yield t ther individuals engaged in skiing r sledding wh are ahead r wh are dwn the slpe.
