2 JOB DESCRIPTION COMMUNITY SUPPORT WORKER Cmmunity Services in the Nrthamptnshire Regin Respnsible T: Registered Manager This is t prvide supprt t peple in their wn hme, t ensure peple lead engaging and fulfilling lives, supprting peple t develp life skills and cnnecting peple t their cmmunities, this includes all elements f scial care and supprt that are detailed belw. Rles are available in Nrthamptn, Crby, Wellingbrugh and the surrunding areas ffering supprt t peple with a variety f supprt needs, primarily supprting peple with a Learning Disability. Vacancies can ffer full and part time hurs. Principle Duties and Respnsibilities T develp warm and trusting relatinships with service users t encurage them t express their needs, views and cncerns. T respect the service user s right t privacy and t ensure that their dignity is maintained at all times. T be flexible and respnsive t the needs f service users as directed by their Individual Care Plans. Supprt service users t develp practical and scial skills t retain ptimum cntrl ver their lives. T prvide persnal care in a sensitive and dignified way and in accrdance with the service users wishes. T advise and supprt service users in all aspects f maintaining the safety, hygiene and cmfrt f their hme. T prmte the service user s self esteem and enable them t express their preferences and make chices and decisins. T enhance the cnfidence and cping abilities f service users thrugh encuragement and psitive feedback. T infrm the Line Manager and relevant agencies f any cncerns r significant changes in their needs and circumstances. T reprt any cncerns regarding children r vulnerable adults with immediate effect t the Line Manager/Service Manager/Deputy Manager r the Duty On Call. T supprt service users in cmplying with prescribed medicatin and self medicatin prgrammes. T liaise with the service user s Cnsultant, GP in respect f cmpliance with prescribed medicatin. T reprt all side effects related t medicatin. T wrk within agreed risk management guidelines and t assist service users in reducing risks t themselves r thers. T prmptly reprt all cncerns regarding risks t Line Manager r the Duty On Call and relevant agencies. T maintain a high standard f custmer care and t encurage feedback frm service users and ther agencies. T prmptly reprt and dcument all cmplaints, suggestins and feedback. T take apprpriate actin in the event f emergencies, ensuring that the Line Manager r the Duty On Call Manager is infrmed prmptly. T fllw Health and Safety guidelines carefully and t alert the Line Manager immediately f any cncerns in relatin t Health and Safety issues. T ensure that accurate recrds are kept in the prescribed frmat. T dcument all wrk undertaken in supprt f service users, their general prgress, any cncerns and any cmmunicatin r liaisn with ther agencies. T identify training needs in discussin with yur Line Manager and t attend training events and curses as required. 8. All emplyees shuld be aware that due t the nature f wrk Creative Supprt undertakes there is a requirement t supprt service users with daily living skills and individual activities which will include mving and handling and may invlve supprting peple with persnal care needs. 10. In accrdance with the Health and Scial Care Act 2008, t actively participate in the preventin and cntrl f infectin within the capacity f the rle. 11. T prvide respectful persnal care in accrdance with the needs, wishes and preferred rutines utlined in the individual s persnal supprt plans. 6 TERMS AND CONDITIONS COMMUNITY SUPPORT WORKER Cmmunity Services in the Nrthamptnshire Regin Emplyed by Creative Supprt Limited Pay Structure Pint Pint Pint Pint and 19 year lds year lds year lds 7.05 Waking Night Supplement: If yu wrk a waking night shift yu will receive yur hurly rate f pay, plus 0.50ph fr each waking night wrked. Smking Envirnment Successful applicants will be wrking in Service Users hmes, sme f whm smke. Applicants must therefre make it clear where they wuld be willing t wrk with these Service Users, r if they will nly wrk with nn-smkers. Bank Hlidays An enhancement is paid fr wrking at Christmas and New Year. We d nt pay enhancements fr wrking evenings, weekends r any ther public hliday. Bnus Emplyees will be awarded a ne ff bnus payment f (pr-rata fr emplyees wh are cntracted t wrk less than 18.5 hurs per week) nce they have successfully cmpleted their prbatinary perid, which is subject t the fllwing: Page 6 f 8 Attendance is satisfactry End Prbatinary Review is satisfactry Inductin Checklist is cmplete Line Managers recmmendatin Level 2 and/r Level 3 Health & Scial Care Diplma All emplyees will be required t undertake and cmplete the Level 2 and/r Level 3 Health and Scial Care Diplma, r if the emplyee already has an NVQ but n Learning Disability qualificatin, they will need t cmplete the Level 2/3 Certificate in Learning Disabilities (applicable t Supprt Wrkers wrking in learning disability services nly) as a cnditin f their emplyment. If yu hld NVQ 2/3 health and scial care r equivalent yu will f curse nt need t d the award again, but we may supprt yu t undertake qualificatins. Hurs f Wrk Full r part time hurs. Full time hurs are 38 hurs per week. T be wrked flexibly n a rta which will include evenings, weekends and public hlidays accrding t the needs f the service. Part time

7 hurs t be agreed subject t a minimum f 15 hurs per week. Disclsure Checks All appintments will be subject t DBS enhanced disclsure, ISA and POCA checks. Prbatinary Perid The first six mnths will cnstitute a prbatinary perid. When this is successfully cmpleted, emplyment will be cnfirmed. Hlidays 20 days plus 8 statutry days pr rata Sickness Plicy Creative Supprt perates a discretinary cmpany sick pay benefit scheme which is fr the purpses f preventing hardship during times f serius illness. The eligibility criteria and cnditins fr payment f Cmpany Sick Pay (inclusive f SSP) are shwn in the Emplyee Handbk. Yu may be eligible fr Cmpany Sick Pay benefits subject t cmpliance with these criteria as fllws:- Creative Supprt d nt pay fr the first three days f any sickness absence. First six mnths service - Nt eligible fr Cmpany Sick Pay thugh yu may be entitled t SSP. Six mnths t eighteen mnths service - Up t a maximum f fur weeks at full pay. Eighteen mnths plus service - Up t a maximum f eight weeks at full pay fllwed by fur weeks at half pay. Part time emplyees will receive Cmpany Sick Pay benefits as detailed abve but pr rata t actual hurs wrked each week. Cmpany Sick Pay benefits may be withdrawn r temprarily suspended where perfrmance r attendance is unsatisfactry. Pensin Creative Supprt perates an aut-enrlment pensin scheme with the Peple s Pensin. Life Assurance All staff under Creative Supprt cntract are entitled t free life assurance. This is a valuable benefit which prvides a lump sum equal t tw times annual salary. Discretinary Benefits Creative Supprt ffers discretinary benefits in additin t statutry benefits. These include: Paid paternity leave Enhanced maternity leave Cmpassinate leave Carers leave up t 5 days per annum Emplyee Cunselling Service All staff, their partners and members f their husehld have access t an independent cnfidential, 24 hur telephne cunselling service and t legal and financial advice.
