1 ADDENDUM TO REQUEST FOR IFB 5234-BC UNLV Wright Hall Build Out University of Nevada, Las Vegas Purchasing Department 4505 Maryland Parkway Las Vegas, Nevada (702) Date of Addendum: June 9, 2016 THE FOLLOWING ADDITIONS, DELETIONS, CLARIFICATIONS AND/OR CHANGES SHALL BE MADE AND INCORPORATED IN THE SUBJECT IFB: The addition of the following documents has been uploaded to the UNLV website Question 1 Is this a prevailing wage rate project? Answer 1 The Project is estimated to cost approximately $200, $225, If Bidder submits a bid or the Successful Bidder issues a change order that exceeds $250,000.00, the following applies: IFB documents Section A - Submission Instructions Bid Preparation and Submission: n) Pursuant to NRS, any contract for construction work for which the estimated cost exceeds $250,000 shall be subject to the provisions of NRS, including but not limited to payment of prevailing wages, regardless of whether the construction work qualifies as a "public work" as defined by NRS. 1. In accordance with NRS, Contractor agrees that the Project is subject to the prevailing wage requirements under Nevada Law. Please note that if a change order causes a contract to exceed $250,000, the Owner will audit the entire contract period. Contractor agrees to comply with the Prevailing Wage Act and all other provisions of NRS that are applicable to the

2 Question 2 Project. Contractor shall obtain a State of Nevada Public Works Number as required by the State Labor Commissioner. Contractor shall use the State Labor Commissioner s prevailing rate of per diem wages established for the Nevada System of Higher Education which is 90% of the rate for the locality in which the improvements are to be constructed for each craft or type of workman needed to construct the improvement. Subject to the provisions of applicable law, Contractor agrees not to pay less than the specified prevailing rate of wages to the contractor and its employees selected to construct the improvements. Contractor will include the substance of the prevailing wages requirement of this Section as contractual language in all contracts and lower tier subcontracts. In addition, all solicitations and contracts shall contain the applicable prevailing wage rates. Contractor will monitor compliance to the payment of prevailing wages pursuant to Nevada Administrative Code 338. Contractor shall keep accurate records showing the name, occupation; actual per diem wages paid to each employee used in connection with construction of the improvements and other information as required by 338, 070. Such records shall be open to inspection and reproduction by the Owner during normal business hours. Contractor will send one (1) copy of each wage report to UNLV s Project Coordinator no later than 15 days after the end of each calendar month. This Section 6 shall be deemed to incorporate any future modifications to the NRS or NAC with respect prevailing wage requirements that are applicable to the Nevada System of Higher Education. The Public Works Number for this Project is PWP# N/A Prevailing Wages Rates for Clark County must be used. See PWP Website at Click on Public Works/Prevailing Wages by County, and then click on Clark to view or print the Prevailing Wage rates for this project. Contractor shall report to the Labor Commissioner and the Owner the name and address of each subcontractor performing work on the project within 10 days after the subcontractor commences work on the project and the identifying (PWP) number for the public work. Substitution Request Only one manufacturer listed for the fume hood and would like to request for ICI fume hoods to be approved for this project. See attached specifications Answer 2 The fume hood specification was written to match existing laboratories to provide for uniformities / standard for all the laboratories. If a substitute item is bid, two (2) copies of complete specifications of the substitute item must be submitted. Alternate specifications must demonstrate compliance with all fume hood specifications. Please refer to Exhibit C, Division 11 Equipment, Question 3 Would like to clarify that the Data symbols on sheet E1.01 receive (2) CAT6 data cables to each location. The Electrical Legend on page E0.01 does not clarify nor do the IT Wiring standards. Answer 3 Yes, two CAT6 data cables at each location.

3 Question 4 What is the deck height? Answer 4 Deck height is 15-0 Floor to Floor Question 5 Is the ventilation duct really tucked in the back corridor? Answer 5 Location shown on mechanical plans is per the Record Drawings. The chase that runs up to the roof is along this outer wall, so this location makes sense. However, visual confirmation was not able to be performed during design. Question 6 Does UNLV use a proprietary sprinkler or alarm system? Answer 6 No proprietary vendor or manufacturer for sprinklers. UNLV prefers a Notifier brand alarm system. Question 7 Where will project parking and staging be located? Answer 7 Once a contractor has been selected, Planning & Construction will work with them to determine a proper staging and parking area. Likely part of the lot at the east end of Harmon by the museum will be sectioned off. Question 8 Can you provide pictures of the inside of the electrical panels? Answer 8 See attached sheets. Panel CH2 was incorrectly called out as being in C215 on sheet E6.01. It is located in C236. CH2 / C236
