2 ADDENDUM NO:1 TO REQUEST FOR QUOTATION CRFQ E-Tender Sys No: INTRODUCTION Addendum No: 1 is issued against the tender for TURNAROUND JOBS IN PROCESS UNITS IN FCCU COMPLEX AT BPCL MUMBAI REFINERY DURING SEPT OCT This Addendum shall form the part of tender document for Package-2, FCCU COMPLEX E-Tender No: and shall be signed and submitted along with the techno-commercial bid. The description of the Addendum is given below; Package 2: FCCU COMPLEX COMMERCIAL Section No/Page No. of the document in the tender NEW CLAUSE Sub heading Ref Addendum issued DETAILS FOR GST a) Bidder to quote all taxes on inputs/input services and capital goods for which no credit is available against output taxes and is included in the base price with following details: 1. Type of tax i.e. octroi, entry tax, cess, service tax, etc 2. Rate of Tax 3. Base value on which tax is calculated 4. Tax amount included in the base price b) In case of bidder not being a manufacturer or first stage /second stage dealer and not giving any cenvat invoice under central excise act, the amount of excise duty included in the base price. ANTI-PROFITEERING CLAUSE OF GST If any new tax is introduced on sale of goods/services by VENDOR to OWNER in lieu of one or more of the then existing taxes or as a new tax altogether and the rate and impact of the new taxes is less than the rate and impact of existing tax or taxes which it replaces, VENDOR shall pass on to OWNER the benefit thereof by of commensurate reduction in the amount payable by the OWNER to VENDOR. If on the other hand, the rate of the new taxes is in excess than the rate of the new taxes it replaces, the OWNER on satisfactory proof shall reimburse the VENDOR the additional tax paid by the VENDOR as the result of the imposition of the new taxes provided they are within the contractual completion date.

3 Explanation: For the purpose of above clause, impact means and includes the addition/reduction of taxes suffered on inputs, input services & capital goods. Further, impact of stranded taxes like CST on Interstate purchases, entry tax/octroi, etc (if applicable) also needs to be considered. The vendor shall take steps viz uploading invoice in GSTR 1, payment of the tax liability on the said invoices and filing of Returns etc. and comply with all the requirements of applicable laws including GST laws for the time being in force to enable the OWNER to avail tax credit/s including input tax credit. Any loss or non-availability of input tax credit by the OWNER due to noncompliance of applicable tax laws including but not limited to GST laws in force or otherwise, on the part of VENDOR, an amount equivalent to any tax liability accruing to the OWNER and/or to the extent of any loss accrued to the OWNER due to the non-availability of input tax credit or any liability accrued to the OWNER shall either stand cancelled or deducted from the payment due to the VENDOR or shall be reimbursed by the VENDOR as the case may be till such default is either rectified or made good by the VENDOR and the OWNER is satisfied that it is in a position to claim valid input tax credit within the time-lines as per applicable laws. Any cost, liability, dues, penalty, fees, interest as the case may be which accrues to the OWNER at any point of time on account of non-compliance of applicable tax laws or rules or regulations thereof or otherwise due to default on the part of VENDOR shall be borne by the VENDOR. An amount equivalent to such cost, liability, dues, penalty, fees, interest as the case may be, shall be reimbursed by the VENDOR within 30 days. Any GST as may be applicable on such recovery of amount shall also be borne by VENDOR and same shall be collected by the OWNER. 4 VOLUME III ATTACHMENT 11 SECTION R VOLUME III ATTACHMENT 11 SECTION L VOLUME III ATTACHMENT 11 SECTION P RRS & CO BOILER PACKAGE MODIFICATION JOBS MISC JOBS Sr No 15 REFRACTORY REPAIR IN CO BOILER COMBUSTION CHAMBER Following jobs are included in Lump sum Scope: Carry out renewal of complete refractory lining of 2-T and longitudinal Beam of approximate area - 7 m2 and complete pipe header of approximate area m2 as per as built Windsor drawing WRPL-BPCL-02F03-RD01. Carry out refractory repairs of complete 3 nos. duct with Insulyte HT and Resco AA22S with hexmesh of approximate area 9 m2 Carry out repairs of radiation wall brick lining of area 4.0 M2 as per as built Windsor drawing WRPL-BPCL-02F03-RD01. Carry out repairs of radiation wall castable lining of area 4.0 M2 with 125 MM THK Insulyte 11 LIZ and 125 MM THK. Tabcast 94 with inconel 600 anchors. Provide separate anchors for each layer. Carry out renewal of the complete sloping roof refractory lining completely as per as built Windsor drawing WRPL-BPCL-02F03- RD01 with total thickness of 240 mm. (94 % white tabular alumina dense castable thickness of 165 mm and 75 mm thick ceramic fibre board 320 Kg/m deg C. with Inconel 600 anchors). Approximate area of the roof is 24m2. Contractor shall fabricate the sloping roof and carry out application of the refractory prior to S/D. During S/D the existing roof shall be cut and to be replaced with the new fabricated one. Kindly note additional Modification job as per following has been added to Lump sum scope. The drawings for the same are uploaded. Sr No MOC no Job Description 8 FCC COMPLEX RELOCATION OF FCC CHARGE HEATER O/L TO FCC HP STEAM HEADER BLOCK VALVE ALONG WITH SPACER. ADDITIONAL STEAM TRAP AT STEAM TO HVU MANIFOLD TRIFURCATION POINT ALONG WITH DRIP LEG TO BE PROVIDED. Kindly note that drawing of the above modification will be provided after the award of PO. It is an IBR Job and contractor has to carry out necessary jobs as per Point no. 11 of Scope of Work from Volume III Attachment 11 Section L in addition to general scope. The Job involves welding of 2 nos Flange Joints of 8 and 2 nos Butt Joints of 8, also a drain line of ¾ with steam trap and 2 valves of ¾ are to be provided. Structural Jobs as per following Inspection Notifications are to be carried out during S/D. Payment shall be carried out as per applicable SOR. Rates in SOR are inclusive of any scaffolding required for executing the job. Notification No Structural steel repair works in FCCU Work to be carried out: Structural steel repair works in FCCU 1. Location: south west corner of FCCU: The gusset plates light pole structures are perforated. Carry out renewal with new 10

5 mm MS plate as existing. 2. West side of 12-V-08: Staircase str. north/west corner vertical member ISMB 350 is perforated at base plate level. Carry out patch welding with 10mm MS Plate. 3. MAB Suction filters supporting structure: first floor level, few bracings are perforated; connection cleats with supporting structure are also severely corroded. Carry out renewal of the members as existing. 4. Reactor working platform at second floor level: bottom support structural members/plates connected to structural Members are perforated and thinned out. : Carry out renewal/patch welding with 10 mm MS plates. 5. Reactor riser second floor: grating found damaged; to be renewed. 6. In between sixth and seventh floor horizontal member (ISMB 150) along with gusset plates is perforated. Carry out patch welding. 7. Connection plates of horizontal bracing at fourth floor, west side are detached: Carry out welding as existing. Notification No E-13 E/F Supporting steel structure Work to be carried out: 1. Structural steel platform in front of 20-E-13 E/F, beams were found severely perforated and thinned out. Replace perforated, thinned out and damaged structural beams as per existing. 2. Paint the structure with AES Rev-1 Notification No FCCU AFC Platform Corroded Work to be carried out: Observations: Heavy corrosion is found in chequered plates of the below mentioned floors (a)gcu Deb AFC Floor (b)20 E 13 EF Floor (c)20 E AFC Floor (d)20 E 12 E-H AFC Floor After removal of chequered plates it was observed that: 1. Heavy corrosion is found in the steel sections supporting the staircase in GCU Deb AFC Floor. 2. A steel beam supporting gratings is found perforated in 20 E 13 AFC Floor. 3. A steel column along with the bracing is found Perforated & damaged in 20 E 13 AFC Floor. Recommendations: Remove all corroded chequered plates as it is not recommended to use it. 1. Carry out patchwork of supporting columns of staircase & Replace the bottom plate, supporting the columns in GCU Deb AFC Floor. 2. Replace the perforated beam of 20 E 13 AFC Floor. 3. Replace the bracing as existing & carry out fireproofing of

6 column as per standard procedure in 20 E 13 AFC Floor. Notification No Steel works in FCCU Work to be carried out: Observations: MEROX UNIT: 1. Base plate of 24-V-04 & 24-V-02- Base plate of equipment found heavily corroded. 2. In between pipe rack-4 and 24-V-02, 24-V-04- Four steel pipe supports found heavily corroded V-02, beside pipe rack-4-3 out of 4 steel supports found corroded V-09, beside pipe rack-4- Perforation found in steel beam supporting stairs V-08- Gratings of staircase found heavily corroded and thinned out. 6. Beside 24-V-09- Scaling found in base plate of column. No pedestal is provided in one out of four supports. 7. Beside 25-V-09- Scaling found in one steel pipe support. 8. NC-02 and 24-V-01- Grating found heavily corroded and pitting observed. REACTOR STAIRCASE: 1. Between landing no. 5 & 6, Between C & D column gusset plate of diagonal bracing corroded and detached near column C Landing no.6- Between C & D, Holes at ISMB200 tie beam web. 3. Between landing no. 7 & 8, Between B & C column, Landing Support column ISMB 200 bottom web is corroded and is with holes. 4. Between landing no. 7 & 8, Between C & D column, Gusset plate of diagonal bracing found with hole near column C. 5. Between landing no. 8 & 9 Between C & D column, Diagonal Bracing angle corroded near column C. 6. Landing no. 9, Between B & C column, ISMB 250 web corroded for 1.5 m long and one hole near column C. 7. Between landing no. 9 & 10, Landing grating in front of lift to be weld. 8. Landing no. 10, Between C & D column, Toe board, mid rail, hand rail and hand rail post detached. 9. Landing no. 10, Between C & D column, ISMB 200 Web corroded near column C and 400mm long hole found at web 10. Between landing no. 10 & 11, Bend in top step. 11. Landing no. 11, Between C & D column, ISMB 200 Web corroded for full length and 150 mm long hole found at web 12. Landing no. 11, Between D & A column, ISMB 250 web corroded. 13. Landing no. 12, Between C & D column, ISMB 200 web corroded and 300 mm long hole at web 14. Landing no. 12, Between C & D column, Toe plate found corroded and detached 15. Landing no. 12, Between B & D column, Hand rail and hand rail posts 3 nos. detached from bottom. 16. Between landing no. 12 & 13, C column, Full length hole at web

Landing no. 14, Between C & D column, Gusset plate of diagonal bracing corroded and found with hole near C. 20. Landing no. 14, Landing grill to be welded properly. 21. Landing no. 14, Between C & D column, ISMB 200 web corroded and bottom flange thinned down. Holes found at web. 22. Landing no. 15, Between C & D column, ISMB 200 web corroded and bottom flange thinned down. Holes found at web. 23. Between landing no. 15 & 16, Between C & D, Holes found in the bottom and top gusset plates of diagonal bracing. Diagonal bracing angle ISA 75x75x6 also corroded 24. Landing no. 16, Between C & D column, Two holes found in the ISMB 200 web and web detached from column C 25. Landing no. 17, Between C & D column, ISMB 200 web corroded and two holes found in the web 26. Landing no. 18, Between C & D column, Hole found in the ISMB 200 web. 27. Between landing no. 4 to 14, C column, Lot of pitting corrosion at west side flange 28. Between landing no. 4 to 9, C column,lot of pitting corrosion at web 29. Lift structure casing plate corroded and found with holes all round the structure. HEATER NEAR PIPE RACK-1 AND PIPE RACK-3 JUNCTION: 1. Stairs beside burner 4- one no. C Channel support found perforated. 2. One monkey ladder C channel found perforated at bottom. 3. I section supporting heater s grating- 3 nos. found perforated and 4 nos. corroded. 4. Plate supporting angle section, around the heater, to support grating found perforated. STRUCTURE BESIDE HEATER AND PIPE RACK-3 1. Stairs north side: 1st Level platform grating found corroded. One beam in 2nd level platform found perforated. 2. Beside 30-V-05, scaling found in plate attached to railing support. 3. In 30-V-05 floor, Base plate found corroded. Severe cracks observed in pedestals. 1 bracing, 2 beams, 3 columns of floor grating supports found perforated. RECOMMENDATIONS: 1. Remove the scaling of structural members and offer for inspection. 2. Renew the perforated, thinned out structural steel members as existing; paint the new and corroded structural members as per AES-5430 rev-1, system 1 & 13 respectively. 3. In case replacement of perforated/thinned out steel members is

8 not feasible at site, repair the same with patch plate of 10mm thick either side or four side of perforated sections covering lapping of patch plate in good surface area for transfer of stresses based on actual on-site inspection. Observation: STAIRCASE 1. In staircase scalling, corrosion and weld damage found near angle joint at around 95 locations. 2. 2nd floor staircase steps found detached at few locations, as observed moving from 2nd floor to 1st floor downwards. 3. Chequered plates outside machine room found thinned out. 4. 8th floor to Machine room staircase to be replaced completely. 5. In structure beside lift west side, Plate below shed found perforated and shed found damaged at two locations. 6. Railings beside structure having equipment 020-V-25 to be replaced. 7. Beside FT-712, burner 1 & 2, 5 I section supports to be repaired by providing patch plate at bottom. REACTOR: Reactor steel platform cantilever support embedded in lagging, in view of this remove the lagging surrounding supports and offer for inspection. FRACTIONATOR COLUMN: Steel support of fractionation column is embedded in lagging, in view of this remove the lagging surrounding supports and offer for inspection. RECOMMENDATIONS: 1. Steps support angle and its welding found corroded. In view of this renew the corroded & thinned out angle & renew the welding. 2. Provide proper joint to the detached members in standard manner. 3. Replace the outside chequered plates with gratings. All other terms and conditions of the tender shall remain unaltered. *******************
