2 HISD DAVILA ELEMENTARY SCHOOL ROOF REPLACEMENT ADDENDUM #1 JOB NO A PAGE 1 To the Plans and Specifications for Jaime Davila Elementary School Roof Replacement, this addendum forms a part of the Contract Documents and modifies said documents as follows: PART 1 - GENERAL 1.1 Bids, Both Part A and Part B, Section AB, M/WBE Forms will be received no later than 2:00 PM, May 1, 2013, at the Board Services Office, 4400 West 18th Street, Houston, Texas Bids submitted without M/WBE forms, properly completed, will be considered not responsive. Bernard Willingham, with HSD Supplier Diversity Business Assistance can be reached at or at bwilling@houstonisd.org for questions related to M/WBE requirements. 1.2 Earl Finley, C.P.M, A.P.P., HISD CFS Sr. Sourcing Specialist can be reached at for questions. His is wfinley@houstonisd.org. However, all project related questions will be forwarded to Noe Almaguer with Molina Walker Architects. 1.3 Campus will have Summer School between June 10, 2013, and July 18, 2013, Monday thru Thursday. Contractor is allowed to work during this time, but if there is disruption during construction, then Work will be performed after 2:30 PM, Fridays and weekends. 1.4 Submit the Bid sealed in an envelope, with a label on the exterior providing the proposer s identification, project name, due date and time. 1.5 Bid Bond in the amount of ten percent (10%) of the full bid amount is required. The Bid Bond is to be in a standard size envelope affixed to the outside of the Bid envelope. Performance and Payment Bonds are required. 1.6 Estimated construction cost is $800,000 to $850,000. Testing has been done for asbestos, and there is none. 1.7 The estimated project duration is 90 days. HISD would like to get most, if not all, of this completed by end of summer. 1.8 Project has Liquidated Damages, $250 per day. 1.9 No substitutions for roof specifications are allowed Wage minimum rates are listed in Section BD, Wage Rate Schedule Roof Replacement drawings have been approved for permit and waiting for permit payment. They are at the City, awaiting Contractor pick up and process permit payment. The Contractor will include permit fee of $2, in Bid Testing Dates. Whenever the school has testing dates such as STAAR test, etc., contractor will need to work around these dates as not work is allowed during testing periods Work Identification Badges are required for all General Contractor s employees. See specifications Section II-2. Security is critical on the project as well as keeping Principal John Baker informed of construction operations through Johnston, LLC Unit prices will be included ON Bid Proposal Form for decayed wood and damaged lightweight concrete fill if encountered during removal of existing roof. See attached Specification, Section UNIT PRICES, and CB2 BID FORM. ADDENDUM 1

PART 2 - MANUFACTURER/PRODUCTS APPROVED AS EQUALS NOT USED PART 3 - SPECIFICATIONS 3.1 Section CB2: BID FORM ALTERNATES & UNIT PRICES, REPLACE Form in its entirety with attached CB2 BID FORM. Section on Unit Prices was added. 3.2 Section UNIT PRICES, ADD section in its entirety. PART 4 - DRAWINGS NOT USED PART 5 - CLARIFICATIONS AND RESPONSES TO QUESTIONS 5.1 Question #1: What is the required minimum R-Value for roof system? Response #1: R-22 per Specifications, Section , Part 6.6.A Question #2: What is the thickness of the existing lightweight concrete fill? Response #2: Existing concrete lightweight fill has a minimum 3-inch depth per Record Drawings. See attached core pictures for conditions above lightweight concrete fill. 5.3 Question #3: What FM requirements are to be used to secure roof system to deck? Response #3: FM 1-90 per Specifications, Section , Part 1.4.C.A. 5.4 Question #4: What is the roof board minimum thickness? Response #4: The nominal thickness is 3.1 inches per Specification, Section , Part 2.6.A Question #5: What type of insulation cover board will be used? Response #5: 1/2-inch thick high density polyisocyanurate foam core roof insulation complying with ASTM E108 and UL 790 per Specifications, Section , Part 2.6.B. 5.6 Question #6: Will there be any new roof curbs as indicated in Key Notes in Sheet A3-9? Response: No, no new cubs will be required on project. Contractor is to remove and reinstall existing roof top equipment as required for installation of new roof system. 5.7 Question #7: Can cold-applied adhesive be used to attach roof insulation to substrate or to another insulation layer? Response #7: Only to another layer of insulation and if approved by specified roofing manufacturer, Siplast, Inc. or Soprema, Inc. 5.8 Question #8: Will the Bid Due Date be extended? Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section. 1.2 SUMMARY A. Section includes administrative and procedural requirements for unit prices. 1.3 DEFINITIONS A. Unit price is an amount incorporated into the Agreement, applicable during the duration of the Work as a price per unit of measurement for materials, equipment, or services, or a portion of the Work, added to or deducted from the Contract Sum by appropriate modification, if the Scope of Work or estimated quantities of Work required by the Contract Documents are increased or decreased. 1.4 PROCEDURES A. Unit prices include all necessary material, plus cost for delivery, installation, insurance, applicable taxes, overhead, and profit. B. Unit Cost does not include wood nailers scheduled to be replaced in Contract drawings as part of Base Bid. B. Unit Price No. 2: Repair of Existing Lightweight Insulating Concrete (LWIC) 1.
