The pricing responses (1.1A & 1.1B) have been revised. The revised pricing forms are included as an attachment to this addendum. [two (2) pages] 2. The deadline for the submission of questions for this solicitation remains 2:00pm, Thursday, May 7, RESPONSE TO QUESTIONS: 1. Question: When would be the last day to arrange a site visit? Response: The last day for a site visit is Tuesday, May 19, To schedule a site visit please contact Mr. Scott Plank, Head Mechanic, Norwalk Fire Department, via telephone at or via at SPlank@Norwalkct.org All requests for such site visits must be at least 24-hours in advance of this site visitation deadline.

2 2. Question: Was the pre-bid conference mandatory? Response: No, the pre-bid conference was not mandatory. 3. Question: I am trying to complete bid for the removal of the underground storage tanks located in Norwalk. I have one question regarding the asphalt replacement. Is it 1 course, 2 course and 1and half inch or 2 inch? Any help would be greatly appreciated? Response: The asphalt (bituminous concrete) shall consist of two (2) two (2) inch courses, totaling four (4) inches of asphalt product. For your reference, a copy of the City of Norwalk Department of Public Works Standard Detailing Drawings can be accessed at the link pasted below: 4. Question: Will you be issuing a new bid pricing form, to reflect the elimination of items 4 and 5, as well as the addition of the concrete pad and hookup of the 275? (If you do, you can throw in a line item for the cost per ton, or even better per square foot, for additional? Response: Yes, new pricing response forms (1.1A & 1.1B) have been issued with this addendum. These pricing response forms have been revised to correspond to the scope of work revisions outlined in addendum #1. 5. Question: Concerning the piping for the above ground 275 tank that is to be placed by the generator, What size of piping is running from the existing 10, 000 gallon underground fuel tank to the generator as of now? Response: The emergency generator is currently piped with an 1 inch pipe bushed down to 3/8 feeding the fuel filter housing which is located inside of the generator enclosure. 6. Question: What are the approximate dimensions of the concrete pad that you wanted installed for the new 275 gallon fuel oil tank? Response: The concrete pad should measure approximately four (4) feet wide by six (6) feet long and be a minimum of four (4) inches thick. Once the concrete pad is installed, the successful Contractor shall be responsible for anchoring the new 275 gallon tank to the concrete pad and then connecting that tank to the emergency generator. A pre-bid conference for this project was held 2:00pm, Thursday, April 30, 2015 at Fire Support Services Facility, 100 Fairfield Avenue, Norwalk, CT. A copy of the sign-in sheet from that pre-bid conference is attached to this addendum. [one (1) page]. 2. The 1,500 gallon diesel above ground storage tank located near the front entrance of the facility is staying in place and is not part of this project. 3. The 2,500 gallon heating oil above ground storage tank located on the driveway side of the facility is staying in place and is not part of the project.

In lieu of relocating one of the existing above ground fuel storage tanks, the City of Norwalk shall be purchasing a new 275 gallon above ground fuel storage tank to be placed adjacent to the generator. The successful contractor shall be responsible for connecting this new fuel storage tank to the existing generator. 5. The remaining underground fuel storage tanks (UST s): [500 gallon waste oil, 2,000 gallon heating oil, and both of the 10,000 gallon fiberglass tanks] shall be removed. 6. Included with this addendum is a copy of the Connecticut Department of Energy and Environmental Protection UST Compliance Inspection Checklist for this facility, dated 05/13/2013. [Ten (10) pages] RESPONSE TO QUESTIONS: 1. Question: Is there any fuel in the existing tanks? Response: Yes. However, the city shall remove the existing fuel. Any water or sludge shall be removed by the contractor. 2. Question: If there is fuel, what type of fuel is in each tank? Response: The 10,000 gallons contain diesel fuel, the waste oil tank contains waste oil, and the 2,000 gallon tank contains #2 heating oil. 3. Question: Do you require (48 hour) hazmat protocols for this project? Response: Yes, everyone on-site must have 40hr hazwopper training. 4. Question: Who will perform the closure sampling? Response: The City s consultant, Big East Environmental, will be performing closure sampling. 5. Question: Does the City require a tightness test before the removal of the tanks? Response: Yes, refer to the bid specifications for the testing requirements. 6. Question: Do we need to remove the footing structures on any of these tanks? Response: No, but they will need to be sufficiently opened to allow for access to the soil beneath the tanks for testing. 7. Question: Do you want all of the underground lines removed? Response: Yes. 8. Question: Do you want all of the underground lines removed? Response: Yes, all of the underground lines for the UST are to removed.

9 9. Question: Please verify who will be responsible for removing (and disposing) of the fuel(s) in each tank? Response: The city shall remove the existing fuel. Any water or sludge shall be removed by the contractor. 10. Question: Are the existing tank single wall or double wall? Response: Asphalt coated bare steel and fiberglass. 11. Question: Does all of the existing concrete get removed? Once concrete is removed do we re-surface with new concrete or new asphalt? Response: Yes all concrete shall be removed and replaced with asphalt. Additionally, include with your submission, a per-ton unit price for the supply and installation of asphalt outside the perimeter edges of the work site area. Submit this unit pricing information on your company letterhead. 12. Question: What is the budget for the project? Response: The total budget for this project, including hard and soft cost costs is $35, Question: What is the estimate start date for the project? Response: Barring any budgetary constraints, the City anticipates that the project would commence in mid-june or early-july, Question: Are there any worksite restrictions that we need to consider? (ex. Closure of the driveway area? Response: No. However, the successful contractor would be expected to coordinate all on-site work with the City s consultant, Big East Environmental, and the Norwalk Fire Department. 15. Question: Are we responsible for connecting the existing tank (tank being relocated) to the existing generator? Response: Yes. However, instead of relocating one of the existing above ground fuel storage tanks, the City of Norwalk shall be purchasing a new 275 gallon above ground fuel storage tank to be placed adjacent to the generator. The successful contractor shall be responsible for connecting this new fuel storage tank to the existing generator. 13. Question: Was attendance at the pre-bid conference mandatory?
