1 Job Title: MS Consultant (PT,OT, SW, CSW, RN) Reports To: Practice Leader Local Area Responsible For: A MS Consultant (MSC) is responsible for providing a person-centred, responsive and timely service, through phone or face to face contact with people affected by Multiple Sclerosis (PabMS) or other progressive neurological disorders. Position Purpose: A MSC will support PabMS or other progressive neurological disorders to reach their goals, through conducting comprehensive and/or discipline-specific assessment (as agreed to by the PabMS), providing episodic case management & psycho-social support, targeted intervention delivery of evidence-based information, making referrals and advocating on behalf of clients to enable access to appropriate services. ORGANISATION CHART General Manager- Services Innovation Senior Manager, Care Practice Leader- Local Area MS Consultant Position Dimensions: Budget: Staff Numbers (Guide): Geographical Spread: Other: NIL NIL Local metropolitan or rural area; ACT/NSW/Vic Support other services as & when required Discipline-specific Overview: MSC s may also undertake the following discipline- specific tasks: Occupational Therapist: Nursing: Conduct comprehensive assessment of capacity and needs within the environment most relevant to the client. Provide specialist intervention to promote occupational performance and role fulfilment including fatigue management, cognitive strategies and equipment prescription Provides clinical assessment, nursing interventions and symptom management advice (e.g. continence) Provides immunotherapy support in accordance with client needs and choice, including training and PD MS CONSULTANT FINAL 2014 Page 2

3 Physiotherapists: Social Workers: Community Support Workers: education in the use of disease modifying medication and self-injection techniques, side effect management and individual follow up. Promotes proactive management & monitoring of MS symptoms through early interventions to achieve wellbeing Conduct specialist assessment of physical function, mobility and the needs of the client to identify MS related issues and goals. Provide recommendations on suitable activity levels, including referrals for therapy or exercise programs as appropriate. Provide recommendations on suitable equipment including mobility aids and specialised seating or positioning aids as required Provide education regarding self-management to promote a healthy lifestyle and minimise the impact of MS symptoms. Undertake effective and holistic assessments appropriate to people affected by MS (PabMS) including analysis of their social, cultural and physical contexts, strengths and capacities and the assessment of the nature and level of any risk. Support PabMS with decision-making, identifying their needs and accessing services appropriate to meeting those needs e.g. future planning, accommodation. Select from a range of specific social work theories, methods, skills, techniques and interventions appropriate to working with PabMS to address the needs and goals of individuals/families/groups and communities e.g. use of appropriate counselling skills to address issues such as the impact of diagnosis, adjustment, grief and loss. Contribute to identifying and addressing systemic and policy inequities through such methods as social action, advocacy, research and evaluation. Undertake comprehensive psycho-social assessments and individualised planning using an enabling person centred strength based approach Identification and action of required interventions and support Assistance with Co-ordinating, managing life stages, transitions and supports Promoting and motivating the individual to navigate choices to maximise participation and well-being PD MS CONSULTANT FINAL 2014 Page 3

JOB REQUIREMENTS Key Result Area 1 Person-Centred Support Provide a timely response to new referrals allocated by Practice Leader Undertake person-centred and comprehensive assessments, that may include discipline specific assessments Promote and support client self-management by adopting a person-centred planning approach to all client interactions, and empowering clients to actively participate in treatment planning, goal setting and to take action on agreed plans Using a multi-disciplinary approach, undertake holistic needs assessments to develop appropriate Plans Monitor planned outcomes using evidence based tools and revise plans as required to ensure it remains relevant and meets client s needs Partner with PabMS to identify and work towards the achievement of their goals Share your comprehensive knowledge of the support and services available to PabMS to both internal and external stakeholders to ensure a streamlined, people-centred approach Utilise advocacy and negotiation skills to establish the required network of supports to minimize risk and achieve agreed goals of PabMS Promote & make referrals to appropriate internal and external services as necessary to ensure that PabMS are able to access appropriate services that meet their needs Provide Psycho-social support and referral to external services where required. Promote integrated case management /coordination through collaboration and elicit cooperation with the various sectors Provide evidence based information and advice by tailoring the delivery of such information and education to the individual needs of PabMS Key Result Area 2 Relationship Management Undertake a strength based approach to develop effective partnerships that leads to supported self-management PD MS CONSULTANT FINAL 2014 Page 4

5 Develop strong and collaborative working relationships with key external stakeholders including other service providers Provide accurate and timely information, advice and consultancy services to relevant stakeholders Participate in clinics by providing consultations and assessments at clinics as required Develop constructive working relationships which support open and honest feedback and enhances service delivery Key Result Area 3 Administration Ensure that all required internal and external client documentation is completed and stored in line with policy and procedure and professional standards Utilise Outlook to ensure calendar entries are up to date and accessible to the business. Maintain knowledge of and adhere to current policy and procedure and external regulations applicable to the role Participate in continuous improvement activity Participate in team and departmental meetings as required Undertake regular reporting requirements, as required Actively participates in Performance Review discussion as required. Key Result Area 4 Organisational Accountability Supports the practice leader achieving budget by responsibly utilising resources within the area Use a multi-disciplinary approach to promote and achieve the strategic vision of MSL Facilitate access to and deliver education programs to individuals and groups in conjunction with the Education Coordinator to increase audience s knowledge of MS and its management Undertake data collection, benchmarking and other research and knowledge gathering activities as required to achieve and monitor best practice MS management PD MS CONSULTANT FINAL 2014 Page 5

Selection Criteria Essential: OR A tertiary qualification in Social Work, Occupational Therapy, Physiotherapy or registration as a Registered Nurse Tertiary qualifications & a minimum 4 years experience in the disability sector Where required, holds current registration with Australian Health Professional Regulation Agency (AHPRA) or be eligible for membership with AASW Demonstrated experience in providing clinical services to people with chronic neurological conditions, including MS Demonstrated understanding and an ability to implement the principles of person-centred practice Ability to deliver targeted interventions in a required time (specific to profession) PD MS CONSULTANT FINAL 2014 Page 6

7 Desirable Ability to network and develop effective working relationships Demonstrated ability to work as an effective team member in a multi-disciplinary team environment Exhibits high level of time management and organisation skills including the ability to prioritise tasks autonomously.
