1 ADDENDUM NO. 1 February 6, 2013 TO: PLANS and SPECIFICATIONS for BUILDING IMPROVEMENTS MARCELINE NUTRITION CENTER 229 WEST HAUSER STREET MARCELINE, MISSOURI The Architects Alliance, Inc. 631 West Main Street Jefferson City, Missouri Bidders are hereby informed that the construction plans and/or specifications are modified as follows: GENERAL: 1. Pre-bid meeting minutes and sign-in sheet from the January 23, 2013 Pre bid meeting are attached for use and reference. SPECIFICATIONS: 1. Section ALUMINUM-FRAMED ENTRANCES AND STOREFRONTS MODIFY as follows: a. Paragraph 2.1 ADD Manko 2450 Series Aluminum Storefront and Manko 135 Series Doors to the list of acceptable manufacturers. b. Paragraph 2.5 DOORS REVISE Door Design to be medium stile; nominal minimum width, with 10-inch nominal bottom rail. c. Paragraph 2.9 ALUMINUM FINISHES REVISE finish to be Color Anodic Finish: Dark Bronze. 2. Section DOOR HARDWARE Paragraph 3.7 Hardware Schedule ADD door 110-A to hardware Group No. 4. DRAWINGS: 1. Sheet A101 CLARIFICATION Contractor shall install infill existing opening as required and to match adjacent construction depth as shown on E4/A303. Furnish and install additional drywall, prime and paint to match existing on interior of existing facility as indicated install new resilient base to match existing base where new drywall is installed. 2. Sheet A302 ADD Contractor shall furnish and install resilient transition strip from new floor to existing resilient floor tile. Color as selected by Architect/Owner from manufacturer s full range. 2 3. Sheet A303 ADD Contractor shall furnish and install elastomeric joint sealant around the exterior perimeter of all openings. Color as selected by Architect from manufacturer s full range. Contractor shall furnish and install latex joint sealant around the interior perimeter of all exterior openings. Color as selected by Architect form manufacturer s full range. Contractor shall furnish extruded aluminum subsill under all window openings per manufacturer. 4. Sheet A501 DOOR SCHEDULE ADD Door 110-A to be 3-0 x6-8 x1-3/4 solid core wood door type WD-1 and frame HMF-1. Door hardware for door 110-A shall be Group No Sheet A501 FINISH SCHEDULE ADD R2-4 Resilient base to match existing along south wall of Existing Multi-Purpose 110 and Existing Sitting Area 111 as required due to new construction. Prime and paint new drywall installed in Existing Multi-Purpose 110 and Existing Sitting Area 111 to match existing. 6. Sheet M101 MODIFY as follows: a. ADD location of thermostat on HVAC Floor Plan per attached drawing M Temperature controls and thermostat shall be compatible for dual fuel heating control of the heat pump furnace. b. REVISE Furnace Schedule as shown on attached drawing M101.2 c. REVISE Condensing Unit Schedule to Heat Pump as shown on attached drawing M101.2 ATTACHEMENTS: 1. The following items were discussed. I. Sign in A sign-up sheet was passed around and all attendee s were asked to sign in. II. Introductions of Design Team John Moon a. Owner Representative 1. Mechanical / Electrical / Plumbing Dana Tennill (not present / John Newman / Joe Bushman (not present) III. Project Review a. An attendance sign-in sheet was passed around to all participants. All attendants were asked to sign the pre-bid sign-in as the pre-bid is mandatory for general contractor s wishing to submit bids for the project. b. Invited contractors may obtain bid documents from American Document Solutions, Forum Shopping Center, 1400 Forum Boulevard, Suite 1C, Columbia, Missouri for a refundable 0903

4 $ deposit payable to Marceline Housing Authority. General Contractors will be allowed two sets and Subcontractors and Suppliers one set while supplies last. c. The drawing set includes twenty-seven (27) 24x36 inch drawings. Plans and specifications will be available for viewing at plan rooms including Builders Association and Architect s office and viewable online at planroom.adsmo.net. d. If contractor s have picked up extra sets and are not currently using them, or do not intend to bid the project we ask the drawing sets be returned to American Document Solutions at the earliest convenience. e. Bids are due 2:00 PM Tuesday, February 12, 2013 at the main office of the Marceline Housing Authority located at 229 West Hauser Street, Marceline, Missouri and will be publicly opened and read aloud. f. A 5% Bid Bond is required with all bids. Bonds should be payable to Marceline Housing Authority. Bids shall be good for sixty (60) days. g. The following forms included in the project manual shall also be submitted with bids: Certification by Bidder Regarding Equal Opportunity, Form HUD-950.1, Certification of Bidder Regarding Section 3 and Segregated Facilities, Bidder Qualifications, and Minority and Women s Business Enterprise Utilization Worksheet. h. Project is exempt from sales tax. i. Project is prevailing wage rate in Linn County and Payroll records will be required from the contractor and subcontractors by the Owner. The wage rates will be verified again prior to the bid date by Lance Rains and any updates will be included via addenda. j. Certificates of Insurance and a Performance and Payment Bond in the amount of 100% of the bid will be required of the successful bidder for the project. Insurance requirements are outlined in the project manual including builder s risk insurance. k. The successful bidder shall commence work on the date to be specified in a written Notice to Proceed and Substantially Complete within 150 consecutive calendar days. l. Contractor s shall be responsible for obtaining all building permits for the project. m. 5 Alternate No. Seven Architectural Casework Further information regarding Alternates is outlined in the Alternates section on the drawings. The CDBG requires that Alternates be taken in order they are listed. o. There is one unit prices for the project for unsuitable soil removal by the cubic yard beyond that indicated in the construction documents p. The four allowances for the project are as follows: Allowance No. One Unit Cost Allowance include $ per thousand for face brick. Allowance No. Two Contingency Allowance include $15, for use according to the Owner. Allowance No. Three Testing Allowance include $8, for test and inspections indicated to be provided by the Owner. Contractor shall coordinate. A schedule of anticipated tests and inspections are include in the project manual. Allowance No. Four Bad Weather Days include 45 Bad Weather days within the completion period for the project. q. The Owner and public will occupy the site for the duration of the project. The successful contractor shall take the necessary precautions to protect the public and occupants, and maintain all egress doors throughout the project. r. The Contractor shall notify the Owner of any interruptions of utility services for either project and coordinate these interruptions with the Owners schedule. A. Architectural Review John Moon a. The existing building was constructed in b. The project consists of construction of an approximate 2,200 square foot addition of similar construction and materials to the existing Nutrition Center. Work consists of earthwork, storm drainage, cement concrete paving, lawns and grasses, cast-in-place concrete, unit masonry, metal fabrications, rough carpentry, sheathing, shop fabricated wood trusses, interior architectural woodwork, building insulation, asphalt shingles, EPDM membrane roofing, sheet metal flashing and trim, joint sealants, steel doors and frames, flush wood doors, aluminum entrances and storefront, door hardware, glazing, gypsum board, epoxy floor paint, resilient wall base and accessories, painting, signage, fire extinguishers and cabinets, toilet and bath accessories, mechanical, electrical and plumbing. c. The contractor shall remove the existing sun porch and select roof framing as indicated and necessary for addition. d. Owner will relocate the existing freezer and compressor to the exterior. This work to be coordinated by the Contractor. e. Parking, staging and dumpster areas will need to be coordinated with the Owner. Access to the site is limited and the contractor will be required to patch and repair all damage to the existing lawns, sidewalks, drives and parking lot. IV. Geotechnical / Civil / Structural Review Tom Kelly a. The site under the proposed addition will be undercut by approximately 5-6 feet from the existing building finish floor and the existing soils will need to be reconditioned and placed in lifts per the geotechnical report. A twenty-four inch low volume change zone will be installed under the building slab Page 3 of 4

6 b. Areas of the site will need to be regraded and select portions of the existing sidewalk will need to be removed and replaced. c. Underground utilities servicing the adjacent housing units run through the project site. Relocation of these utilities are included as alternates to isolate the cost. The contractor will need to minimize the interruption of services and coordinate the relocation of utilities with the Housing Authority. d. The addition will consist of concrete foundations and footings with load bearing wood studs and shop fabricated wood trusses. Overbuilt areas of the existing building roof framing will need to be removed, verified and replaced. V. Mechanical / Plumbing / Electrical John Newman a. A new 5-ton forced air HVAC system will service the addition. b. The existing condensing units will be relocated and replaced by the Owner. The contractor will need to coordinate and ensure a route for the refrigerant line is available. c. Two new restrooms with tank type toilets, and wall mounted lavatories are included in the base bid for the project. The work also includes a mop sink and floor drains as indicated. A new water heater servicing the building addition will be provided. d. The sewer line for the addition will tie into the existing sewer line and exit on the south side of the addition. A new 1 water service will be extended from Hauser street to the addition. e. The existing gas service will need to be disconnected and reconfigured as shown on the plans. f. The existing transformer will be relocated as part of the base bid. A new electrical panel will be installed to service the new outlets and lighting for the addition. g. The underground electrical servicing other housing units will be relocated as part of an Alternate bid. VI. Questions a. A contractor inquired if the undercut area under the building addition could be filled in entirely with waste lime in lieu of reconditioning the soil. John Moon indicated he would confirm if this option was acceptable with the geotechnical engineer and clarify via addendum if acceptable. (CLARIFICATION This is NOT acceptable and contractors shall infill the undercut area per the geotechnical report) b. A mechanical contractor inquired about the mechanical system size and unit selection. John Moon indicated he should contact Dana Tennill with Tennill and Associates regarding these concerns and for any clarifications. VII.
