1 April 4, 2016 ADDENDUM 3 TO: FROM: RE: Vendors Vianka Colin, Purchasing Director INVITATION TO BID ITB/FFS-15/16-79 CONSTRUCTION OF A BUILDING FACILITY AT INDIAN LAKE RANGER STATION This addendum is to provide all potential bidders with answers to questions submitted in reference to Invitation To Bid number ITB/FFS-15/ Deletions are struck through, and additions are highlighted. Questions / Answers 1. The landscape shows 17 large tree-type symbols and labels as a hedge 3 ft. in height. Could we get a clarification as to what these items are? The trees shown on the plan have to be 8 ft. in height. Is there a caliper requirement? See the revised plans. 2. There are not specifications provided on the well size, pump size, tank size, etc. Will there be an engineered detailed site plan for this project? See the revised plans. 3. Who is the power company? Has load been calculated for service to be built? Ocala Electric Utility is the utility company. The load shall be calculated by the contractor. 4. Will there be any additional cost to build service and supply O/H to site? This shall be covered by FFS. 5. Where will the O/H service line terminate or be located on site property? It is unknown at this time.
2 ADDENDUM 3 April 4, 2016 Page 2 6. Is the contractor to include U/G costs from pole to service meter can? If so, is there an estimated cost/budget? This shall be the contractor's responsibility; there are no estimates. 7. Will NOVA be responsible for coordination and costs associated with State Fire Marshal as described in ITB? Yes. 8. Is the contractor to include costs associated with surveys/staking/layouts/as-builts, or is owner responsible for all? This shall be the contractor's responsibility. 9. Is contractor responsible for all testing (compactions/concrete/proctors, etc.) costs and should include in bid? Yes. 10. Are we required to use firm supplying Geo-Tech report for testing if required? No. 11. Any other jurisdiction or authority requirements/permits/approvals needed prior (besides DOH)? State Fire Marshall. 12. Will electronic submittals regarding materials be acceptable? Architect to review? Return/review time? Electronic submittals are acceptable. The architect will review and return as soon as possible. 13. References requirement in bid should be for similar work, not roofing as ITB lists, correct? Correct. 14. Civil/Site specifications needed for all well and associated systems equipment. See the revised plans. 15. Are civil/site specifications needed for all fire tank & suppression source systems equipment? A 10,000 gallon fire suppression tank per nfpa Civil/Site Plans? See the attached revised plans. 17. We need plans/details for the pole barn building. There is no pole barn at this time.
3 ADDENDUM 3 April 4, 2016 Page What is the plate height of the exterior walls? The elevations show 9, the sections show 10 4? Which is correct? 9 is correct. 19. I can t find a Hardie bond board freeze, please advise? The band/trim is called a frieze but is just a 10 piece of trim. 20. Sections appear to show a 2" x 6' sub fascia with a 1" x 2' drip. The plans call out a 1" x 6' fascia with a 1" x 2' drip. Will the 1" x 6' be a finished product or wrapped? Is there a sub fascia behind the 1" x 6' or is this the fascia? The project requires a 1" x 6' fascia, 2" x 4' pt sub-fascia. 21. Please provide specs/sizes for the fire suppression tank. See the answer to question Please provide specs on hooking up electric to fire suppression tank. Install electrical for fire suppression tank per NFPA Please confirm offsite electric to onsite electric locations. Unknown at this time. 24. What size water heater does the owner want? Please provide specs. See the electrical plan specifying an instant hot water heater. 25. What are the carpet and tile specs? There are none. Contractor shall submit samples for approval. 26. Are there any specs for backsplash? No Contractor shall submit samples for approval. 27. Are there any specs for cabinets and counters? No. Contractor shall submit samples for approval. 28. Are there any specs on interior framed walls? I assume 2x4 walls. 2" x 4' interior walls. 29. Is there any sound insulation? No. This is not a part of the project. 30. Is proposed fence by contractor or owner? If by contractor are there any specs? This shall be provided by the contractor. It shall be 6' chain link per DFOT index 802.

There is a size discrepancy regarding the size of the administration building on the civil plans vs the architectural plans. See the attached plans. 34. In addition to a bond, is builder s risk also required? Yes. 35. How will the power be brought in? Power will be from underground to the building from the last pole. The Department is waiting on the utility company for more information. 36. Will power locations be provided on the Civil plans? No. Contact the local power company, Ocala Electric Utility. 37. Who is the power service through? Ocala Electric Utility. 38. Who is responsible for electrical meter fees? Contractor. 39. Who is responsible for any testing fees such as soil or otherwise? Contractor. 40. Please provide size and specs for well and septic. Please see the revised plans. 41. What permit related fees is the contractor responsible for? None. 42. What permit related fees is the owner responsible for? All. 43. How should bidders obtain project information such as addendums? Contact the purchasing office at (850) or Vianka.Colin@FreshFromFlorida.com.

Will there be a 2x4 sub fascia for soffit nailer and to help keep 1x6 fascia straight? Yes, 1" x 6' fascia, 2" x 4' pt sub-fascia 46. Is permacast column a split column? We will not be able to install HD to 4x4 inside column? Permacast is structural, is 4x4 needed? This project requires a 4" x 4" inside split PermaCast column. To the extent this addendum gives rise to a protest, failure to file a protest within the time prescribed in Section (3), Florida Statutes, shall constitute a waiver of proceedings under Chapter 120, Florida Statutes.
