2 Sheet 2 f 13 RLD Respnsible Land Disturber RLDA Regulated Land Disturbance Activity R&B Rad and Bridge SWM Strmwater Management SWPPP Strmwater Pllutin Preventin Plan VDOT Virginia Department f Transprtatin VSMP Virginia Strmwater Management Prgram 1.0 BACKGROUND 1.1 Sectin (e) f the 2007 VDOT R&B Specificatins requires all land disturbance activities that disturb 10,000 square feet r greater (2500 square feet r greater in the area defined as Tidewater, Virginia in the Chesapeake Bay Preservatin Act) (see the latest versin f IIM-LD-11) t have a SWPPP. 1.2 The VSMP General Permit fr the Discharge f Strmwater frm Cnstructin Activities (hereafter referred t as the VSMP Cnstructin Permit) als requires a SWPPP fr activities cvered under that permit. While a SWPPP is an imprtant cmpnent f the VSMP Cnstructin Permit, it is nly ne f the many requirements that must be addressed in rder t be in full cmpliance with the cnditins f the permit. Thse persns wh versee r perfrm activities cvered by the VSMP Cnstructin Permit must review and understand all f the cnditins and requirements cntained within that permit. 1.3 Effective July 1, 2013, the DCR Strmwater Prgram, including the Cnstructin Permit Prgram, was transferred t the DEQ. The sectins f the Virginia Administrative Cde (VAC) referenced herein reflect new numbering as a result f the prgram transfer. 2.0 SWPPP APPLICABILITY AND REQUIREMENTS A SWPPP identifies ptential surces f pllutants which may reasnably be expected t affect the strmwater discharges frm the RLDA, and any supprt areas included in the VSMP Cnstructin Permit cverage fr the RLDA, and describes and ensures the implementatin f practices t minimize pllutants in such discharges. Fr the purpses f this IIM, the RLDA is defined as the prpsed cnstructin r maintenance related land disturbing prject r activity that generates the need fr acquiring cverage under the VSMP Cnstructin Permit and/r requires an ESC Plan The required cntents f a SWPPP fr thse land disturbance activities requiring cverage under the VSMP Cnstructin Permit are fund in Sectin II, D f the General Permit sectin f the VSMP Regulatins (9VAC ).

3 Sheet 3 f Except fr the items dealing with the pst cnstructin strmwater management requirements, the majrity f the items that must be addressed in the SWPPP fr land disturbance activities requiring VSMP Cnstructin Permit cverage must als be addressed fr thse land disturbance activities that d nt require VSMP Cnstructin Permit cverage but d require an ESC Plan in accrdance with the requirements f the Virginia ESC Law and Regulatins. 2.4 When the land disturbing activity requires cverage under the VSMP Cnstructin Permit, the SWPPP must als include a cpy f the VSMP Cnstructin Permit, the VSMP Cnstructin Permit Registratin Infrmatin frm LD-445, the VSMP Cnstructin Permit Cntact Infrmatin frm LD- 445A, the SWPPP Certificatin frm LD-445E and the VSMP Cnstructin Permit cverage letter received frm DEQ shwing a prject specific permit number. 2.5 The SWPPP fr the RLDA is t include any n site supprt facilities used exclusively fr the RLDA (e.g., brrw and dispsal sites, the cntractr s strage and fueling areas, etc.) (see the current versin f IIM-LD-242 fr guidance related t SWPPP infrmatin fr supprt facilities). 2.6 Fr thse RLDAs requiring cverage under the VSMP Cnstructin Permit, Sectin II B.1. f the General Permit sectin f the VSMP Regulatins (9VAC ) requires the SWPPP t be signed by a persn s identified in Sectin III K.2 f that same dcument. Fr a State Agency, that persn is the principal executive fficer r his designee. Fr VDOT prjects, that authrity has been delegated t the RLD fr each specific RLDA. 2.7 Many f the items required in the SWPPP are inherently cntained in the cnstructin plans (r ther such dcuments) by means f the ersin and sediment cntrl plans and the pst cnstructin strmwater management plans and in ther VDOT dcuments such as the R&B Standards and Specificatins, which can be incrprated int the SWPPP by reference. 3.0 FORM LD-445E 3.1 Fr thse land disturbing activities requiring cverage under the VSMP Cnstructin Permit, the Cnstructin Permit requires that the SWPPP fr any supprt facilities be included in the permit cverage fr the RLDA be develped and included with the SWPPP fr the RLDA prir t issuance f permit cverage. 3.2 On mst VDOT land disturbing activities, it is the respnsibility f the cntractr r ther such persn perfrming the land disturbance activity t identify the lcatin f the supprt facilities and prvide the SWPPP fr such t the prject engineer/rld fr review and apprval (see the current versin f IIM-LD-246 fr further discussin n supprt facilities).

4 Sheet 4 f Since the VSMP Cnstructin Permit cverage fr VDOT RLDAs is nrmally btained prir t the identificatin f the supprt areas, a mechanism is required whereby the prject files can be dcumented, and DEQ can be assured, that all f the infrmatin fr the supprt facilities, as well as ther required infrmatin nt available at the time the VSMP Cnstructin Permit cverage fr the RLDA is applied fr, has been, r will be, included in the SWPPP fr the RLDA. The mechanism t be used fr this purpse will be SWPPP Certificatin frm LD- 445E Frm LD-445E is als t be used t identify the VDOT persn respnsible fr the inspectin f the ersin and sediment cntrl facilities The DEQ has apprved the signature f the RLD n the LD-445E frm as meeting the SWPPP signatry requirements cntained in Sectin II B.1. f the General Permit sectin f the VSMP Regulatins (9VAC ) Frm LD-445E is t be cmpleted by the VDOT RLD fr all land disturbing activities requiring VSMP Cnstructin Permit Cverage and/r an ESC Plan/SWPPP A cpy f cmpleted frm LD-445E is t be retained with the ther SWPPP dcuments fr the RLDA Fr thse land disturbing activities requiring cverage under the VSMP Cnstructin Permit, the VDOT RLD is t send the cmpleted LD-445E frm t the District VSMP Cnstructin Permit Crdinatr fr inclusin with ther VSMP Cnstructin Permit data that is submitted mnthly t the Central Office VSMP Cnstructin Permit Crdinatr. 4.0 SWPPP GENERAL INFORMATION SHEETS 4.1 In rder t prvide a clear understanding f what is required in a SWPPP and t prvide a reference as t where thse items are lcated within the cntract/cnstructin dcuments, a set f SWPPP General Infrmatin Sheets has been develped. The SWPPP General Infrmatin Sheets prvide a summary f the infrmatin required in Sectin II D. f the General Permit sectin f the VSMP Regulatins (9VAC ) and, where nt included n the General Infrmatin Sheets, prvide a reference t where that infrmatin can be fund within the cntract/cnstructin dcuments fr the RLDA (e.g., the cnstructin plans r ther such dcuments, the VDOT R&B Standards and/r Specificatins, cntractr supplied dcuments, etc.). 4.2 The SWPPP General Infrmatin Sheets incrprate many f the ntes previusly included in the ESC General Ntes as well as thse necessary t identify and describe the pst cnstructin strmwater management plan fr the RLDA (if applicable).

5 Sheet 5 f The SWPPP General Infrmatin Sheets are t be included in the cnstructin plan set (r ther such dcuments) fr all land disturbance activities requiring VSMP Cnstructin Permit cverage and/r an ersin and sediment cntrl plan. Cmpletin and inclusin f the SWPPP General Infrmatin Sheets in the cntract dcuments satisfies ne f the many requirements cntained in the VSMP Cnstructin Permit. Thse persns wh versee r perfrm activities cvered by the VSMP Cnstructin Permit must review and understand all f the cnditins and requirements cntained within that permit. 4.4 The SWPPP General Infrmatin Sheets are updated frm time t time t clarify and/r include additinal requirements as a result f changes t the VSMP Regulatin, the VSMP Cnstructin Permit r VDOT s Apprved ESC and SWM Standards and Specificatins. Prir t finalizatin f the cnstructin plans r ther such dcuments fr a prpsed land disturbance activity, the Prject Manager r ther such prject authrity is t verify that the mst recent SWPPP General Infrmatin Sheets are included. 4.5 The SWPPP General Infrmatin Sheets have been develped in tw frmats as fllws: Available in the CADD sheet 2000 cell library (referenced as SWPPP1, SWPPP2 & SWPPP3) fr use with thse land disturbance activities that have a frmal set f cnstructin plans (i.e., thse develped under a Minimum (M) Plan r Cmplete (C) Plan Prcess) Available in Falcn under the Engineering Services eng-scr directry (N Plan sub-directry) as an 8.5 X 11 letter size wrd dcument fr use with thse land disturbance activities develped under a N Plan (N) Prcess r fr nn-rutine maintenance activities. 4.6 The SWPPP General Infrmatin Sheets are t be cmpleted by the ESC Plan Designer, the Hydraulic Engineer r ther such persn wh has the respnsibility fr develping the ESC and pst cnstructin SWM Plan (if applicable) fr the RLDA. 4.7 Infrmatin required by thse ntes n the SWPPP General Infrmatin Sheets designated with an asterisk is t be develped/prvided by the cntractr. Infrmatin required by thse ntes n the SWPPP General Infrmatin Sheets designated with a duble asterisk is t be prvided/cmpleted by the VDOT RLD All infrmatin/ntes in Sectins I thrugh VI f the SWPPP General Infrmatin Sheets are applicable t land disturbance activities requiring cverage under the VSMP Cnstructin Permit. 4.9 Fr land disturbance activities requiring an ESC Plan but exempt frm the VSMP Regulatins r the need fr cverage under the VSMP Cnstructin Permit, the infrmatin nted n the SWPPP General Infrmatin Sheets in Sectin IV and Sectin VI, is, typically, nt applicable. Thse sectins, as well as any ther ntes/infrmatin in ther sectins f the SWPPP General

6 Sheet 6 f 13 Infrmatin Sheets nt applicable t a specific land disturbance activity shuld be deleted, struck thrugh r nted as NA (i.e., nt applicable t the land disturbance activity) Sectin V f the SWPPP General Infrmatin Sheets requires a lcatin map that clearly identifies the prject lcatin and all surface waters, such as rivers, streams, lakes, pnds, etc. (including names where applicable), within a ne mile radius f the prject site. Instructins fr placing a lcatin map in Sectin V can be fund at the web addresses nted n the SWPPP General Infrmatin Sheets. Thse unable t access the nted sites shuld cntact the District r Central Office Hydraulics Sectin, as apprpriate, fr assistance. Other methds that prduce the desired map may be used in lieu f thse nted The permanent BMP infrmatin (when applicable) in Sectin VI is t be cmpleted by the Hydraulic Engineer (r ther such persn develping the pst cnstructin SWM Plan) and is t be based n the pre-cnstructin design. This infrmatin is t be updated when any changes t the pst cnstructin SWM Plan are authrized during the cnstructin phase f the activity. Such changes are t be made as a frmal revisin t the plans. When submitting a request fr terminatin f the VSMP Cnstructin Permit cverage, the RLD is t use the infrmatin in the Permanent BMP table(s) in cmpleting the BMP infrmatin sectin n frm LD-445D Sme f the ntes n the General Infrmatin Sheets require prject specific user input. Sme examples f the infrmatin required are as fllws: Sectin I General Nte 1 - Activity Descriptin (Examples) This radway cnstructin prject cnsists f adding tw additinal parallel lanes t an existing tw lane rural radway facility. This radway cnstructin prject cnsists f imprving an existing urban radway intersectin by adding left turn and right turn lanes. This radway cnstructin prject cnsists f replacement f an existing bridge with a new bridge and imprvements t the existing radway appraches. This radway cnstructin prject cnsists f widening an existing urban street and adding additinal turn lanes. This radway maintenance prject cnsists f re-grading and enlarging the radside ditches and replacing drainage pipes alng an existing rural radway. The cnstructin plans/dcuments. 2. The SWPPP General Infrmatin Sheets (with all ntes cmpleted with apprpriate infrmatin). 3. The ESC Plan. 4. The pst cnstructin SWM Plan (if applicable). 5. The VDOT R&B Standards and Specificatins, Supplemental Specificatins, Special Prvisins and Special Prvisin Cpied Ntes. 6. A cpy f the VSMP General Permit Fr Discharges Of Strmwater Frm Cnstructin Activities (the Cnstructin Permit) (when applicable). 7. A cpy f the VSMP Cnstructin Permit cverage letter received frm DEQ (when applicable). 8. A cpy f the VSMP Cnstructin Permit Registratin Infrmatin frm LD-445, (when applicable). 9. A cpy f the SWPPP Certificatin frm LD-445E. 10. Dcuments required t be develped/prvided by the cntractr fr ersin and sediment cntrl and strmwater pllutin preventin assciated with any supprt facilities t be included in the VSMP Cnstructin Permit cverage fr the RLDA. 11. A Recrd Set f Plans (see Sectin 6.2 f this IIM fr mre infrmatin) 5.2 All dcuments related t the SWPPP fr a RLDA (except fr the ESC and SWM design cmputatins and supprting data) shall be maintained at the activity site and shall be readily available fr use by thse with SWPPP implementatin respnsibilities. All dcuments related t the SWPPP fr a RLDA shall be

SWPPP related infrmatin nt included in the cnstructin plans/dcuments, the VDOT R&B Standards, Specificatins, Supplemental Specificatins, Special Prvisins r Special Prvisin Cpied Ntes and the ESC and SWM design cmputatin files is t be kept in a designated separate paper and/r electrnic file. Where n facilities are available at the activity site t maintain the SWPPP dcuments, they are t be kept at a lcatin cnvenient t the activity site where they will be readily available fr use by thse with SWPPP implementatin respnsibilities and wuld be available fr review by thers upn request during nrmal business wrking hurs. Where the SWPPP dcuments are nt stred n site, a cpy f such dcuments, except fr the ESC and SWM engineering calculatins and dcumentatin, shall be in the pssessin f thse with day t day peratinal cntrl ver the implementatin f the SWPPP (e.g. the VDOT RDL, the VDOT ESC Inspectr, the cntractr s ESCCC persn, etc.) whenever they are n site. 6.0 SWPPP COMPONENTS 6.1 The fllwing list utlines the majr cmpnents f a SWPPP, the persn(s) respnsible fr ensuring that the cmpnent is addressed in the SWPPP fr a RLDA and hw that cmpnent is addressed in the cnstructin plans r ther such dcuments fr a VDOT land disturbing activity A cpy f the VSMP Cnstructin Permit registratin statement and cverage letter (when applicable). The RLD ensures that a cpy f the VSMP Cnstructin Permit Registratin Infrmatin frm LD-445, a cpy f the SWPPP Certificatin frm LD-445E and the VSMP Cnstructin Permit cverage letter received frm DEQ is maintained in the SWPPP file fr the RLDA A cpy the VSMP Cnstructin Permit (when applicable). The RLD ensures that a cpy is maintained in the SWPPP file fr the RLDA. A cpy f the VSMP Cnstructin Permit can be btained at: uctin_permit_var10.pdf A narrative descriptin f the nature f the cnstructin activity, including the functin f the prject. The ESC Plan Designer incrprates prject specific infrmatin int the apprpriate nte(s) n the SWPPP General Infrmatin Sheets fr the RLDA The intended sequence and timing f activities that disturb sils at the site (e.g., grubbing, excavatin, grading, utilities and infrastructure installatin). The Cntractr r ther such persn develps/prvides prject specific infrmatin. The RLD ensures that the infrmatin is maintained in the SWPPP file fr the RLDA.

The RLD ensures that the infrmatin is maintained in the SWPPP file fr the RLDA Estimates f the ttal area expected t be disturbed by excavatin, grading, r ther cnstructin activities. The ESC Plan Designer btains the infrmatin and incrprates it int the apprpriate nte n the SWPPP General Infrmatin Sheets fr the RLDA A descriptin f any ther ptential pllutant surces, such as vehicle fueling, strage f fertilizers r chemicals, sanitary waste facilities, etc. The Cntractr r ther such persn develps/prvides prject specific infrmatin. The RLD ensures that the infrmatin is maintained in the SWPPP file fr the RLDA Identificatin f the nearest receiving waters at r near the cnstructin site that will receive discharges frm disturbed areas f the RLDA. The ESC Plan Designer determines the infrmatin and incrprates it int the apprpriate nte n the SWPPP General Infrmatin Sheets fr the RLDA The lcatin and descriptin f any discharge assciated with industrial activity ther than cnstructin at the site. This includes strmwater discharges frm dedicated asphalt plants and dedicated cncrete plants that are cvered by the VSMP Cnstructin Permit fr the RLDA. This infrmatin is cvered by a standard nte n the SWPPP General Infrmatin Sheets A legible general lcatin map (e.g., USGS quadrangle map, a prtin f a city r cunty map, r ther map) with sufficient detail t identify the lcatin f the cnstructin activity and surface waters within ne mile f the cnstructin activity. The ESC Plan Designer r the Hydraulic Engineer develps and incrprates the lcatin map int Sectin V f the SWPPP General Infrmatin Sheets fr the RLDA A legible site map/plan identifying the fllwing items: Directins f strmwater flw and apprximate slpes anticipated after majr grading activities. The ESC Plan Designer ensures that the apprpriate infrmatin (e.g., grading cnturs, typical sectins, prfiles and/r crss sectins) is included in the cnstructin plans r ther such dcuments fr the RLDA Areas f sil disturbance and areas f the site which will nt be disturbed. The ESC Plan Designer ensures that the apprpriate infrmatin (e.g., plan view cnstructin limits and/r typical sectins/crss sectins) is included in the cnstructin plans r ther such dcuments fr the RLDA.

10 Sheet 10 f Lcatins f majr structural and nnstructural cntrl measures identified in the SWPPP, including thse that will be permanent after cnstructin activities have been cmpleted. The ESC Plan Designer ensures that the apprpriate infrmatin is included in the cnstructin plans r ther such dcuments fr the RLDA Lcatins where stabilizatin practices are expected t ccur. The ESC Plan Designer ensures that the apprpriate infrmatin (e.g., plan view cnstructin limits and/r typical sectins/crss sectins) is included in the cnstructin plans r ther such dcuments fr the RLDA Lcatins f surface waters. The ESC Plan Designer ensures that the apprpriate infrmatin is included in the cnstructin plans r ther such dcuments fr the RLDA Lcatins where cncentrated strmwater discharges frm the cnstructin site. The ESC Plan Designer ensures that the apprpriate infrmatin is included in the cnstructin plans r ther such dcuments fr the RLDA Lcatins f any supprt areas (e.g., material, waste, brrw r equipment strage areas) that are t be included in the permit cverage and the SWPPP fr the RLDA. The Cntractr r ther such persn prvides prject specific infrmatin. The designated RLD ensures that the infrmatin is maintained in the SWPPP file fr the RLDA Lcatins f ther ptential pllutant surces, such as vehicle fueling, strage f chemicals, cncrete wash-ut areas, sanitary waste facilities, including thse temprarily placed n the cnstructin site, etc. The Cntractr r ther such persn prvides prject specific infrmatin. The designated RLD ensures that the infrmatin is maintained in the SWPPP file fr the RLDA Areas where final stabilizatin has been accmplished. The Cntractr r ther such persn prvides prject specific infrmatin. The designated RLD ensures that the infrmatin is maintained in the SWPPP file fr the RLDA The SWPPP shall include a descriptin f all cntrl measures that will be implemented as part f the cnstructin activity t minimize pllutants in strmwater discharges. Fr each majr cnstructin activity identified, the SWPPP shall clearly describe apprpriate cntrl measures, the general sequencing during the cnstructin prcess in which the cntrl measures will be implemented, and which peratr (i.e., cntractr) is respnsible fr implementatin f the cntrl measure.

11 Sheet 11 f 13 The ESC Plan Designer/Hydraulics Engineer develps the ESC Plan and the SWPPP fr inclusin in the cnstructin plans/dcuments fr the RLDA. The Cntractr r ther such persn develps/prvides prpsed revisins t the ESC Plan and the SWPPP as necessary t meet differing field cnditins r cnstructin sequencing. The VDOT ESC Inspectr reviews and the VDOT RLD apprves any changes t the ESC Plan and the SWPPP. The RLD ensures that all required infrmatin is maintained in the SWPPP file and/r dcumented n the Recrd Set f Plans (see Sectin 6.2 f this IIM fr additinal infrmatin) fr the RLDA in accrdance with Sectin (e) f the 2007 Rad and Bridge Specificatins The SWPPP shall include a descriptin f all ersin and sediment cntrl measures (including supprting calculatins) that will be installed during the cnstructin prcess t cntrl any ptential pllutants in strmwater discharges frm the cnstructin site. The ESC Plan Designer develps the ESC Plan and required calculatins fr the RLDA. The ESC Plan is incrprated int the cnstructin plans/dcuments fr the RLDA. The ESC calculatins are maintained in the prject hydraulic files and the lcatin f such files is dcumented by the ESC Plan Designer in the apprpriate nte n the SWPPP General Infrmatin Sheets fr the RLDA The SWPPP shall describe measures t prevent the discharge f slid materials, including building materials, garbage, and debris t state waters, except as authrized by a Clean Water Act 404 permit. This infrmatin is cvered by a standard nte n the SWPPP General Infrmatin Sheets The SWPPP shall describe cntrl measures used t cmply with applicable state r lcal waste dispsal, sanitary sewer r septic system regulatins. This infrmatin is cvered by a standard nte n the SWPPP General Infrmatin Sheets The SWPPP shall include a descriptin f cnstructin and waste materials expected t be stred n site, with updates as apprpriate. The SWPPP shall als include a descriptin f cntrls, including strage practices, t minimize expsure f the materials t strmwater and fr spill preventin and respnse. The Cntractr r ther such persn develps/prvides prject specific infrmatin. The designated RLD reviews and apprves the infrmatin and ensures that cpies f such are maintained in the SWPPP file fr the RLDA The SWPPP shall include a descriptin f, and all necessary calculatins supprting, all pst-cnstructin strmwater management facilities (BMPs) that will be installed prir t the cmpletin f the cnstructin prcess t cntrl pllutants in strmwater discharges after cnstructin peratins have been cmpleted. The VDOT Certified ESC Inspectr verifies inspectin infrmatin n Frm C-107 and the RLD ensures that all f the C-107 frms are maintained in the SWPPP file fr the RLD Where the RLDA discharges t a surface water with an apprved (as f the effective date f the VSMP Cnstructin Permit) Ttal Maximum Daily Lad (TMDL), the pllutant identified in any Waste Lad Allcatin (WLA) assigned t a cnstructin activity must be identified in the SWPPP. The SWPPP shall include strategies and cntrl measures t ensure cnsistency with the assumptins and requirements f any TMDL WLA that applies t the peratr's discharge.
