Ivy Street Canby, OR RE: ASBESTOS ABATEMENT CLOSE OUT REPORT 2015 Eccles Elementary School Hallway Remodel Dear Mr. Layman: Abatement was performed in the Hallways at Eccles Elementary School located at 562 NW 5 th Ave. in Canby, OR from July 15-July 22, Approximately 6,300 square feet of carpet over asbestos containing floor tile and mastic was removed throughout the hallways. Professional Minority performed abatement of the materials identified by Kelsay Environmental and with the use of the Eccles Elementary School AHERA Management Plan. All workers were Oregon-state certified asbestos workers that were verified by Kelsay Envronmental. Methods used were full isolation containment procedures in accordance with 29 CFR Kelsay Environmental managed the abatement and performed area air monitoring outside containment during the abatement activities. The removal area was conducted using a negative pressure enclosure. All samples (except TEM air samples and Quality Control Samples) were analyzed via the NIOSH 582 method by Kelsay Environmental. TEM air clearance samples were analyzed by an accredited NVLAP Laboratory. The enclosure was maintained at level of 0.02 of negative pressure on the water column per DEQ regulations. TEM air clearance sampling was conducted and lab results indicate no asbestos detected. The outside work area air samples were collected and tested by Kelsay Environmental, all tested samples were below the DEQ clearance level of 0.01 fibers/cc.
