1 INDIGENOUS PROJECT OFFICER (CTG) WIDE BAY (Ongoing, full time) The Institute for Urban Indigenous Health Ltd (IUIH) was established to provide a coordinated and integrated approach to the planning, development and delivery of primary health care services to Aboriginal and Torres Strait Islander populations within the South East Qld Region. The IUIH is a lead agency working in partnership with key stakeholders to support the effective implementation of the Council of Australian Governments Closing the Gap initiatives. The Project Officer will provide support to mainstream general practice to improve access to private general practice for Aboriginal and/or Torres Strait Islander people. This will include working directly with a defined number of approved mainstream general practices in the familiarisation of CTG initiatives designed to support these general practices in the provision of appropriate and timely care for Indigenous people in their region, especially those at risk of or who already have a chronic disease. This is a great opportunity to join a committed multi-disciplinary team working across the Moreton Bay region and other IUIH premises as required. This position is based at Caboolture. The Regional Services Unit provides direct clinical services, contracted Allied Health professional services, programs and care coordination support to IUIH and Member Services to improve access for local Aboriginal and Torres Strait Islander peoples to a comprehensive and effective range of extended Health services and clinical programs. The Integrated Team Care (ITC) program aim is to improve access to the most appropriate health services for Aboriginal and Torres Strait Islander people at the right time and in the right place with a view to reducing avoidable acute hospital admissions or readmissions and emergency department presentations; improved use of the healthcare system and to support people to take part in a healthy lifestyle. 3 Role Purpose Role Responsibilities The purpose of the position is to provide support to mainstream general practice to improve access to private general practice for Indigenous people. This will include working directly with a defined number of approved mainstream general practices in the familiarisation of CTG initiatives designed to support Indigenous access to private general practices; and supporting mainstream general practices in the provision of appropriate and timely care for Indigenous people in their region, especially those at risk of or who already have a chronic disease. Planning the CTG Program In conjunction with the Regional Program Manager Lead the periodic monitoring and review of new strategic geographic target areas by the CTG Team; including analyse demographics of indigenous population in Metro South region on a no compete basis with AICCHS client catchment areas Lead the assigning and prioritisation of regional GP Practices Allocate geographic and GP Practices targets to Project Officer and IOWs, as required Lead the Client Referral Pathways policy development and review processes Lead the review of GP Partner Agreements Lead the Client Case Planning and Allocation processes Develop CTG Team Work Plan covering all elements of GP Practice management and Community Engagement activities Strategies to improve GP Practices Service Delivery GP Practice Agreements Analyse own target Partner Practices and develop a Report that o analyses the existing systems, procedures and technologies in each Practice; o analyses baseline client data; o identifies value-add benefits the IUIH/CTG Team can contribute o identifies Care Coordinator and IOW value-adding roles Sign-up Practices to Partnership Agreements in the agreed priority locations Conclude six monthly Action Plans with these partner Practices, specifying inputs from each CTG Team member. Provide CTG Team Referral Forms in hard copy and e-copy to mainstream service providers. Record on MMEx each client referred from each partner Practice [NOT mainstream service provider] upon referral Coordinate other IUIH programs/staff engagement with partner Practices Broker follow-through access for IOWs to client-facing personnel in these partner Practices appropriate for Aboriginal and Torres Strait Islander referred clients. GP Practice Process/System Improvements With the partner Practice Manager and GPs, install specific enhancements to systems and approaches that encourage access by - and enhance quality of services to Aboriginal and Torres Strait Islander people particularly in the area of chronic disease management. Assist where necessary target Practices to achieve accreditation for PIP/IHI and

4 PBS co-payment. Disseminate documentation to Practices, to pharmacies and to other mainstream and Indigenous service providers on the PIP IHI arrangements and the PBS co-payment scheme arrangements. Personally provide or personally coordinate training to one or all of GPs/Practice Managers/Nurses/ Administration staff in PIP/IHI and PBS copayment. Personally provide In-Practice support for prioirty partner Practices on one or all of systems that maximise uptake of health assessments, GP management plans, team care arrangements, and follow up items associated with these including brokering on-the-job support from the CTG Team s Care Coordinator. Assist each partner Practice to implement valid self-identification procedures for Aboriginal and Torres Strait islander people. Improve Cultural Sensitivity of Practices Based on an analysis of each Practice s operations, assist IUIH to design and continuously improve the contents of a cultural training package as well as its delivery methodology suitable to the operational environments of partner Practices. Organise cultural safety training delivery for prioirty Practices Provide one to one advice and support within key identified mainstream general practices to enhance knowledge and skills in providing culturally appropriate health services for Aboriginal and Torres Strait Islander people Strategies to improve Service Delivery of other Mainstream Primary Care Providers Position the CTG Team with effective leaders, managers and staff employed in key stakeholder organisations in each catchment area, including relevant service providers, businesses and community organisations. Broker follow-through access for IOWs to client-facing personnel in these key stakeholder organisations appropriate for Aboriginal and Torres Strait Islander referred clients. Acquire and maintain the currency of resource materials from mainstream service providers relevant to Aboriginal and Torres Strait Islander referred clients. Communicate the CTG Team s Referral Pathways Policy to mainstream service providers. [Noting that CTG support will only provide support in the context of a health-related Care Coordination Plan and the client s links to priority partner Practices. The CTG Team is not a general and geographically untargeted psycho-social service.] Provide CTG Team Referral Forms in hard copy and e-copy to mainstream service providers. Record on MMEx each client referred from each mainstream service provider upon referral. Disseminate documentation to pharmacies and to other mainstream and Indigenous service providers on the PIP IHI arrangements and the PBS copayment scheme arrangements Community Engagement Strategies

5 Identify key community groups in target areas Design and document a Community Engagement Plan for the catchment area of each partner Practice that covers off mainstream service providers and Aboriginal organisations. Implement activities for the Community Engagement Plan, including with assistance from the appropriate ATSIOW as approved by the Regionall Program Manager, in a personal work plan. As part of this personal work plan, actively promote awareness in these priority geographic locations of the CTG Team, the CTG Program, and the CTG partner strategy and its implementation Administration and Reporting Adhere to expenditures and budget requirements in line with IUIH policies and procedures Keep accurate, current case notes, statistics and other client records and files, as required. Ensure data entry of client information into relevant management system (MMeX) Maintain a weekly Activity Diary of all non-client specific activities Provide evaluations and reports on mainstream services capacity and risks as required Develop and periodically update a Report to the Regional Program manager that identifies the other IUIH programs/staff that the Project Officer can access to engage with partner Practices. Demonstrate evidence in fortnightly template reports to the Program manager of practical examples where other IUIH projects/staff have engaged with partner Practices Relationship Management Foster productive working relationships with other IUIH staff in the delivery of seamless support services Actively foster productive working relationships with local networks, government departments, other service providers and suppliers to promote IUIH and remain abreast of emerging issues. Encourage cross-functional collaboration to achieve the best outcome for the organisation. Champion IUIH s Ways Statement both internally and externally to the wider community. Information Management Undertake reporting activities together with other relevant documentation within the scope of your role. Maintain the information flow that supports reliable data and documentation in your area of responsibility. Participate in business planning processes and policy and procedure formulation and improvement within your area of responsibility. Wear/use and maintain/care for personal protective clothing and/or equipment provided and report any defects. Use lifting equipment and assistive devices as required. Participate in rehabilitation process if injured or ill due to work. Demonstrate leadership in IUIH s Work Health Safety goals and objectives remaining complaint wuith WHS Legislation and IUIH s policies and procedures. Carry out your duties safely in accordance with your ability and competence. Other duties consistent with the position where required and/or requested by your Manager. IUIH can direct you to carry out duties which it considers are within your level of skill, competence and training at any time. Role Requirements Essential Current Blue Card (working with children and young people) or willingness to apply for a Blue Card, unless AHPRA registered. Any changes to criminal history should be advised to the supervisor as soon as practicable Current First Aid Certificate Current C Class Drivers Licence (Qld) Ability to travel across South East Queensland (SEQ) as required. Qualifications and Experience Qualifications in Community services or primary health care preferred Desirable work experience similar Health or Community engagement/development programs. Desirable Demonstrated knowledge of the PIP IHI arrangements.

7 Demonstrated knowledge of the MBS co-payment scheme arrangements Demonstrated knowledge of health assessments, GP management plans, team care arrangements, and follow up items associated with these Demonstrated understanding of the general practice sector and an ability to work with mainstream primary care providers Demonstrated ability to take direction, determine priorities and manage own workload in order to meet agreed timelines and objectives Demonstrated project management skills and experience High level communication skills that demonstrate the ability to effectively negotiate and liaise with clients and members of the community, health professionals and GP clinic staff in the provision of professional quality client service.
