1 HealthPathways Manager (Maternity Relief) Location Newcastle HNECC PHN Region Part-time 0.7 FTE Fixed Term till 30 September 2017 HNECC PHN is seeking a temporary part-time HealthPathways Manager to join our expanding team and cover a period of maternity relief. The purpose of the role is to provide effective leadership and management of all aspects of the HNECC HealthPathways program, while ensuring the team contribute effectively to the strategic and operational objectives of the program. The HealthPathways Manager will also be responsible for developing and maintaining collaborative relationships with key stakeholders in the program. 2 Position Title: Location: Reporting to: Direct Reports: Position Classification: Delegated Authority: HealthPathways Manager Flexible (Newcastle or Central Coast preferred) Executive Manager Quality, Commissioning & Performance HealthPathways Project Officers HealthPathways Administration Support Officer GP Clinical Editors Award-free Unclassified As defined in the Delegation Policy Purpose of Position HealthPathways is an online health information tool that supports primary care at the point of care. It is a collaborative program that assists clinical redesign and links patients to the best treatment options with local or specialist services in the most timely and cost efficient manner. The HealthPathways Manager is responsible for providing effective leadership and management of all aspects of the HNECC HealthPathways program, while ensuring the team contribute effectively to the strategic and operational objectives of the program, working within the resources available. The HealthPathways Manager will also be responsible for developing and maintaining collaborative relationships with key stakeholder in the program, including but not limited to general practitioners, general practice and primary care staff, Local Health District staff employed by HNELHD and CCLHD, Streamliners and Alliance Partners as part of the Hunter Alliance initiative. The HealthPathways Manager supports the strategic objectives of HNECC by providing leadership to the clinician led HealthPathways program. This will ensure the program is developed and embedded across the region to maximise local relevance and enable streamlining of referrals to provide appropriate care and redistributes health care services were possible. Key Responsibilities Operational Management Contribute to the development of strategic and operational plans for the HealthPathways program with the identification of key performance indicators for the program Provide effective leadership, management and direction to the HealthPathways team Coordinate the development, maintenance and continual quality improvement of HealthPathways ensuring it remains relevant and useful to health professionals and collaborative partners Support and assist with writing content for HealthPathways in liaison with Streamliners and other stakeholders Develop budget proposals that reflect the needs of the program and maintain financial management of the program Assist with the development and implementation of ehealth processes that relate to HealthPathways to improve integration opportunities across the region, including ereferrals Undertake required aspects of human resource management within the team, including regular performance appraisals in consultation with the Human Resource Manager
3 Participate in quality improvement activities, local evaluation measures and wider evaluation opportunities that may present within the HealthPathways community Actively use the pathways administration system (DOT) to manage pathways develop, triage and allocate feedback, produce reports and share pathways content within the HealthPathways community Produce reports as required relating to the programs performance and against KPI targets Representation and stakeholder relationships Represent HNECC PHN to key external stakeholders including but not limited to, general practitioners, specialist, practice and primary care staff, LHD staff employed by HNELHD and CCLHD, Hunter Alliance staff, and other health service organisations Maintain a partnership with Streamliners Develop and foster effective engagement structures for consultation with key stakeholders and other health service organisations for the purpose of developing HealthPathways and ensuring that the program remains relevant and useful to health practitioners Provide formalised facilitation of workgroup meeting to ensure positive outcomes are achieved and documented in the pathways development process Communicate opportunities for redesign and/or improvements that emerge from pathway development workgroups Develop and oversee educational and promotional activities to promote HealthPathways to internal staff and external stakeholders Professional Development Maintain skills in operational and strategic management Develop and maintain a broad level of knowledge of primary care health systems and factors influencing the sectors and processes relevant to the sector, with a focus on improved coordination, integration and development Clinical Governance Ensure clinical governance principles underpin the HealthPathways program, and are reflected in the content and structure of the site Work Health and Safety While at work, a worker must: Take reasonable care of his or her own health and safety and take reasonable care that his or her acts or omissions do not adversely affect the health and safety of other persons Comply so far as the worker is reasonably able, with any reasonable instruction by management and comply with HNECC policies and procedures relating to health and safety Proactively address any issues that may adversely affect the health and safety of any persons at HNECC Other Have access to confidential data and information, and to preserve the confidentiality of these in accordance with relevant policies and procedures
4 Display an attitude and behave in a manner that is in keeping with the HNECC PHN Core values and guiding principles (respect, innovation, accountability, integrity and cooperation) Identify and develop enhancements and add value to the existing processes through the development of continuous improvement opportunities Provide a no wrong door service approach to enquires which is courteous, friendly, supportive and professional to achieve a prompt response Maintain an awareness of Indigenous health issues and an ability to work in a culturally sensitive manner with an ability to support team members working in a cross cultural environment Selection Criteria Essential Criteria Relevant tertiary qualification in a health related industry (e.g. nursing, allied health or health management) Demonstrated experience in managing an effective team to achieve outcomes Proven ability to coordinate and facilitate working groups with a range of health professionals to achieve outcomes Highly developed organisational and change management skills Excellent interpersonal and communication skills, and a demonstrated ability to network and work collaboratively with a wide range of professionals and stakeholders Demonstrated experience in undertaking complex activities in short timeframes Ability to work independently, set objectives and meet these within appropriate timeframes Ability to undertake change and its implementation within the workplace Highly developed skills and experience in the use of computer applications, in particular Microsoft Office applications and the HealthPathways administration system (DOT) Desirable Criteria Demonstrated project management or change management experience Broad experience in a range of health care settings Sound knowledge of the HealthPathways program including a working knowledge of the pathways administration system (DOT) Special Conditions Some out of hours work on evenings and weekends will be required (e.g. attendance at community forums or meetings) for which flexible working hours may be negotiated with manager Regular travel required including overnight travel Appointment Pre-requisites Certification of tertiary qualifications and professional memberships Verification of current NSW drivers licence Evidence of access to a comprehensively insured motor vehicle National Police Check
