1 FLORIDA DEPARTMENT OF TRANSPORTATION ADDENDUM NO. 1 DATE: 2/18/2011 RE: BID #: ITB-DOT-10/ GB BID TITLE: GADGET GRIPS OPENING DATE: 3/1/2011 Notice is hereby given of the following changes to the above-referenced BID/RFP: ITEM 1: QUESTIONS AND ANSWERS ARE ATTACHED. ITEM 2: District Mailing Addresses and Amount Per District for Promotional/tem Bidders/Proposers must acknowledge receipt of this Addendum by completing and returning to the Procurement Office, by no later than the time and date of the bid/proposal opening. Failure to do so may subject the bidder/proposer to disqualification. 2 2011 CTST Unified Campaign ITB Questions from Potential Vendors 1. Will alternates be accepted? FDOT Traffic Safety Office Response: No Ma am, no alternates will be accepted. 2. What FOB: address do you want me to use? FDOT Traffic Safety Office Response: Would like the potential vendor to include drop shipping the gadget grip promotional item to the CTST District Coordinators. A list of these coordinators w/addresses is attached. 3. Upon researching the gadget grip product, I am seeing that the dimensions are slightly smaller than on your bid request, 2 1/8" X 1 1/4" instead of 2 ¼ x 1 ¼ please see below and confirm the size of the item. Gadget Grips PEARL Rectangle (4-Color Process) Dimensions: 2 1/8" X 1 1/4" Imprint Area: Entire area, provide 1/4" bleed. FDOT Traffic Safety Office Response: The above dimensions are acceptable. 4. The item is packaged as follows: Please explain your request of bundles of 2,500. FDOT Traffic Safety Office Response: The bundles include the following options: 150, 250, 500, 1000, and We choose the 2500 bundle to get the best price for this promotional item. We do want the individual cellophane bag w/instruction booklet per each item. 5. What is the total quantity needed? 2,500 total divided between seven districts or 2,500 shipped to each of the seven districts to total 17,500? The attachment has the District point of contact addresses and the amount they indicated that they would like to use towards this statewide campaign. 6. Is this section of your bid sheet (see below) to include ALL costs, including drop shipping and all set-up and product costs, etc. Everything on the one line and nothing itemized? FDOT Traffic Safety Office Response: Please see attached, it contains the answer to your question. 7. You will be supplying full color artwork at size, correct? FDOT Traffic Safety Office Response: No, we re leaving this up to the vendor. There is only two colors, black font with a white background. Of course, we want the artwork to fit within the dimensions discussed above in question #1. 2

3 8. Is it one gadget grip per backing card? They typically come 2 or 4 units to a card with instructions that is inserted into a cellophane bag. Can you clarify how you would like them packaged? FDOT Traffic Safety Office Response: We want to have 1 unit inserted into a cellophane bag. 9. TOTAL QTY? Did you want only 2,500 units bid? Noted they are to be bundled in 2,500 units but what is the TOTAL number of units to be produced? Obviously, the more you do the cheaper your cost would be. FDOT Traffic Safety Office Response: Please use the attached document to know who much money has been allocated per district office, the district locations for drop shipment, and the quantity of gadget grips per district. 10. Drop Ships: How many units are going to each location? Should the freight/set ups etc, be included in the bundle price? FDOT Traffic Safety Office Response: The attached spreadsheet answers this question. 11. DO YOU HAVE A VECTOR FORMAT (.eps or.ai) file of the artwork for this item available (SPECIFICALLY THE PUT IT DOWN LOGO). FDOT Traffic Safety Office Response: No, we re leaving this up to the vendor. There is only two colors, black font with a white background. 12. What is the TOTAL QUANTITY IS IT JUST 1 BUNDLE OF 2500 FOR EACH DISTRICT OFFICE? FDOT Traffic Safety Office Response: Please use the attached document to know who much money has been allocated per district office, the district locations for drop shipment, and the quantity of gadget grips per district. 13. In the bid packet, the wording describes Original Gadget Grips. However, the picture is of the Pearl Gadget Grips. Original Gadget Grips and Pearl Gadget Grips have different adhesives and they have different pricing. Can you look at the website and let me know which one needs to be quoted? Website for Original: Website for Pearl: FDOT Traffic Safety Office Response: Website for Pearl: These grips usually come packaged either 2 per pack or 4 per pack. Is it the desire of the FDOT to receive these as 1 per pack for a total of 2,500 packs with 1 Gadget Grip per pack? Below is the website for the manufacturer. It shows how this item is usually package in either 2 or 4. However, we can order them package as 1 per pack if that is the preference. 4 15. Is standard packaging acceptable versus 2500 in one box? FDOT Traffic Safety Office Response: Yes & No, please use the attached document to know who much money has been allocated per district office, the district locations for drop shipment, and the quantity of gadget grips per district. District two is the only district that had an odd amount that covered 2 packages and $4,000 towards a third (this $4,000 could be completed with standard packaging) If this causes any problems then the District two amount will be lowered to $10, The Technical Specifications does not mention that each Divisions bundle will be personalized? FDOT Traffic Safety Office Response: Yes each grip needs to be personalized, in the spreadsheet that you were provided today, the cost breakdown included a.5o cent charge for each grip to be personalized and a $1.49 per grip. The cost of $1.49 was based on the purchase of 2500 grips. 17. Will they be personalized or will all the grips use one artwork? FDOT Traffic Safety Office Response: Yes, by personalized, this meant that each grip will have the ITB artwork. 18. I would appreciate your confirming if we are to base the quote on 16 bundles plus the amount extra we can get for location two per the footnote or are we to utilize the total dollar amount for each location and base the cost on this larger quantity that they could purchase. FDOT Traffic Safety Office Response: If you can get a larger quantity using the total amount, I would say yes, let s look into this opportunity. 19. This question comes up as you have indicated that the artwork is identical on all of the units which mean that they have overestimated their bundle cost significantly and could therefore get many more units than indicated if we are using the amount of money as the factor in determining the quantity to be ordered. FDOT Traffic Safety Office Response: Would be very interested to see what the total amount could purchase. 20. One last question. In researching the product there are several products that fall into the "Gadget Grip" category. So that I may provide the State the best product and price, per specifications, do you know the name of the manufacturer? 5 21. The Variable Identification mentioned in the attachment I received is meant to be for personalization or consecutive numbering. I assume you do not want these personalized, do you want them consecutively numbered? FDOT Traffic Safety Office Response: The gadget grip website states the following: Variable Identification Process (V.I.P.) combines your corporate logo with unique names, coupon codes, phone numbers or other personalized data. In this case the artwork attached to the ITB has our corporate name and logo for this project. Your right we don t want this promo item number, we do want the corporate name and project logo on each unit. Based on the information from the gadget grip website, we will have to pay for the VIP. 5

If this will cause a problem then only plan for two packages of 2500 at a cost of $9,950.
