3 DHSMV-ITB CUSTOM TRAUMA KITS QUESTIONS AND ANSWERS All written questions are reproduced in the same format as submitted by the Bidder. Question(s) received from BloodStone Division, LLC: 1. Question: Our questions reference the list of contents below: 3.2 Technical Specifications: g - One (1) Sterile Latex-Free Nasopharyngeal Airway with Surgical Lubricant flexible, soft material, 4 in. H x 8 in. W x.75 in. D; Question: Could you please specify the NPA size desired? I.e. Female / Small Male 6mm or 7mm for average male, (Size 28 FR fits 90% of adults). Exact size rather than exterior dimensions will enable us to provide the NPA that meets your requirement. The Department requires size 28 FR. Question(s) received from Safeware, Inc.: 2. Question: In section 3.2, technical specifications, a Condor Brand Combat Assault Pack, Condor part #126 is referenced. Does the backpack bid/supplied need to be the Condor brand part # 126 referenced, or would an equivalent backpack from a different manufacturer meeting the same specifications listed, be acceptable? Only the backpack model specified in Section 3.2, TECHNICAL SPECIFICATIONS, is acceptable. 3. Question: The Condor combat assault pack, part # 126, is a large multi compartment backpack, and far larger than needed for the medical supplies/contents listed in the kit specification. Are there other items that the department will be putting in these backpacks after they are delivered with the listed medical supplies/contents? If not, a smaller more manageable pack might be beneficial in both use and in cost. Would an alternate, more appropriately sized bag be considered? Part 1: Yes, the Department intends to fill the backpacks with additional items. Part 2: No. Please see the answer to question #2. 4. Question: The Condor combat assault pack, part # 126, is a large multi compartment backpack. Is there a specific way or order that the department would like the listed medical supplies/contents assembled in the various compartments of the backpack? The Department has no preference for the order in which medical supplies are assembled within the pack. DHSMV-ITB , Questions and Answers, Page 1 of 5
4 Question(s) received from Bound Tree Medical: 5. Question: Are we able to use alternative products that are similar or identical to the specifications provided that may help provide additional cost savings? Many agencies are switching to a Chitosan based dressing known as ChitoGauze XR Pro. It is packaged in the z-fold, vacuum packed package, with the same dimensions as Combat Gauze. This hemostatic gauze is effective with individuals that have clotting disorders or take medications (such as aspirin or blood thinning agents) where Kaolin based products require the bodies clotting cascade to work. The ChitoGauze XR Pro does not require the bodies clotting cascade to be intact, which is vital as up to 25% of the American population is taking blood thinning agents. The Department is only seeking bids for the items specified in the ITB. 6. Question: It is stated that The Vendor must deliver all shipments in their entirety to all FHP locations within ninety (90) days of purchase order issuance. With the estimated quantities being provided for each Troop, can we expect that each Troop will purchase their estimated total of kits spread out through the term of the contract, or will the purchase for each Troop be all at once for the estimated quantities provided? The Department intends to but cannot guarantee that it will place an order for each Troop at the same time during the initial contract term. For additional information, please refer to Section 3.2, TECHNICAL SPECIFICATIONS, subsection C., Estimated Quantities. 7. Question: It is stated in the General Contract Conditions on page 3 that During the Contract term, if the Customer becomes aware of better pricing offered by the Contractor for substantially the same or a smaller quantity of a product outside the Contract, but upon the same or similar terms of the Contract, then at the discretion of the Customer the price under the Contract shall be immediately reduced to the lower price. Because this would be a custom kit in which we will have built for the first time with no other kit like it currently being made or sold, can you confirm that in order for this clause to apply, it would need to be a kit made with all of the same exact components? When the Contractor offers better pricing outside the Contract for substantially the same or a smaller quantity of the custom trauma kits or replacement medical supplies listed in FROM 3 PRICE SHEET, at the discretion of the Department, the Contractor must reduce the Contract price of the trauma kits or replacement medical supplies. For purposes of this ITB, Section 4.(b) of the General Contract Provisions, PUR 1000, only applies to those products (not their components for which there are no prices bid) listed in FORM 3 PRICE SHEET. Products listed in FORM 3 PRICE SHEET include: Complete Custom Trauma Kits (as described in the ITB); Condor Compact Assault Pack (Item #126); Combat Application Tourniquet single routing buckle, windlass rod, windlass clip, windless strap, stabilization bar; and 4-inch Israeli Emergency Bandages. Question: For the pricing we are to provide for years 2-3 and years 4-6, are we able to provide our same price that we would offer for Year 1 with a contingency that, where applicable, we are able to take price increases given there are manufacturer increases for the items within the kit? If there are no increases on any items within the kit, the price per kit would then stay the same. All pricing submitted by the Bidder shall be firm, fixed pricing. The Bidder is not permitted to provide caveats or exceptions to the pricing submitted with their bid. Question(s) received from T-MED Services, Inc.: 9. Question: Our company is very interested in preparing a bid for this RFQ. A question has been raised regarding one of the items on the bid. One (1) CPR Pocket Resuscitator with: latex-free replaceable one-way valve Filter, splash-resistant, elastic strap, storage case.; Is the item requested along the lines of a traditional pocket CPR mask such as this one? Or, does the department want a more modern, suitable product such as a "pocket resuscitator Bag Valve Mask device" such as this? The Department requires a traditional pocket CPR mask. Question(s) received from BioLogistex, Inc.: 10. Question: I would like to submit for approval to have the CELOX hemostatic z-fold and CELOX z-fold Rapid gauze accepted as an alternate for the current listed line item under section 3.2-2f of Combat Hemostatic Gauze Dressing. CELOX is FDA and CoTCCC approved. CELOX is unique from Combat Gauze in that it works outside the body's clotting cascade, whereas Combat Gauze actually needs to engage the body s clotting cascade in order to stop bleeding. Thus, CELOX works effectively for any individual on a blood thinner such as Coumadin, Heparin and Warfarin (which is a very large percentage of the US population), any individual who may have a bleeding disorder such as hemophilia, or who may have hypothermic bleeding conditions due to blood loss. In dealing with such a diverse population we are bound to encounter an individual that fits this criteria and having an enhanced item that will effectively stop bleeding DHSMV-ITB , Questions and Answers, Page 3 of 5
6 for individuals with these additional needs is necessary. Lengths of the items vary however, please be aware that quantity is not proportionate to efficacy. Supporting literature attached. Furthermore, other differences that demonstrates the superiority of CELOX to Combat Gauze encompass biocompatibility, biodegradability and efficacy. I have attached a supporting power point. Please also reference summary document titled Supporting Literature. Documents are attached should further detailed information be sought from source directly. Please see the answer to question #5. Question(s) received from Blue Spear Solutions, LLC.: 11. Question: Our team is preparing a proposal for the ITB described above. I was wondering if FHP is seeking any value-added services related to training and certification for this bid? We currently have the only nationally supported online learning management system for Bleeding Control Techniques-M.A.R.C.H. Our online course is on a secured server and is licensed by the US Department of Defense as part of the national Stop The Bleed Campaign. Our program is managed by our Chief Medical Director and licensed/practicing Emergency Medical Physician. At the completion of the course all FHP Troopers will receive an individual credentialed certification number authorized by our Medical Director. The course consist of high resolutions instructor-led video instructions on how to properly apply: Stopgap Measures / Pressure Point / Direct Pressure Techniques Self Aid Tourniquet Application w/ Generation 7 Combat Application Tourniquet Buddy-Aid Tourniquet Application w/ Generation 7 Combat Application Tourniquet Wound Packing Techniques w/ Quikclot Combat Hemostatic Gauze Kaolin Treating a penetrating wound to the chest w/ Hyfin Chest Seals 3 Vent Airway Management Skills Minimizing the logistical consideration for training and certifying all FHP troopers uniformly and expeditious under national field care standards will benefit the State of Florida. Moreover, our program will significantly reduce the time and cost associated with traveling to and from multiple Troop locations and/or FHP Training Academy. Should this be of interest to FHP, please let me know and we shall included it into our proposal.
