2 SUMMARY K.L FASHION LTD. is housed in a G+2 storied owned industrial building with 2 shades. One of the shade is for boiler another is for Generator and substation. The factory was constructed gradually started from 2004 and ended in But the production started in The total building height is approximately 9.23m and total floor area is about 25,000sqft. There are 600 workers working for the factory. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The Specific timing of improvements, including any requested extensions due to design / installation constraints shall be submitted to the Accord for approval. Page 2 of 12
3 FINDINGS AND RECOMMENDATIONS FINDING NO: E- 1 CATEGORY: DESIGN, DRAWINGS & RECORDS 1. As-built electrical SLD, wiring layout designs and drawings, machine layouts are not prepared. 2. Thermo graphic scanning of the entire electrical system has not been performed. 3. Insulation resistance test of electrical equipment has not been performed. 4. Earth resistance test is has not been performed. 5. Electric safety training program is not initiated. 1. The factory must have As-built electrical SLD with electrical wiring layout designs and drawings. Any changes in load, protection system, conductors, Generation and supply system must be reflected in the As-built SLD and drawings. 2. Thermo graphic scanning of the entire electrical system must be performed on tri-annual basis and recorded. 3. Insulation resistant test of all the cables must be performed once every 5 year cycle and recorded. 4. Earth resistance test should be done in five years cycle and recorded. 5. Electrical safety training and awareness program for the electrical personal and workers must be initiated and recorded. REMEDIATION TIME FRAME: 25 WEEKS FINDING NO: E-2 CATEGORY: GENERATOR ROOM Bottom of the generator output panel is not sealed. Make circular hole at the base plate and provide cable gland according to the respective cable size for cable entry and exit so that the cables are not stressed on the sharp edges of the hole of panels. 6 FINDING NO: E-7 CATEGORY: GENERATOR ROOM Generator room wall has several holes; rain water may enter to the room. Seal the holes of the room and install exhaust fan for the ventilation. REMEDIATION TIME FRAME: 12 WEEKS Rain water may enter to the room FINDING NO: E-8 CATEGORY: GENERATOR ROOM Combustible materials stored and excessive dust/ lint deposit in generator room Electrical room must be free from materials (that are not required for regular operations) stored inside the room. Construct cable trench to protect the cables to ensure the mechanical protection of the cable laid on floor otherwise cable insulation may damage due to falling object or stepping of occupants onto it. PRIORITY: P3 Rubber mat missing FINDING NO: E-11 Loop connection of MCBs inside panel. (Typical) Wiring looped at MCB terminals may be replaced by using Bus bars to distribute different circuits. (Typical) Terminate single cable (lugs/sockets) on a single point of bus-bar to get better electrical continuity and avoiding loose connection. (Typical) Remove all the multiple connections made at a single point of MCCB in panel and connect individual branch cables to individual MCCB in panel providing individual lug according to the respective cable size. 9 FINDING NO: E-15 Burnt sign visible at terminal(s) inside panel.(typical) Arrange periodic inspection & thermal scan to identify the overloading, loose connection, unbalanced load which may cause the excessive heat-rise and take action accordingly. REMEDIATION TIME FRAME: 12 WEEKS Burning signs inside panel FINDING NO: E-16 CATEGORY: WIRINGS Cables/wires passing through wall not protected and remaining gaps around the cable/wiring not sealed. Cables passing through permanent walls must be protected in steel/pvc pipes and remaining holes around the pipe must be sealed. Seal all the penetrations using appropriate fire rated material and ensure the cable insulation does not get damaged during sealing work. Power cables passing through the wall. Page 9 of 12

Remove lint and dust from the cable duct and cover it with noncombustible material; make a periodical cleaning program.
