2 SUMMARY The (G+6) storied building constructed in 2003 house two factories namely; Taratext (BD) Ltd. and S.P.M Design Ltd. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The Specific timing of improvements, including any requested extensions due to design / installation constraints shall be submitted to the Accord for approval. TARATEX (BD) LTD. Page 2 of 17

3 FINDINGS AND RECOMMENDATIONS Finding #: E- 1 TRANSFORMER ROOM No protection between the transformer and surrounding area. The transformer must be installed with barrier walls between transformer and other panels. The walls must be fire resistant and should have height up to the ceiling. No barrier wall between transformer and surrounding electrical installations. Finding #: E- 2 TRANSFORMER ROOM Arcing horns on HT bushings of transformer, not installed. Arcing horns may be installed as per the transformer manufacturer's requirement. Arcing horns on HT bushing of transformer are not installed. TARATEX (BD) LTD. Page 3 of 17

4 Finding #: E- 3 Flexible PVC conduit is used for service cable protection. The cables must be supported on Ladder cable tray with cover. Flexible PVC conduit is not adequate for protection. Cables supported on external walls must be laid horizontal/vertical to the wall, supported in cable trays/ladder. 6 Months. Conduit running on wall is not supported adequately for protections. Finding #: E- 5 HT cable while entering, it touches the sharp steel edges of the panel. Cables must be protected from sharp edges of the panels. Provide rubber grommet to the sharp edges of the panel. Remaining openings should be filled with fire sealant material. HT cable entering and leaving panel not protected. TARATEX (BD) LTD. 5 Finding #: E- 6 Excess cable length not arranged and supported. Excess length of existing HT cables coiled near transformer must be protected and laid safely. Excess HT cables coiled (not arranged) and laid on floor inside transformer room. Finding #: E- 7 Cable laid directly on concrete floor. HT cable must be supported in cable trays or laid in trenches. The cable must be protected against physical injury. 6 Months HT power cable laid directly on floor inside transformer room. Finding #: E- 8 Cable entering electrical room, through wall & fence, is not protected. Cables passing through permanent walls must be protected in steel pipes and remaining holes around the pipe must be sealed. 6 Finding #: E- 9 SWITCH BOARD & PANELS Panel base plates removed to allow cable entry. Panel base plates must be installed, at all time, and cable(s) entering panel must be firmly fixed with cable gland. Panel base plate not installed. Finding #: E- 10 SWITCH BOARD & PANELS Spacing around the MCCBs narrow and crowded inside panel. Rearrange the devices inside panel such that it can be easily accessible during operation and maintenance. Wires crowded inside panel may be securely fastened installing ducts or using cable ties. MCCB installed close to each other inside panel. Within 1 Month Finding #: E- 11 SWITCH BOARD & PANELS Cables looped at the MCCB Individual connection shall be taken from bus bar for individual circuit breaker but where not possible, it is recommended to use pin type bus bar instead of looping. 7 Finding #: E- 12 Category : GENERATOR ROOM Cables terminating to generator output terminal box are laid on floor. Cable must be arranged and latched properly on the cable tray. Provide cover made of non-combustible material preferably metallic sheet to protect the cables insulation from physical damage as well as prevent the ingress of debris, dust and lint. Cable from generator output terminal to LT panel drawn in flexible conduit, laid on floor. Finding #: E- 13 CABLE & CABLE SUPPORTS Disconnected cables from equipment are left naked. The used cables on the ceiling must be tapped properly with electrical tape to avoid short circuit and electric shock. The cables must be protected and supported above floor. Immediate Wire disconnected from load left open without proper insulation. Finding #: E- 14 CABLE & CABLE SUPPORTS Ducts not covered and cables in it are randomly placed. Cable ducts must be cleaned regularly and covered to prevent ingression of dust and lint. 6 Months Cable/wires in wiring duct congested/overloaded. Wires not arranged properly. 8 Finding #: E- 15 CABLE & CABLE SUPPORTS Flexible PVC conduit wiring not supported. Flexible conduits (standard and approved for industrial use) must be securely clamped or fastened at regular intervals. Flexible conduit use for wiring and power socket extension not supported on wall. Power socket not installed permanently on wall. Finding #: E- 16 CABLE & CABLE SUPPORTS Flexible PVC conduit wiring not supported. Cables must be protected and supported on tray, duct or conduits to protect against weather and possible physical damages. Wiring extended from wiring duct to ceiling light in flexible conduit not supported adequately. Flexible conduit sagging. Finding #: E- 17 CABLE & CABLE SUPPORTS Wiring fixed to ceiling by clamps or saddle at longer intervals. Existing wiring in flexible PVC conduits fixed to ceiling must be additionally supported in tray or ducts. Flexible conduit must not be used for long point wiring (except for special wirings). 9 Finding #: E- 18 CABLE & CABLE SUPPORTS Cable/wiring duct cover is missing. Wiring duct has deposit of lint and dust. The cover of the tray/duct should be installed to keep it free from the ingress of debris, dust and lint. Establish a periodic cleaning program to keep all the duct/trays/channel dust-free. Top cover of wiring duct above work bench left open. Wiring duct has deposit of lint and dust. Finding #: E- 19 WIRINGS Cables passing through window grills. Cables must be protected and supported and installed through a safe route. Existing cables passing through window and ventilators must be removed immediately. Cables drawn in PVC conduits passing through ventilator. Finding #: E- 20 WIRINGS Wiring/cables passing through doors or gate. Existing cables or wiring passing installed through door or gate must be protected against possible damages from the shutters or door panel. 10 Finding #: E- 21 WIRINGS Wires joined in wiring supports, between terminations. Existing joints in wiring between terminals must be checked and tightly connected using sockets and then insulated using heat shrink insulating tubes. Junction boxes with cover may be used to protect joints. Branching/Tapping of circuit from existing wiring in wiring duct. Finding #: E- 22 WIRINGS Wiring in flexible PVC conduit not supported. Wiring in flexible PVC conduit must be additionally supported and securely fixed at regular intervals. Within 1 Month Flexible conduit on ceiling not supported adequately (Typical) Finding #: E- 23 WIRINGS Wiring duct overloaded with cables and wires. Additional ducts may be installed in tiers or replace the existing duct. The existing ducts may be widened or wiring system may be redesigned to reroute by installing additional distribution panels. 11 Finding #: E- 24 WIRINGS Wires exposed and not supported between ducts. Wires between ducts must be protected and supported in conduits or in ducts.
