2 SUMMARY The is currently operated in a seven (B+G+6) storied building. The building was constructed in 2009 and production was started from The factory had approximately 1,019 workers. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which would be further dealt with as part of follow-up inspections. The table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to each finding. The implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design/installation constraints, shall be submitted to the Accord for an approval. Page 2 of 17
3 FINDINGS AND RECOMMENDATIONS FINDING NO: E-1 CATEGORY: DESIGN, DRAWINGS & RECORDS Electrical Single Line Diagram (SLD) has inadequate information, and thus is unusable. Prepare an accurate SLD with all necessary information, so any experienced person will then be able to understand the system. REMEDIATION TIMEFRAME: 12 WEEKS FINDING NO: E-2 CATEGORY: DESIGN, DRAWINGS & RECORDS Thermographic scanning of the entire electrical system has not been tested and recorded. Thermographic scanning for the entire electrical system must be performed on a bi-annual basis and recorded. REMEDIATION TIMEFRAME: 8 WEEKS FINDING NO: E-3 CATEGORY: DESIGN, DRAWINGS & RECORDS Insulation resistance test of electrical equipment is not performed. Insulation resistant test of all the cables must be performed once every 2 year cycle and recorded (this must require a complete power shut off). REMEDIATION TIMEFRAME: 8 WEEKS Page 3 of 17
4 FINDING NO: E-4 CATEGORY: DESIGN, DRAWINGS & RECORDS Electrical safety training program is not initiated/conducted. Electrical safety training and awareness program for the electrical personnel and staff must be initiated and recorded. REMEDIATION TIMEFRAME: 8 WEEKS FINDING NO: E-5 CATEGORY: DESIGN, DRAWINGS & RECORDS Instruction for CPR (Cardiopulmonary Resuscitation) or Electrical shock restoration is not present. Hang this first aid and CPR instructions near all electrical equipment (LT panel, MDB, FDB, DB, SDB) on a visible location. FINDING NO: E-6 CATEGORY: DESIGN, DRAWINGS & RECORDS Maintenance records are unavailable. Maintenance Manager or Safety Officer must keep accurate records and ensure that they reflect actual factory day to day operations. Page 4 of 17
5 FINDING NO: E-7 CATEGORY: LIGHTNING PROTECTION Lightning Protection System (LPS) is needed but has not been installed yet. Design and install Lightning Protection System (LPS) in the factory; the LPS designs must be submitted to Accord before starting installation. REMEDIATION TIMEFRAME: 16 WEEKS FINDING NO: E-8 CATEGORY: EARTHING SYSTEM Earth Pit resistance record is unavailable. Record earth pit resistances for all the earth pits, and do it once a year. REMEDIATION TIMEFRAME: 8 WEEKS FINDING NO: E-9 CATEGORY: EARTHING SYSTEM Earth pits are not identifiable. Clearly identify each earth pit, and mark it for periodic maintenance purposes. 6 FINDING NO: E- 10 CATEGORY: GENERATOR ROOM Inadequate working clearance around the generator/s. Working clearance around each generator should be 1.07m. If multiple generators are installed then the gap between two generators must be equal to the width of bigger generator. REMEDIATION TIMEFRAME: 12 WEEKS Diesel generator sets. FINDING NO: E- 11 CATEGORY: GENERATOR ROOM Generator body has been earthed by inadequate/improper sized (type) cable. Ensure minimum two earthing connection for generator body (consult with your generator supplier). Generator body earthing. FINDING NO: E-12 CATEGORY: GENERATOR ROOM Earthing cable remains uninsulated. Insulate the earthing cable properly. PRIORITY: P3 REMEDIATION TIMEFRAME: 1 WEEK Generator body earthing cable. PRIORITY: P3 REMEDIATION TIMEFRAME: 1 WEEK Generator batteries without terminal cap. FINDING NO: E-14 CATEGORY: GENERATOR ROOM Earth conductors are improperly jointed to extend earth connection. 8 FINDING NO: E-15 CATEGORY: TRANSFORMER ROOM Inadequate working clearance around the transformer. Working clearance around transformer should be 1.07m. REMEDIATION TIMEFRAME: 16 WEEKS Transformer in a congested place. FINDING NO: E-16 CATEGORY: TRANSFORMER ROOM Excess cables coiled and kept at the back of panel. Rearrange cables using cable tray/ladder; trim the unnecessary cables and use only the required length. (Note: if it is a HT cable, do not trim it instead rearrange it to remove coils). Cables behind the LT panel. FINDING NO: E-17 CATEGORY: TRANSFORMER ROOM Transformer body earthing is not connected properly. Connect the earthing cable properly. 9 FINDING NO: E-18 CATEGORY: TRANSFORMER ROOM Transformer Silica gel is discolored, breather cup empty. Replace silica gel by a new one; or dry it under sun and reuse it, if color regains. Fill the transformer breather oil cup with transformer oil. Silica gel. FINDING NO: E-19 CATEGORY: TRANSFORMER ROOM One arcing horn is missing. Install the missing arcing horn. Arcing horn on HT bushing. FINDING NO: E-20 CATEGORY: TRANSFORMER ROOM Inflammable materials are stored beside the transformer. Remove the inflammable materials from transformer room. REMEDIATION TIMEFRAME: IMMEDIATELY Oil container beside the transformer. Page 9 of 17

FINDING NO: E-22 CATEGORY: CABLE & CABLE SUPPORTS Factory's service cables have excessive sagging/inadequate support. Excessive sagging should be fixed by adding adequate support to avoid any arcing/flashing at the Grid terminal/factory terminal. HT cable. Page 10 of 17
11 FINDING NO: E-23 CATEGORY: CABLE & CABLE SUPPORTS Power cables entering to or exiting panel are not properly fixed. Power cables entering or exiting the panel must be fixed through Panel base/top plate using cable glands (metal/pvc). Cable trays/ladders may be used to support the cables. REMEDIATION TIMEFRAME: 6 WEEKS Changeover switches. FINDING NO: E-24 CATEGORY: CABLE & CABLE SUPPORTS Power cable is covered by sand in the cable trench (or cable trench is filled with sand). If the cable trench is designed to be covered with sand, the trench may be left as is; otherwise remove sand from cable trench to avoid physical damage to the HT cable. REMEDIATION TIMEFRAME: 6 WEEKS Cable trench. FINDING NO: E-25 CATEGORY: CABLE & CABLE SUPPORTS Cables passing through wall/floor slab are not protected at the entry/exit point(s). Cables passing through permanent wall/floor slab must be protected. Seal the opening by fire rated material protecting power cables preventing any smoke form passing thorough the gap. 12 FINDING NO: E-26 CATEGORY: CABLE & CABLE SUPPORTS Sanitary PVC pipes are used for cable support. Use aluminum/ms channel instead of PVC pipe. REMEDIATION TIMEFRAME: 8 WEEKS PVC pipe for cable support. FINDING NO: E-27 Excessive lint/dirt under the distribution panel. Clean all lint and dirt under electrical distribution box/panels. REMEDIATION TIMEFRAME: 3 WEEKS Dust and lint under the panel. Page 12 of 17
13 FINDING NO: E-28 Changeover switch contacts smeared with bearing grease. Clean excess grease from the contacts. Use thin layer of contact grease for lubricating. PRIORITY: P3 Changeover switch contacts. FINDING NO: E-29 Panel doors are not connected with earth. All metal panel doors must have an earth connection of at least 4 rm earth cable. Door earthing. FINDING NO: E-30 Phase barrier/separator is missing. Put a purposely made phase separator (rubber type) between two phases; also terminate cables by proper sized cable lugs and cover cable lugs by heat shrink. LT panel. Page 13 of 17
14 FINDING NO: E-31 Distribution Board's top/bottom is left open (typical issue). Seal each distribution board's top/bottom; and use cable glands holding/supporting cables. Panel base plate is missing. FINDING NO: E-32 Cables insulation at termination point is deteriorated. Check the cables and re-insulate them. Termination point of MCCBs. FINDING NO: E-33 MCCB is not fixed on the panel. MCCB must be fixed on the panel. PRIORITY: P3 MCCB laid on the panel. Page 14 of 17
15 FINDING NO: E-34 Cables terminated at electrical busbar without cable lugs. Terminate all the power/earth/neutral cable with proper sized cable lug. Termination without lug. FINDING NO: E-35 Multiple cables terminated at MCCB terminals. Terminate each power cable at single terminal and use proper sized cable lug. Multiple cables termination can be used if all are all soldered properly and powering a single circuit. Multiple connection. FINDING NO: E-36 Power cables are bent excessively. Avoid power cable bending in electrical system; in unavoidable case bend cables without any stress but not less than 135 degrees. Excessive bend on cable. Page 15 of 17

Remove the wires or terminate properly. Unterminated wires. FINDING NO: E-39 CATEGORY: TRANSFORMER ROOM Oil level in transformer oil tank is below minimum level. 17 FINDING NO: E- 40 Inflammable materials stacked adjacent to electrical panel. Inflammable materials must be stacked maintaining a safe clearance of at least 1.5 m from any electrical installation. REMEDIATION TIMEFRAME: IMMEDIATELY Inflammable materials. FINDING NO: E- 41 Hot spot was found at the termination point of MCCB. Check the connection and load of the phases.
