Read and understand this wner s manual befre installing and using the frk extensins. Ensure that all safety and warning labels stay in place and are legible. The frk truck s frks must be inserted all the way int the frk extensins s that the frk extensins lcks are lcated behind the heel f the frks. D nt attempt t lift a lad using nly the end f the frk extensin. Any lad lifted using frk extensins must be fully supprted by the extensin. Always carefully watch the lad n the frk extensins when a lad is elevated. D nt use a frk extensin if it appears t be damaged r bent. D nt perfrm any mdificatins t the frk extensin withut the manufacturer s apprval. Failure t receive authrizatin fr changes t the equipment culd vid the warranty. Repairs are t be dne nly by persnnel qualified t perfrm the required wrk. REPLACEMENT PARTS: There are n replaceable parts fr this prduct. In any cmmunicatin with the factry, please be prepared t prvide the frk extensin s serial number, which is indicated n a safety label. RECEIVING INSTRUCTIONS Every unit is thrughly inspected prir t shipment. Hwever, it is pssible that the unit culd incur damage during transit. Inspect the unit clsely when it arrives. If yu see evidence f damage r rugh handling t either the packaging r t the prduct when it is being unladed, immediately make a nte f it n the Bill Of Lading! It is imprtant that yu remve the prduct s packaging upn its arrival t ensure that there is n cncealed damage r t enable a timely claim with the carrier fr freight damage. 2 ROUTINE MAINTENANCE & SAFETY CHECKS ALL FE MODELS (A) Befre each use: 1.) Check fr damage t the frk extensin and its lcking mechanism. 2.) Verify that the frk extensin lcking mechanism has prperly engaged. (B) Inspect mnthly fr: 1.) Damage, bending, r fatigue. Check especially fr bending f the part f the lck that secures the frk extensin behind the heel f a frk truck s frks. 2.) Readability f the safety labels. OPERATION INSTRUCTIONS ALL FE MODELS Ensure that all emplyees invlved in the peratin f frk extensins understand and fllw these instructins! The standard mdel FE frk extensin is intended t be used with 2 thick frk truck frks, and the frk extensin mdel used must match the frk truck s frk width. It is intended t be used t prvide additinal supprt fr frk truck lads whse length are up t 150% f the frk truck s frk length. Per ASME B , Safety Standard fr Lw Lift and High Lift Trucks, Part II, Paragraph 5.4.3:... Frk length shuld be at least tw-thirds f lad length. Nte: The rated center f gravity f the frk truck s frks des nt change with the use f frk extensins. OSHA 29CFR, Chapter XVII, Part (a)(4) states, Mdificatins and additins which affect capacity and safe peratin (f a frk truck) shall nt be perfrmed by the custmer r user withut manufacturer s prir written apprval. Capacity, peratin, and maintenance instructin plates, tags, r decals shall be changed accrdingly. And (a)(5) states that If (a frk) truck is equipped with frnt-end attachments ther than factry installed attachments, the user shall request that the truck be marked t identify the attachments and shw the apprximate weight f the truck and attachment cmbinatin at maximum elevatin with lad laterally centered. Installatin: FE-x-xx: With the frk truck s frks abut 1 ff the flr, lift the nse end f the frk extensin up at apprximately a 45 angle. Place the ppsite end f the frk extensin nt the tip f the frk, with the lcking lp under the frk. Slide the frk extensin nt the frk until it reaches the heel f the frk and lwer the extensin nt the frk. FE-x-xx-P: Remve the lck pin and place the frk extensin ver the frk truck s frk s it is all the way back against the frk s heel. Reinstall the lcking pin behind the frk truck s heel. FE-x-xx-AL: Align the frk truck s frks with the pen end f the frk extensins. With the frks level r slightly tilted frward, drive the frk truck frward until the frk extensins reach the heel f the frks. Raise the frks apprximately three feet and pull the frk extensins utward t verify that the lcks are prperly engaged. Nte: T remve the aut-lck frk extensins frm beneath a lad, the extensins must nt be in cntact with the grund. Fr pallets r skids, lwer the frks until the pallet r skid just rests n the grund. Tilt the frks slightly frward, then lwer them anther ½ t ¾. Back ut slwly. Cautin: Never use a frk extensin if it appears t be damaged r bent. Lading: Any lad lifted using frk extensins must be fully supprted by the extensin. Cautin: D nt exceed the frk truck s lad rating at its listed center f gravity. Cautin: D nt attempt t lift a lad using nly the end f the frk extensin. Permanent damage t the frk extensin, damage t prperty, r injury t persnnel culd result. Cautin: Always carefully watch the lad n the frk extensins when a lad is elevated. Ensure that all safety and warning labels stay in place and are legible. Refer t the labels page in this manual. 3 SAFETY LABEL IDENTIFICATION -- ALL FE MODELS * Prduct safety signs r labels shuld be peridically inspected and cleaned by the prduct users as necessary t maintain gd legibility fr safe viewing distance -- ANSI (10.21). 5 PRODUCT WARRANTY 30-DAY LIMITED WARRANTY The manufacturer warrants fr the riginal purchaser against defects in materials and wrkmanship under nrmal use fr 30 days after the date f shipment. Any part that is determined by the manufacturer t be defective in material r wrkmanship and returned t the factry, shipping csts prepaid, will be, as the exclusive remedy, repaired r replaced at ur ptin. Labr csts fr warranty repairs and/r mdificatins are nt cvered unless dne at manufacturer s facilities r pre-apprved in advance by the manufacturer. Any mdificatins perfrmed withut written apprval f the manufacturer may vid warranty. This limited warranty gives purchaser specific legal rights which vary frm state t state. All specificatins are subject t change withut ntice. LIMITATION OF LIABILITY T the extent allwable under applicable law, the manufacturer s liability fr cnsequential and incidental damages is expressly disclaimed. The manufacturer s liability in any event is limited t, and shall nt exceed, the purchase price paid. Misuse r mdificatin may vid warranty.
