3 Shipping Dcuments Bill f Lading Each less-than-trucklad (LTL) r trucklad (TL) shipment must be accmpanied by a Bill f Lading (als referred t as a BOL r B/L). Multiple rders shipped t a single facility n the same day must be cnslidated n ne Bill f Lading, regardless f different purchase rder numbers. Bills f Lading must be cmpleted in their entirety; including the ttal number f pieces and pallets, cmmdity descriptins, Natinal Mtr Freight Classificatin (NMFC) cdes (LTL nly), and freight classes (LTL nly). In additin t this standard required infrmatin, each BOL must cntain the fllwing detail: Shipment Date: The date the carrier picks up the shipment. Nte: Pre-dating Bills f Lading is a fraudulent practice. The dcument must always reflect the date that the rder(s) actually shipped. Supplier Name ( Ship Frm ): Nte: When utilizing a distributin r ther third-party cmpany t fulfill purchase rders, the shipper name must read as the supplier name prvided in the purchase rder c/ the distributin cmpany name. e.g.: Write Brthers Pencils c/ American Distributin Shipper Address ( Ship Frm ): The address where the prduct is actually shipping frm (nt necessarily the same as a crprate address). Destinatin Facility ( Ship T ): United Statiners Supply Cmpany + Facility # Nte: In the case f an NL (nn-listed) rder, the Ship T party r Cnsignee will be the custmer that is receiving the drp shipment. Supplier Accunt Number ( Ship T line 1): A uniquely assigned fur (4) digit number (include all leading zers). FOB Terms ( Ship T line 1): Origin/Destinatin Page 3 f 22

Stck purchase rders are seven (7) characters in length (include leading zers). Nt Listed (NL) rders will begin with NL r N fllwed by a six (6) character purchase rder number. 6 Shipping Dcuments Packing List A legible cpy f the packing list must accmpany the Bill f Lading n all cmmn carrier (LTL), trucklad and cnslidated shipments. The riginal cpy f the packing list shuld be lcated n the utside f the first pallet clsest t the tail f the truck and in a highly visible psitin. This packing list must be remvable. 8 Mde Selectin Small Package Small Package ruting applies t shipments weighing frm punds, shipping either directly t a United Statiners Supply C. facility r drp shipping t a United Statiners Supply C. custmer, and which meet United Parcel Service s (UPS) shipping criteria. All packages shipping t a United Statiners Supply C. facility must be marked as Cllect and billed t the facility s assigned UPS accunt number. Please cntact the Inbund Transprtatin team r yur Prduct Manager fr the applicable inbund UPS accunt number(s). In additin t meeting all f the shipping criteria as utlined abve, NL (nn-listed) small parcel rders that ship directly t United Statiners Supply C. custmers must be shipped as Prepaid with the transprtatin cst added t the invice. The seven digit United Statiners Supply C. purchase rder number r NL (nn-listed) rder number fllwed by a dash (-) and the fur digit United Statiners Supply C. Supplier Accunt Number must be entered in the Reference Number 1 field f the UPS bill-f-lading. e.g.: Girth, represented by number 1 n the diagram, is the ttal distance arund the package r bject at its widest pint, perpendicular t the length. Length, represented by number 2 n the diagram, is the lngest side f the package r bject. Add the length and girth measurements tgether t get the ttal package size. UPS has established specific weight and size limits fr packages. The restrictins belw nly pertain t individual packages. Packages may be up t 150 lbs. (70 kg). Packages may be up t 130 (330 cm) in length and girth cmbined. Packages may be up t 108 (270 cm) in length. Packages that weigh mre than 70 lbs. require a bright yellw UPS heavy package label. It s extremely imprtant that accurate shipment characteristics are presented at the time the pickup is scheduled as this infrmatin will be used t determine the prper shipping mde (less-than-trucklad, cnslidatin r full trucklad). Shipment Weight The number f Pieces and standard (48 x40 ) and nn-standard Pallets. A packaging descriptin fr any prduct n a nn-standard pallet. 11 Ruting Guide Adherence Chargebacks and Assessments The ruting instructins cntained in this guide are mandatry fr all Cllect shipments, r suppliers with an established freight tariff r freight allwance prgram. Failure t adhere t these instructins, unless specifically authrized by a United Statiners Supply C. Transprtatin assciate, will result in a chargeback t the supplier fr the difference in the transprtatin/accessrial/fuel expense, plus a $100 administrative fee fr each shipment nt in cmpliance. Please nte that in the event f a billing discrepancy, that this may include shipments that are tendered directly t an underlying carrier rather than thrugh USSC s preferred inbund prvider C.H. Rbinsn. Freight expenses that are the result f imprperly defined billing terms r deliberate misuse will result in a chargeback t the supplier fr the transprtatin csts, plus a $100 administrative fee fr each shipment. 12 Shipment Receipt, Cunt, and Inspectin Please be advised that all inbund shipments are subject t cunt and inspectin. The fllwing steps will be taken in rder t facilitate this prcess in a thrugh and effective manner: STEP ONE: Initial Cunt & Inspectin United Statiners Supply C. will cunt and inspect all pallets and cartns fr visible damages and visible shrtages at the time f delivery. The carrier s delivery receipt will be marked with the fllwing stamp, with visible exceptins ntated in the upper Ntice prtin. STEP TWO: Final Cunt & Inspectin United Statiners Supply C. will perfrm a cmprehensive cunt and inspectin during the detailing and stcking prcesses. Cncealed damages will be ntated in the Final Shipment Exceptin Reprt prtin f the delivery receipt stamp. Additinally, United Statiners Supply C. will detail infrmatin regarding visible damages n the fllwing Ntice f Refused Damage Merchandise Return t Vendr Frm, and fax r it t the supplier within seventy-tw (72) hurs frm the time f delivery, excluding Saturdays, Sundays and hlidays. 13 Payment by Receipt Payment fr inbund merchandise is based n the ttal quantity f the item(s) received and accepted. Please be advised that the quantity reflected n the purchase rder r the supplier s invice is nt taken int cnsideratin when calculating the payment amunt. Visible Damages & Visible Shrtages FOB Destinatin Terms: Visibly damaged cartns will be refused at the time f delivery and returned t the delivering carrier. ASN-cmpliant (Advanced Shipping Ntice) suppliers will receive an autmaticallygenerated Shipment Discrepancy Ntice (SDN) in the event f any verages r shrtages; nn-asn-cmpliant suppliers will receive an autmatically-generated SDN fr partial line verages r shrtages. Cncealed Damages & Cncealed Shrtages FOB Destinatin and FOB Origin Terms: Cartns r merchandise cntaining cncealed damage will be held fr dispsitin frm the supplier fr a maximum f sixteen (16) business days, after which unclaimed prduct will be dispsed f. United Statiners Supply C. will nt issue payment t the supplier fr the cncealed refused damaged merchandise, any cncealed shrtages, r any unclaimed prduct that was dispsed f. USSC will nt issue payment t the supplier fr either the refused damaged merchandise r any visible shrtages. FOB Origin Terms: Visibly damaged cartns will be received at the time f delivery, but nt placed int stck. USSC will issue payment t the supplier fr the visibly damaged merchandise and/r visible shrtages. Page 13 f 22

14 Claims Filing Damage r Lss Visible Damages & Visible Shrtages FOB Destinatin Terms: Supplier will be respnsible fr pursuing any applicable claims with the delivering carrier. Cncealed Damages & Cncealed Shrtages FOB Destinatin and FOB Origin Terms: Supplier will be respnsible fr pursuing any applicable claims with the delivering carrier. 15 Shrt Payment Disputes Payment disputes must be submitted in writing t the United Statiners Supply C. receiving facility (identifiable by the first tw digits f the purchase rder number) within sixty (60) calendar days f payment date. Nte: Please allw seven (7) calendar days frm the date f receipt f the Shipment Discrepancy Ntice (SDN) t accunt fr split shipments r ther temprary recnciliatin issues. Only the United Statiners Supply C. receiving facility may authrize any settlements r credits, and will prvide a respnse, including a payment status update, within sixty (60) calendar days f the shrt payment dispute date.
