2 BID PROPOSAL FORM (Revised 2//200) TRUSTEES OF THE CALIFORNIA STATE UNIVERSITY ing System Project # SBCMP California State University, San Bernardino 5500 University Parkway San Bernardino, CA The Trustees of the California State University, on behalf of the State of California (hereinafter called the Trustees): The undersigned bidder hereby offers, in the amount stated below, to furnish all labor, materials, tools, equipment, apparatus, facilities, transportation, and permits for Project Number SBCMP Campus Wide Energy Management ing System at San Bernardino campus and Palm Desert Campus, and hereby agrees to enter into contract for Project Number SBCMP if this offer is accepted by the Trustees. TOTAL AMOUNT OF BASE BID $ Lump Sum (Use Figures Only) The above amount is to be stated in figures only and is the total amount bid for the entire contract work including all applicable taxes. Any alteration, erasure, or change must be clearly indicated and initialed by the bidder. The bidder agrees that if there are any discrepancies or questions in the figures, the Trustees will use the lower figure despite the bidder s intent. The Trustees reserve the right to reject any and all bids and to waive any irregularities. The Trustees estimated construction budget for project including the additive alternative is $300, Alternative ADDITIVE ALTERNATIVE The following additive alternative is an integral part of this proposal, and to be responsive, the bidder shall quote for the Base Bid, and also for the following listed additive alternative. If the Base Bid is lower than the Construction Budget, the Trustees reserve the right to award a contract on the basis of Base Bid alone, or, if the budget allows, on the basis of Base Bid plus the additive alternative as the Construction Budget allows. The Bidder shall hold the lump sum prices for all alternatives for 60 calendar days after the start date of the Notice to Proceed. The Trustees reserve their right, within 60 calendar days after the start date of the Notice to Proceed, to add into or deduct from the awarded contract amount by change order, any or all alternatives that were not previously awarded at the listed lump sum amounts, without any delay or impact to the project and with no markup or markdown. We are hereby requesting the 5% Small Business Preference. 3 BID PROPOSAL FORM (Revised 2//200) TRUSTEES OF THE CALIFORNIA STATE UNIVERSITY The bid is subject to the provisions contained in the Contract General Conditions (note especially Article 2.00 et seq. regarding instructions to bidders), and the bidder agrees that failure to comply with the conditions thereof shall be basis for rejection of this bid. The bid must be submitted on this Proposal Form, completely filled out and in a sealed envelope and delivered to Procurement and Support Services, Sierra Hall, Room SH25, at California State University, San Bernardino, before 2:00 PM on Thursday, March 4, 200, or it will be disregarded. Only bids from licensed contractors with current C0 General Electrical and/or C7 Low Voltage Communications and Wiring Systems Contractor s licenses will be accepted. Bidder shall enclose with this Proposal Form bidder s security in the amount equal to at least ten (0) percent of the amount of bid (see Article 2.04(c) of the Contract General Conditions). If the bidder is awarded the contract and then fails to execute the contract, this bidder s security shall be forfeited to the State. Bidders are required to submit with bid proposal, on the reference form provided, evidence of successful completion of three (3) jobs of similar size and nature which have been successfully completed by bidder within the last three (3) years. References must include contact names and telephone numbers, description of work completed, date and amount of contract. Failure to submit list of references with bid proposal may result in rejection of bid. The time period for completion of the project shall be forty five (45) Calendar Days from the construction start date as stated on the Notice to Proceed. Liquidated damages shall be five hundred dollars ($500.00) for each calendar day completion is delayed beyond the time prescribed for the project.

Furnish and install a new data outlet at each meter location. All new data outlets will be surface mounted on a surface adjacent to the location of the electrical meter. Conduit, wiring and data jacks will be provided by the contractor. Provide all rigid pathways from the data outlet to the meter and make final connection. Connect meter to data outlet using a patch cable routed inside the rigid pathway. Coordinate final locations of data outlets with John Muir, the campus RCDD. Refer to telecom specifications in bid package for this work. 4. Provide all necessary fire stopping when penetrating through fire walls. All penetrations shall be above the ceilings of any space. 5. All pathway penetrations through building exteriors shall be sealed using Dow Corning 790 silicone building sealant; color to match building. ing System Addendum #2 2//200

Bidders will place an order with Square D to provide the following engineering services related to the Square D PowerLogic metering system: PMCS Conceptual Design PMCS Network Configuration ION Enterprise Server Configuration & Programming Device Configuration Graphical User Screen Development Report & Alarm Configuration Onsite startup and commissioning 7. Provide, install and commission new server with latest ION Enterprise base software. Contractor to obtain Square D services to commission new server and ION Enterprise. s at CSUSB Palm Desert Campus to report to same server used for CSUSB San Bernardino campus. 8. Contractor to contract with Square D to provide 2 day onsite Operator training to commence after installation and commissioning of Square D PowerLogic metering system. 9.
