2 Renovation of FCT 21 Research Medical Library Addendum 4 RFI Questions and Answers Question 1 There are three (3) add alternate scopes of work called for in the project documents, however the Respondent s Pricing and Delivery Proposal and Execution of Offer that was on the MD Anderson website has NOT USED listed under section 1.2 for Alternate Scopes of Work. Will an alternate bid document be issued with space for the alternates to be proposed? Answer 1 Please refer to the newly posted Pricing and Delivery Proposal entitled, Pricing and Delivery Proposal and Execution of Offer for posting - FCT 21 RM Library Question 2 Will a full time Safety supervisor be required to be onsite at all times work is taking place? Answer 2 Please refer to project specification Section Owner Safety Requirement. Question 3 Per Sam Layegh at the prebid conference, please confirm that for all work that does not affect the occupants of the 20 th floor may be performed during normal working hours, 6am 6pm Monday Friday. If this is incorrect please provide the nominal working hours for the project. Answer 3 All work that does affect occupants and/or utilities to the 20 th floor or any other floors other than project construction area (Library), shall be performed after hours. Question 4 - Page D111 shows the existing partition / soffit where the ceiling transitions from 18-5 to 10-6 to be removed and page A120 shows the same partition to be reinstalled to the deck. What is the design intention behind this? Does the current partition not extend all the way to the structure above the ceiling and therefore needs to be extended. Refer the attached marked up pages. Answer 4 - Refer to the attached sheets D111 A5 Demolition RCP and A120 A5 Reflected Ceiling Plan for clarification. Revisions have been clouded and noted as 01/23/2017 Drawing Addendum #02. The gyp bd 10-6 AFF is existing to remain including the cove lighting Question 5 - Page A120 calls for the contractor to relocate the fixtures designated R1-R13 from the Open stacks FCT to the new Open Stacks Area FCT This presents an issue because FCT is in Phase 1 of the renovation. However, FCT is in Phase 2. Is it the intent that temporary fixtures be installed in FCT until the R1-R13 fixtures are ready to be relocated during Phase 2? If so, please provide a specification. Answer 5 - Refer to attached sheet G101 B5 Phase 1 Demolition / Construction Plan for clarification. Revisions have been clouded and noted as 01/23/2017 Drawing Addendum #02. Please note that fixtures R09 R13 are located in Phase 1 work area.

3 Question 6 - Will contractor be responsible for any Data scope other than conduit and backboxes? Answer 6 - Refer to the previously issued revised sheet E200, Drawing Addendum No. 1 for scope clarification. Question 7 - Will contractor be responsible for any Security scope other than conduit and backboxes? Answer 7 - Refer to the previously revised sheet E200, Drawing Addendum No. 1 for scope clarification. Question 8 - Wattstopper Lighting Controls and associated systems are no longer acceptable to be installed or used under MD Anderson Master Specifications. The existing lighting control system panel is WattStopper LCVR24S. Existing zoning and circuiting needs to be clarified as to what is existing. Notes 9,10, and 11 continually refer to new switching devices, new relays and adding additional circuits. Can the following be provided: accurate existing circuiting, new circuiting as required, new counts and types of LV relays with part numbers that will be required with new fixtures, new zones, Etc. Answer 8 - Remove Wattstopper Lighting Control system and its connections from the new renovation lighting scope. New lights shall be controlled via Eaton occupancy sensors and wall switches. Existing switches, circuits, and lights that are not to be reused shall be removed as indicated in the contract documents. Any light fixture not part of the renovation shall remain on the existing Wattstopper system Question 9 Phase 1 shows a partial replacement of the ceiling. It is MD Anderson s policy for heat detectors to be installed, heat detectors to be commissioned, and sprinkler heads to be turned up if the ceiling is to be removed. There are numerous inspections that must be passed before the ceiling can be installed and covered back up. Will MD Anderson require the ceiling area in phase one, in which the ceiling is called to remain, to be removed and sprinkler heads turned up in this area as well. Typically if part of a construction area has the ceiling removed and sprinkler heads turned up the entire construction area must have the ceiling removed and sprinkler heads turned up. Answer 9 Contractor is responsible to follow all MDACC safety policies. Contractor is also responsible to provide demo plan for EH&S inspector to review before removing any ceiling. Question 10 - Please advise if there are any activities not allowed during normal hours while inside the immediate construction area during each phase. Answer 10 - Refer to G101 C4 Schedule of Work and A3 General Notes for limitations on demolition / construction activities Question 11 - Please advise if all work outside of the immediate construction area for each phase will need to be after hours. Please advise as to what after hours consist of. Answer 11 - Refer to G101 C4 Schedule of Work and A3 General Notes for limitations on demolition / construction activities. After hours are defined as 6PM 6AM, 5 days per week M-F & weekends.

4 Question 12 - Please advise if handling or delivery of materials and equipment to the jobsite is restricted to a certain time frame. Answer 12 - Refer to G101 A3 General Notes Item R for elevator use. Question 13 Will contractor be responsible for any Commissioning other than coordination with Commissioning agent? Answer 13 Contractor is only responsible for coordination of work with the Commissioning agent. END OF REQUEST FOR INFORMATION
