29 Owner Information RSC Property Information QP Profile Information Certification Statement QP Certification Statement Owner Certification Statement Supporting Documents QP Certification Statement B As the qualified person, I certify that: * A phase one environmental site assessment of the RSC property, which includes the evaluation of the information gathered from a records review, site reconnaissance, interviews, a report and any updates required, has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation. A phase two environmental site assessment of the RSC property, which includes the evaluation of the information gathered from planning and conducting a site investigation, a report, and any updates required, has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation. The information represents the site conditions at the sampling points at the time of sampling only and the conditions between and beyond the sampling points may vary. As of 2013/04/18, in my opinion, based on the phase one environmental site assessment and the phase two environmental site assessment, and any confirmatory sampling, there is no evidence of any contaminants in the soil, ground water or sediment on, in or under the RSC property that would interfere with the type of property use to which the RSC property will be put, as specified in the RSC. Ground water sampling has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation As of 2013/04/18, in my opinion, based on the phase one and phase two environmental site assessments and any confirmatory sampling, the RSC property meets the applicable full depth background site condition standards prescribed by section 34 of the regulation for all contaminants prescribed by the regulation in relation to the type of property use for which this RSC is filed, except for those contaminants (if any) specified in this RSC at Table 2, Maximum Contaminant Concentrations Compared to Standards Specified in a Risk Assessment. As of 2013/04/18, the maximum known concentration of each contaminant in soil, sediment and ground water at the RSC property for which sampling and analysis has been performed is specified in this RSC at Table 1, Maximum Compared to Applicable Site Condition Standards. I am a qualified person and have the qualifications required by section 5 of the regulation. I have in place an insurance policy that satisfies the requirements of section 7 of the regulation. I acknowledge that the RSC will be submitted for filing in the Environmental Site Registry, that records of site condition that are filed in the Registry are available for examination by the public and that the Registry contains a notice advising users of the Registry who have dealings with any property to consider conducting their own due diligence with respect to the environmental condition of the property, in addition to reviewing information in the Registry. The opinions expressed in this RSC are engineering or scientific opinions made in accordance with generally accepted principles and practices as recognized by members of the environmental engineering or science profession or discipline practising at the same time and in the same or similar location. 50 Phase Two Environmental Site Assessment Proposed Residential Development 36 Hazelton Avenue, Toronto ON BRM C0 September 2013 Phase Two ESA Conceptual Site Model Introduction This section presents the Phase Two Conceptual Site Model (CSM) developed from observations made during the Phase Two ESA (exp, September 2013) of the Site. A CSM provides a narrative, graphical and tabulated description integrating information related to the site geologic and hydrogeologic conditions, areas of potential environmental concern or potentially contaminating activities, the presence and distribution of potential contaminants of concerns, contaminate fate and transport and potential exposure pathways, where applicable. These components are described in the following sections. The presentation of this Phase Two CSM makes reference to the following drawings, attached: Drawing 1: Site Location Plan Drawing 2: RSC Study Area and Adjacent Land Use Drawing 3a: Potentially Contaminating Activities Drawing 3b: Area of Potential Environmental Concern Drawing 4: Borehole Location Plan Drawing 5: Cross Section Key Map Drawing 6: Cross Section A-A Drawing 7: Cross Section B-B Drawing 8: Groundwater Contour Map Site Description, Features and History The RSC Property, hereinafter referred to as the Site, is located at 36 Hazelton Avenue (formerly 34 and 38 Hazelton Avenue) in the City of Toronto, Ontario. It is situated on the west side of Hazelton Avenue, approximately 30 m north of Scollard Street. The legal descriptions, municipal addresses, PINs and assessment rolls of the Site are as follows: 38 Hazelton Avenue: Lot 6, Plan 302 (Yorkville). City of Toronto, PIN , Assessment Roll No Hazelton Avenue: Lot 7, Plan 302 (Yorkville). City of Toronto, PIN , Assessment Roll No The Site is rectangular in shape and covers an area of 1,540 m 2. At the time of this investigation, the Site was under construction for the proposed residential development on Site. No permanent structures were present on Site. The majority of the Site, with the exception of the eastern extent along Hazelton Avenue, was generally excavated to a depth of approximately 7.6 m below the original or adjacent road grade as part of the bulk excavation activities that were on-going at the Site. The brick façade of the former structure remains on Site along the east end of the excavation, which would be incorporated into the proposed building. 1

51 Phase Two Environmental Site Assessment Proposed Residential Development 36 Hazelton Avenue, Toronto ON BRM C0 September 2013 The Phase Two Property is currently located in a mixed commercial and residential neighborhood in the City of Toronto. The Site was initially developed prior to 1880s for residential use. The property was then redeveloped with a school building, known as the St. Basil s School, in approximately the late 1920s and occupied by the Metropolitan Separate School Board for institutional use until the late 1990s. The Site building was then briefly utilized as commercial offices and an alternative learning centre until approximately the early 2000s, when it was left vacant until demolition at the beginning of the current construction activities. The former structure was a three (3) storey school building with one (1) basement level located at the eastern portion of the Site. The topography of the Site is relatively flat in general. Surface water in the vicinity of the Phase Two Property is collected catch-basins that are connected to the municipal storm water sewer system. The Site is bound by Hazelton Avenue to the east, commercial land use to the south and west, as well as residential land use to the north. Site Conditions The Site is located in an urban and developed area of the City of Toronto where all properties within a 250 m radius of the Site are serviced by a municipal water source. The Site is under redevelopment at the time of this Phase Two ESA and the majority of the Site had been excavated to approximately 7.6 m below original grade as part of the bulk excavation activities. The eastern (i.e., front) brick façade of the former Site building is preserved at the east end of the Site. A small paved asphalt driveway is located at the northeast corner of the Site. There are no water bodies on the Site. The nearest surface water body to the Site is Lake Ontario, located approximately 3.7 km south of the Site. Based on investigations conducted for the Site, the depth to groundwater in the Phase Two Property was measured to range from 14.5 to 15.0 m below original or adjacent road grade. The Site is not located in close proximity to an area of natural significance (ANSI). No domestic drinking water wells were identified within the Phase One Study Area. The Site is not an environmentally sensitive area in accordance with Section 41 of the regulation. The Site is not within an area of natural significance; it does not include and is not adjacent to an area of natural significance; and, it does not include land that is within 30 metres of an area of natural significance. The Site is not a shallow soil property as defined in Section 43.1 of the Regulation, nor does it include all or part of a water body or is adjacent to a water body or includes land that is within 30 metres of a water body. The nearest natural water body in the vicinity of the Site is Lake Ontario, located approximately 3.7 km south of the Site. 2

52 Phase Two Environmental Site Assessment Proposed Residential Development 36 Hazelton Avenue, Toronto ON BRM C0 September 2013 Potentially Contaminating Activities Based on the findings of the Phase One ESA, all of the potentially contaminating activities (PCAs) identified within the Phase One Study Area are illustrated in Drawing No. 3a of this CSM. Based on the conclusions of the Phase One ESA, the PCAs that were considered to present an Area of Potential Environmental Concern to the Site are presented below: PCA 1 #28 Gasoline and associated products in fixed tanks: The presence of a former underground storage tank (UST) for fuel oil storage located at the northeast portion of the Site. The UST was removed from the Site in the 1990s. PCA 3 - #10 Commercial auto body shop; #52 Storage, maintenance, fueling and repair equipment, vehicles and materials used to maintain transportation systems; and #28 Gasoline and associated products in fixed tanks: The present of a former automobile repair and refuelling facility west adjacent to the north end of the Site. Area of Potential Environmental Concern The following Areas of Potential Environmental Concern (APECs) were identified: APEC 1: The presence of a former underground storage tank (UST) for fuel oil storage located at the northeast portion of the Site (i.e., PCA 1). In April 2013, five (5) boreholes were advanced at the APECs on Site to depths ranging from 6.6 to 9.6 m below existing grade at the time of drilling (or approximately 6.6 to 18.5 m below original or adjacent road grade), with geological conditions interpreted accordingly. Three (3) of the five (5) boreholes were advances within the bulk excavation area. Refer to Drawing No. 4 for an illustration of the borehole locations. The general soil stratigraphy encountered at the Site, as interpreted from the recovered soil samples, generally comprised surficial concrete slab or fill over native deposits of silty sand, silty clay and sandy silt. No apparent evidence of environmental impacts, such as odour or staining, was noted in the soil samples recovered from the boreholes advanced on Site. The depth of bedrock was determined to be in excess of 18.5 m below original or adjacent road grade at the Site (~Elev m), as bedrock was not encountered within the maximum investigated depths of this Phase Two ESA. The depths to groundwater within the overburden unit in the Phase Two Property measured over the course of this Phase Two ESA ranged from 5.26 to 6.84 m below existing grade (or approximately 14.2 to 14.6 m below original or adjacent road grade), with the corresponding geodetic groundwater elevations from approximately Elev to m. Given the significant depth of the groundwater table at the 4

54 Phase Two Environmental Site Assessment Proposed Residential Development 36 Hazelton Avenue, Toronto ON BRM C0 September 2013 Site, the likelihood of adverse impacts to groundwater is considered to be low in general. The groundwater flow direction, based on groundwater level measurements, was interpreted to be towards the northeast in general. The horizontal hydraulic gradient was calculated to range from 0.08 to m/m, with a geometric hydraulic gradient of m/m. No evidence of free product was detected during water level measurements. No apparent hydrocarbon or chemical odours were noted in the water samples collected from the Phase Two Property. The water table at the Phase Two Property is under unconfined conditions and is subject to seasonal fluctuation. However, it is not expected that the seasonal fluctuations in groundwater levels will result in significant alterations in the direction of groundwater flow. Refer to Drawings Nos. 6 and 7 for geologic cross sections at the Site. The direction of interpreted groundwater flow is illustrated in Drawing No. 8. Summary of Phase Two ESA and Contaminants of Concern (COC) For the purpose of this Phase Two CSM, contaminants of concern (COC) were identified by screening analytical results reported for the soil and groundwater samples collected from the RSC Property against the Table 1 Site Condition Standards (SCS), Full Depth Background Site Condition Standards for residential/parkland/institutional/industrial/commercial/community property use and coarse textured soil, as presented in Soil, Ground Water and Sediment Standards for Use Under Part XV.1 of the Environmental Protection Act, dated April 15, The objective of the Phase Two ESA was to assess the subsurface soil and groundwater conditions at the APECs identified. To address the potential contaminants of concern (PCOC) of the Site based on the Phase One ESA, the analytical parameters included in this Phase Two ESA consisted of volatile organic compounds (VOCs), benzene, toluene, ethylbenzene, xylenes (BTEX) and/or petroleum hydrocarbons (PHC) F1 to F4. Selected soil and/or groundwater samples were analyzed for these parameters according to the potential contaminants identified. The concentrations of VOCs, BTEX and PHC (F1 to F4) in the soil samples analyzed in this Phase Two ESA were below the Table 1 SCS for residential/parkland/institutional/industrial/commercial/community property use for coarse textured soils. The concentrations of VOCs, BTEX and PHC (F1 to F4) in the groundwater samples analyzed in this Phase Two ESA were below the Table 1 SCS for all types of property uses for coarse textured soils. Based on the findings of this Phase Two ESA, no contaminants of concern (COC) were identified in the soil or groundwater at the Phase Two Property. 5

55 Phase Two Environmental Site Assessment Proposed Residential Development 36 Hazelton Avenue, Toronto ON BRM C0 September 2013 Contaminant Fate and Transport, Human Health & Ecological Receptors, Exposure Pathways As no COCs were identified on Site, there are no date and transport mechanisms, no human health or ecological exposure pathways, and no issues relative to vapour intrusion at the Phase Two and RSC Property. Underground Utility The only underground utility currently identified on Site was a gas service at the northeast corner of the Site. As no COCs were identified based on this Phase Two ESA, no potential effects to the distribution and migration of contaminates were identified. Proposed Development on Site The construction of a multi-storey residential structure has been proposed on Site. The proposed building will consists of three (3) underground parking levels, which would cover the entire property to the west of the brick façade of the former building on Site. The proposed development will be serviced with the municipal water system. 6
