1 Grampians Partners in Recovery The Grampians Partners in Recovery (GPIR) Support Facilitator role is suited to an experienced professional with a background in working with client groups with complex needs and mental health issues, and a commitment to innovation and systems change that results in better outcomes for this target group. Support facilitators will lead partnership and capacity building effort in a sub-region of the Grampians Medicare Local catchment, and work directly with providers across sectors to achieve better coordinated responses to GPIR clients. GPIR Support Facilitators located across the catchment work as part of a dynamic interagency team reporting to the GPIR Program Manager and local agencies. Position details Positions available and employment arrangements Several Support Facilitator (SF) positions are available with GPIR and will be employed by GPIR member agencies located across the region. Each SF role will have particular sub-catchment responsibilities. Interested applicants are invited to apply for one or more of the positions. The position is offered on a contract basis to 30 June 2016 subject to satisfactory performance during a three-month probationary period as per the dates specified in the Employment Schedule provided by the employer. Full time positions are expected to be worked over five days; the days of work for the Central Goldfields position are negotiable. Specific hours are reflected in the Employment Schedule provided by the employer. The remuneration for the positions includes: commencement salary up to $70,000 or pro rata salary packaging provisions as offered by the employer. The position has a dual reporting line to: the GPIR Program Manager employer agency Manager with responsibility for GPIR Kathy Wilson at Kathy Wilson Consulting

The application and accompanying detail should be ed to: Applications may be lodged any time from Weds 21 August up until 9.30am Mon 2 September The GPIR catchment incorporates four sub-catchments: three rural Central Goldfields, Grampians and Wimmera and one, Central Highlands, which combines urban fringe, rural towns and the regional centre of Ballarat. Within this dispersed catchment there are high levels of socio-economic disadvantage, lower levels of educational attainment and significant numbers of residents experiencing poor physical and mental health when compared with other parts of regional Victoria and Victoria as a whole. Support facilitation positions, while located in a sub-regional catchment, work as part of the regional GPIR team to collaboratively achieve better outcomes for clients and sustainably build service system capacity. Each position is expected to develop robust program linkages both within and external to the employing agency. Service system innovation proactive identification of opportunities for building operational level partnerships between agencies in the sub-region to better facilitate the coordination of care to individuals and their families facilitating and engaging in complex negotiations with key stakeholders in order to develop systems and processes that achieve better service integration and coordinated care for clients in the sub-region and across the region participating in planning for, and organisation of, GPIR regional stakeholder events and contributing to the GPIR Annual Service System Development Plan leading the development of a robust sub-regional GPIR network and providing it with support to develop annual priorities linked to local service gaps and issues; ensuring the plan is aligned with the GPIR Service System Development Plan building reputation and profile sub-regionally and at agency levels ensuring GPIR communications are consistent, well-targeted and appropriate to diverse audiences. continuing the process of needs assessment and opportunity analysis in the sub-region supporting the design and implementation of sustainable local and regional service system innovations. 2

GPIR Financial risk and compliance management implementing local processes for identifying/engaging GPIR target clients collaboratively developing Coordinated Action Plans (CAPs) for GPIR clients and monitoring implementation and review of those plans engaging with service providers to facilitate and support their involvement in developing and implementing CAPs working with providers in a manner that builds sustainable capacity to provide services and supports to clients ensuring consumers and carers are adequately and appropriately engaged in informing and in decision-making in respect to CAPs undertaking, when appropriate, situational assessment to determine risks and challenges, and identify appropriate responses for GPIR clients ensuring full records of all CAPs and associated review processes are maintained for selected clients, on a time limited basis, providing case management support and ensuring that the role is appropriately transferred to a local provider. providing day-to-day management of the local Support Liaison and Information Officer actively and constructively participating in the GPIR interagency team, including in quality assurance activities, coordination and planning processes, and team building operationalising the GPIR intake and allocation systems (including the 1300 number) in the sub-region participating in agreed activities and meetings within the employing agency implementing local business systems and processes that enable the common GPIR teambased approach managing brokerage funds locally implementing systems and protocols to ensure reporting and evaluative data is collected and communicated in a timely and appropriate manner fully engaging in GPIR evaluation and learning activities. working with provider agencies in a manner that builds sustainable capacity to provide services and supports to clients in the GPIR target group, including through the provision of training and workforce development initiatives providing secondary consultation and support to service providers identified in the CAP negotiating additional service provision as required including through limited term brokerage arrangements. managing locally allocated flexible funding and any related sub-contracts in line with the budget and program guidelines liaising with the agency to ensure GPIR, funder and lead agency compliance and reporting requirements are built into local business processes and accurate reports are provided to the Program Manager on time supporting the implementation and assuring the accountability of GPIR short-term projects conducted in the sub-region ensuring GPIR operational protocols are implemented locally in a manner that is fully compliant with relevant legislation including in respect to client rights and records, and privacy. 3
