Use population approaches, data and other evidence based strategies to assist with planning and delivery of service improvement projects (where required) KEY TASKS EXPECTED OUTCOMES Project Management Project manage specific projects including working with the Funding and the Health Outcomes Teams, Clinical teams and members, providers and stakeholders Waitemata District Health Board -JOB DESCRIPTION - Page 1
2 KEY TASKS Develop, understand and actively manage DHB stakeholder relationship Close liaison with Funding and Health Outcomes Team EXPECTED OUTCOMES Provide detailed project plans Provide regular reports on progress of the projects to the Funding & Development Manager / Senior Programme Manager Liaise with stakeholders and project team members and regional partners to ensure the consistency of project development and implementation and regional agreed locality based health care intent Write relevant board reports and Ministry of Health reports as required including CPHAC reports Monitor and evaluate project delivery against required project outcomes and initiate progress reviews as appropriate Identify and maintain relationship with key Health Services stakeholders within the district, regional and national levels to support the project(s) Involve key stakeholders in project initiatives as appropriate Actively work with Maori and Pacific providers and stakeholders as appropriate Represent ADHB Waitemata DHB position with clinical and other stakeholders (including other DHBs) Contribute to Service development projects as required Reducing inequalities and population health gain Ensure all projects have a focus on reducing health inequalities and lead to measurable health gain Strategic and business planning process Provide input, report and analysis for the development of Auckland and Waitemata DHB's strategic and annual plans and Statement of Intent regarding your projects Positive and constructive relationships Working in close collaboration with the other service Auckland DHB Waitemata DHB staff Statutory & Treaty of Waitangi obligations Stakeholders have a known point of contact within Auckland DHB Waitemata DHB Ensures the professional and political integrity of WDHB by carrying out all functions in compliance of the Treaty of Waitangi and by demonstrating a serious commitment to keeping the treaty alive.
