The Department utilizes Bid Express ( as the official medium for electronic bid submittal. All bidders must prepare bids and submit them online via Bid Express. Addenda amendments must be acknowledged in the miscellaneous section of the Expedite (EBS) file and all amendments loaded in order for your bid to be considered for award of this project. Bid express will not accept bids that do not have amendments incorporated. 2 Proposal Addendum For SUM ; PID Project Completion Date Change: No Bid Item Changes, Additions or Deletions: No Please be advised of the following: In the Scope of Services Section 6 replace Sta ± with Sta ± In the Scope of Services Section 6 replace 4700± ft with 5325± ft Replace Section a.in the Scope of Services with the following: a. Remove the existing Guardrail and replace with Single Slope Barrier Type D (Standard Construction Drawing RM-4.5) at the same offset from the Lane Line as the Guardrail (include a 4 PVC Raceway as per Standard Drawing RM-4.3, the Raceway will be installed inside the Single Slope Barrier and not mounted on the outside of the barrier) Replace Section 15.2.B.2.c and Section 15.3.B.2.c in the Scope of Services with the following: c. The color of the highway side of this noise barrier is to be Brick Red (Federal Color Number 20109). The color of the residential side of the noise barrier will be light gray (Federal Color Number Light Gray). The color of the Support Posts will be Buff (Federal Color Number 13522) In the Scope of Services Section a replace Sta with Sta Replace Section b in the Scope of Services with the following: b. Refer to Section 15 for the Noise Barrier Requirements for Structure SUM B. A physical connection between the Ground Mounted and Structure Mounted Noise Barrier is not required but there must be an overlap of the barriers such that there is not a gap as viewed from the highway. Replace Section e in the Scope of Services with the following: e. The Concrete Noise Barrier Panels will be a form lined Brick matching Architectural Polymers #111 Ohio Sand Brick or Approved Equal on the Highway Side. The Concrete Noise Barrier Panels will be a form lined Ashlar Stone matching Architectural Polymers #905 Small Aged Ashlar Stone or Approved Equal on the residential side. Replace Section f in the Scope of Services with the following: f. The Panels will be Sealed/Coated with a color matching Brick Red (Federal Color Number 20109). This will be performed on the Highway Side only. No color will be applied on the residential side of the noise barrier panels. The residential sides of the noise barrier panels will be sealed in accordance with current Noise Barrier Specifications.

3 Replace Section g in the Scope of Services with the following: g. The Panel Caps (Coping), Finials [The finials will be attached to the top of the concrete posts at 250± foot intervals and at the beginning and end of the proposed concrete noise wall. The proposed finial design shall match the existing finials attached to the top of the concrete posts at various intervals along the existing concrete noise walls at the IR 271/SR 8 interchange; SUM ; PID 24508; Plan Sheet No and 1342] and Posts will be sealed/coated with a color matching Buff (Federal Color Number 13552) Replace Section d.iii in the Scope of Services with the following: iii. The color of the highway side of this noise barrier is to be Brick Red (Federal Color Number 20109). The color of the residential side of the noise barrier will be light gray (Federal Color Number Light Gray). The color of the Support Posts will be Buff (Federal Color Number 13522) Replace the paragraph in Section 16.2 of the Scope of Services that begins with All signs will be placed between Noise Barrier and the Roadway with the following All signs will be placed between Noise Barrier and the Roadway, no sign supports will be permitted to be placed on the outside (Residential Side) of the Noise Barrier. All affected Flat Sheet Signs and supports will be removed, disposed of and replaced. Extrusheet Signs and supports may be left in place and the Noise Barrier alignment can go around them or they can be removed, disposed of and replaced. Replace Attachment B with the attached report entitled Revised Noise Wall Foundation Exploration Report SUM-271/8 Noise Wall prepared by S&ME, revised September 2013 Add the following to Section 10 of the Scope of Services: BEST MANAGEMENT PRACTICES/SOIL EROSION AND SEDIMENTATION CONTROL WATER COLUMN AND SEDIMENTATION IMPACTS WILL BE KEPT TO A MINIMUM THROUGH THE USE OF BEST MANAGEMENT PRACTICES FOR SOIL EROSION AND SEDIMENTATION CONTROL. ALL SOIL EROSION AND SEDIMENT CONTROL MEASURES WILL BE IN PLACE PRIOR TO ANY EXCAVATION, GRADING OR FILLING OPERATIONS AND INSTALLATION OF PROPOSED STRUCTURES OR UTILITIES. THEY WILL REMAIN IN PLACE UNTIL CONSTRUCTION IS COMPLETED AND THE AREA IS STABILIZED AS ACCEPTED BY THE ENGINEER. STREAM AVOIDANCE UNDER NO CIRCUMSTANCES WILL THE CONTRACTOR IMPACT BRANDYWINE CREEK. NO EXCAVATION, GRADING OR FILLING OPERATIONS WILL BE PERFORMED IN BRANDYWINE CREEK. UNDER NO CIRCUMSTANCES WILL THE CONTRACTOR STORE CONSTRUCTION EQUIPMENT AND/OR MATERIALS IN BRANDYWINE CREEK.

4 WETLANDS AVOIDANCE UNDER NO CIRCUMSTANCES WILL THE CONTRACTOR IMPACT THE WETLANDS LOCATED WITHIN THE PROJECT AREA. NO EXCAVATION, GRADING OR FILLING OPERATIONS WILL BE PERFORMED IN THESE WETLANDS. UNDER NO CIRCUMSTANCES WILL THE CONTRACTOR STORE CONSTRUCTION EQUIPMENT AND/OR MATERIALS IN THESE WETLANDS. STAGING AREAS ODOT CONSTRUCTION AND MATERIALS SPECIFICATIONS SECTION (PROTECTION AND RESTORATION OF PROPERTY) PROHIBITS THE CONTRACTOR FROM CREATING STAGING AREAS NEAR STREAMS/WETLANDS. AREAS DISTURBED BY EQUIPMENT ACTIVITIES ANY AREAS DISTURBED BY EQUIPMENT ACTIVITIES MUST BE SEEDED WITH NATIVE PLANT SPECIES AND MULCHED TO PREVENT EROSION OF SEDIMENTS INTO WATERS OF THE UNITED STATES. CONSTRUCTION AND DEMOLITION DEBRIS THE CONTRACTOR WILL TAKE PRECAUTIONS TO AVOID AND/OR LIMIT CONSTRUCTION AND DEMOLITION DEBRIS FROM ENTERING THE STREAM(S). ANY DEBRIS THAT DOES FALL INTO THE STREAM(S) WILL BE REMOVED AS SOON AS POSSIBLE. STRUCTURE PAINTING/CONCRETE SEALING OPERATIONS THE CONTRACTOR WILL TAKE ALL PRECAUTIONS NECESSARY TO PREVENT EPOXY-URETHANE SEALER, PAINT OR OTHER MATERIALS USED TO REPAIR, CLEAN, PAINT, SEAL OR TREAT ANY STRUCTURE FROM ENTERING ANY STREAMS, WETLANDS OR OTHER WATERS OF THE UNITED STATES AND TAKE THE APPROPRIATE ACTIONS IN THE EVENT OF A RELEASE.
