What we Do We understand that no two people are the same. That s why at Merrimu, everything we do is about providing the people we support with choice. Merrimu supports Victorians from 18+ years with a physical, sensory or intellectual disability including those with complex support needs, acquired injuries or autism. We also provide transitional support to school leavers. For 40 years, we ve been working with adults, their families and their support people to achieve what is important to them. We help people access services that provide them with their choice of experiences, including: building life skills vocation and education participation connecting with community recreation and social opportunities. Through planning, facilitation and mentoring, we ensure people have choice about the things they need to reach their goals. We plan and facilitate programs in group or individual settings, and continue to provide ongoing mentoring to ensure the program is working well. We also build strong community partnerships to help people access the things they need. And not only do we create a tailor-made program that s unique for each person, we also help them get to where they need to be with our SmartRun transport service. Version 1 Page 1 of 5 June, 2014

2 General Summary Merrimu is currently undergoing a period of significant development, to deliver disability support to a broader geographical area. Our focus is on providing a wider range of people with a disability with a wider range of serviceswhen, where and how they choose them. Facilitation is a vital role and our key differentiation of service delivery. The role will be responsible for planning and facilitation to services in conjunction with our hubs and centers, ongoing accountability of reports and intake of individuals in line with the Services Manager. The Position is located in Merrimu s Bacchus Marsh Office, and will be responsible for assisting the Services Manager and the Executive Team with the facilitation process and sustaining and developing new and current service support initiatives by achieving outcomes in line with each person we support s plans and achieving the maximum KPI s for the position. Position Summary Functional Role Liaison with key stakeholders Working with the Services Manager and as an integral part of the organisation s leadership team the facilitation role is part of the key differentiation strategy which will assist in the organisation s development in Victoria. The will lead all facilitation practices for individuals accessing the centres and the community. 1. Establish contact and build relationships with the person and where possible, their network 2. Communicate effectively and openly with everyone involved in the person s life 2.1. Ensure that all communication meets individual s communication requirement 2.2. Ensure that communication includes and is relevant to stakeholders 3. Develop plans with individuals and their support network 3.1. Uphold the ongoing review process that ensures that a plan always remains active and real for the person 3.2. Implementation of an Individual s Plans to ensure that the support and service options in each individual s plan are established 4. Effectively manage activities to meet the availability of stakeholders, and requirements of the program in a timely manner 4.1. This may require flexibility and out of business hours availability 5. Participate in allocated steps of the intake procedure 6. Contribute to the positive culture of the organisation, including all staff and the Leadership team 7. As part of the Leadership Team manage the ongoing accreditation and compliance of all Services 8.1. Collect data required for reporting and evaluation purposes 8.2. Meet program and organisational accountability requirements 9. Manage appropriate systems, communication channels and reporting mechanisms to enable good coordination 9.1. Develop and present reports to Merrimu as required 10. Enhance Merrimu s complaints process for the new business unit to ensure capturing of complaints at an early stage Contribute to development of an internal reporting process for complaints This role will establish and maintain strong working relationships with the people we support, their families, funders, Services Manager and the Executive Team. 3 Key Selection Criteria Selection will be based on assessing an applicant s skills, knowledge, behaviours and experience, and any other personal qualities relevant to the position as listed below. The will have a willingness to continually expand the focus of the role as Merrimu s Network of services develops. The successful applicant will thoroughly enjoy the challenge of strategic issues, at the same time as being able to pay attention to the detail required in developing and enhancing relationships and drawing new support programs into an operational setting. The role will ideally suit a person who is looking for a position where a wide range of skills is required and where there is an opportunity to develop further skills in person centered practice, evidence of outcomes, ongoing relationship building and documentation accountability. A large component of this position involves relating to people, with frequent interactions with a variety of people from diverse backgrounds. Experience Sound working knowledge of good practices, reporting, recourse planning and implementation, customer networking, information technologies. Skill, Knowledge and Abilities Ability to lead, influence and motivate people, to create and implement plans and review processes. Must have working knowledge of computers and software applications commensurate with the Position, including but not limited to spread sheets. Strong interpersonal, written and analytical skills are required. Comfortable with ambiguity. Sound reasoning skills. Capacity to work independently, prioritise responsibilities and work closely with others. People Skills Interpersonal skills necessary to help people reach their highest potential and achieve personal goals. Must have the ability to work with a large scope of individuals and networks. Intuitive The ability to view the long-range trends and cycles of the organisation. This position also requires the capacity to understand, pro-actively use and develop management reports. Construct proposals to Services Manager about best practices relevant to the role. Working conditions Office environment. Regular travel throughout the state of Victoria, as appropriate: visiting customers, families, community organisations, Funders, Peak Bodies, service organisations, and the like. Job Profile Key responsibilities of this role include: In the context of a developing organisation the role is responsible for the development and review of new and effective plans with the people we support and to promote and assist the development of new program support services. The will assist the organisation to operationalize in a number of Hub s in selected geographical areas. Foster and promote individual outcomes. The Facilitation coordinator will facilitate the development of and maintenance of network relationships, working closely with other relevant roles to achieve required development timeframes and service support outcomes. Maintain the external relationships consistent with the goals, philosophies and desired image of the organisation.
