The Cntract Dcuments fr the abve referenced prject are amended and revised as fllws: QUESTIONS BY POTENTIAL BIDDERS: Questins: What is ADH SET REBAR n detail A/7? Answer: ADH is the abbreviatin fr Adhesive. Reinfrcing bar set in adhesive anchr per specificatin sectin Questins: Where is the right f way lcated arund the statin? Answer: 34 f cnstructin access will be available n nrth east and suth west sides f the building. Questins: Is the crack n the inside between the flr and wall t large fr epxy injectin? Shuld epxy grut be used instead? Answer: Start with epxy injectin fr thin/deep cracks. Once the crack pens t a width greater than ¼, mix epxy resin with dry silica sand at a 1:1 rati as recmmended by resin manufacturer t finish filling the crack. Questins: Wh is respnsible fr mving the verhead lines fr allwing the drill rig t access the suth end? If the cntractr is respnsible will Duke prvide a cst estimate t the bidders? Answer: MSD will be respnsible fr payment fr mving f verhead lines fr drill rig access. Cntractr shall crdinate and schedule wrk with utility cmpanies. See Addendum Item 6. Questins: Why is the suth wall required t have a water repellent when the entire wall needs new mrtar in jints? Only a small sectin is requiring new mrtar. Page 1 f 3

Caulk cracked jints befre painting. See Addendum Item 5. Questins: Shuld the bidders bid under the assumptin that all cnduits need replaced r let this be a call in the future and give MSD cst additin t the prject? Answer: Cnduits d nt need t be replaced. Lsen cnnectin at panels/bxes then retighten after relieving any stresses that have been caused by the building mvement. Questins: Hw lng will the wet well be drained in rder t paint the piping? Clarify what is painted: piping, gates, and supprt steel that is nt stainless? Answer: Wet well will nt be drained. Only painting f the crrsin areas n the piping abve the flr line is required. Questins: Will a cntingency be required? Answer: Cntingency Bid Item is being added. See Addendum Item 2. Questins: Shuld the strip f cncrete be left n the east and suth side r shuld all f the cncrete be replaced? Please clarify fr bidding? CONTRACTOR SHALL COORDINATE THE DROPPING OF THE SERVICES WITH THE UTILITY COMPANIES FOR THE MOBILIZATION AND DEMOBILIZATION OF THE DRILLED SHAFT DRILLING RIG. SERVICE OUTINGS SHALL BE MINIMIZED AND LAST NO LONGER THAN 2 HOURS. SERVICE DROP CHARGES BY THE UTILITY COMPANIES SHALL BE PAID BY MSD THROUGH THE CONTRACT CONTINGENCY ITEM. 5 Cleves Pump Statin Stabilizatin Prject ID: Trade: General This prpsal requires the bidder t prvide bth "UNIT PRICES" and a "BID ITEM TOTAL." In the apprpriate clumns, bidder shall enter price per unit fr labr and materials fr every bid item. Therefre, if the ESTIMATED QUANTITY f the bid item is mre than ne, e.g. 25, the unit price is ne f this amunt, e.g. 1/25th. The "COMBINED UNIT PRICE" is the sum f "LABOR" and "MATERIALS" as unit prices. The "BID ITEM TOTAL" is the sum f the "COMBINED UNIT PRICE" times the "ESTIMATED QUANTITY" fr EACH item.
